4 Meeting bjectives and pening remarks The USAID funded prject n Scaling up the delivery f Infectin and Treatment Methd (ITM) in Tanzania thrugh facilitatin f the ITM value chain, abbreviated ITM2SCALE, held its annual review meeting n Octber at the Oceanic Bay Htel in Bagmy, Tanzania. The meeting brught tgether 33 stakehlders and partners including East Cast fever (ECF) vaccine distributrs, livestck farmers, gvernment agencies such as the Veterinary Cuncil f Tanzania, the Livestck Training Agency, the Tanzania Veterinary Labratry Agency, and scientists frm the Internatinal Livestck Research Institute (ILRI) and the Glbal Alliance fr Veterinary Medicine (GALVmed). The purpse f the meeting was t review achievements, discuss challenges, pprtunities and plans fr the next year. The meeting als prvided an pprtunity fr stakehlders t review its implementatin cycle and plans, and make adjustments as required Welcme remarks The Directr f Veterinary Services in Tanzania, Dr. Abdu Hayghaim, expressed his appreciatin fr the ITM2SCALE prject interventins, emphasizing that the unique nature f ECF vaccine requires expertise t administer. Dr. Hayghaim als infrmed participants that acaricide use in Tanzania is currently being reassessed thrugh an nging study that is lking at impacts f the gvernment subsidy prgram n acaricides cst t establish the extent t which this interventin has reduced ECF incidences and benefited farmers? They hpe t review the mdalities f acaricide subsidy and make the necessary adjustments.prject bjectives and implementatin plans Henry Kiara f ILRI reminded participants f the prject s bjectives which had nt deviated frm the riginal plans. He then review the extent t which bjectives had been achieved. The cnclusin was, while cnsiderable prgress had been made, there were challenges that made achievement f sme bjectives difficult. The aim f the meeting was t review these and incrprate the lessns learned int the planning fr the final year f the prject. (See presentatin here and here). In her presentatin f the prject s thery f change (TC- available here), Isabelle Baltenweck f ILLRI reiterated that ITM2SCALE s bjective is t have mre farmers vaccinating their animals, thereby leading t imprved livestck prductivity and increase farmers incme. Baltenweck emphasized that the TC is imprtant in explaining and utlining hw this change (imprved prductivity and increased incme) will happen. Baltenweck tk the participants thrugh an exercise f reviewing the TC, by fcusing n the nine anticipated shrt term changes as a result f the prject s interventins. Participants were divided int grups t discuss what is wrking well, what can wrk better and the pprtunities that can be explited. 4

7 Cmments and emerging issues: Participants highlighted that there is need t find a way f sharing lessns and pprtunities fr crsslearning between distributrs Change n. 2 (there is sufficient demand fr livestck t absrb the increase in ff-take rates) was nt discussed by any grup; the participants mentined that the issues f service quality are being addressed thrugh the trainings. Hwever, it als emerged that the vaccinatrs d nt have their mindsets tuned twards seeing this as a business Participants nted that there is verlap between changes n 3 (key players will nt change) and 4 (cnducive plicy envirnment). Change n 6 (there will be limited negative envirnmental impact) was nt addressed because there seems t be an agreement amng the stakehlders regarding the training curriculum. Change n 7 (ptential vaccinatrs willing t cst-share the training cst) was nt discussed by any f the grups because it is wrking well. A participant nted that relatinships between stakehlders has significantly imprved: ITM2SCALE has imprved wrking relatinships between institutins including the DVS, LITA and VCT wh are nw able t sit tgether and each actr nw knws what the ther des. Said Hamza Mwamhehe, frm the Veterinary Cuncil f Tanzania Regarding change n 8, it was highlighted that ILRI is nt registered at as a licensed vaccine manufacturing institute, but a research institute; hence, there is need fr strng advcacy and lbbying t facilitate the registratin f the CTTBD vaccine in Tanzania Reprts frm partners Distributrs Since its inceptin, ITM2SCALE in cllabratin with GALVmed has facilitated the entry f three new ECF vaccine distributrs (Vetlife Cnsultants (T) Ltd, Pharmavax Ltd, Dulle Veterinary centre), in additin t the existing tw distributrs (Rnheam and Vetaggr). Fur f these distributrs -Rnheam, Vetlife, Pharamavax and Dulle Veterinary Centre participated in this review meeting. Their presentatins are available via the links belw: Discussin pints frm the presentatins Regarding training csts, it emerged that ptential vaccinatrs were nt willing t share the cst f training. This is cntrary t their views during cnsultatins with vaccinatrs where they expressed willingness t cntribute t their training. A participant nted that thse eligible t train (academic qualificatin) are nt willing t pay, and many f thse willing t pay are nt eligible fr training. Fr instance, it was highlighted that f the 160 vaccinatrs that have been trained by Rnheam, nly 30 f them are active. Key players in ITM will nt change, hence fcus shuld be n the institutins rather than individuals It emerged that the distributrs have t pay taxes twice fr the ear tags 7

8 Cmmunity versight shuld be encuraged- distributrs need t demnstrate the vaccinatin prcess t arrest fraudulent activities Activity reprts shuld be shared with gvernment actrs t enable cnslidatin f reprts n number f animals vaccinated The issue f the DVO s rle in recruitment f trainees was extensively discussed and participants agreed that training shuld be pened fr all thse willing and able t participate, nting that distributrs will have a strategy f targeting peple wh will seize business pprtunities The distributrs mentined that it is nt easy fr them t get animals fr practical training The distributrs als raised a cncern n vaccine packaging, emphasizing the need t cmmunicate changes in packaging t enable them effectively cmmunicate the same t vaccinatrs. Fr example, it was mentined that the new vaccine package with ne-end seal instead f ne with tw-end seal had causing cnfusin amng the vaccinatrs. It was agreed CTTBD wuld cmmunicate any future changes in packaging t the Lcal Technical Representative (LTR) The distributrs als cited difficulty in getting qualified vaccinatrs frm the private sectr It emerged that while the mst successful animal health prfessinals in AI (resulting in high cnceptin rates) are wmen, recruitment f wmen vaccinatrs is nt easy Timely access t the vaccine was cited as a key factr that hindered their perfrmance The distributrs mentined that it was very challenging t maintain the cld chain in the field Delayed prcessing and issuing f certificates upn cmpletin f training was als highlighted as a key challenge. This was explained by VCT that it was due t the time it tk t verify if the trainees were registered and retained by the Cuncil There was a high demand fr a lwer dse package. GALVmed cnfirmed that plans are underway t avail a 10-dse package f the ECF vaccine Distributrs mentined that they need access t ECF training manuals t facilitate their training activities High csts f liquid nitrgen as well as transprtatin f the same was cited as a majr challenge The meeting established that there are already plans t engage sub-distributrs t facilitate increased uptake f ITM There is a new training prgram by SUA which will target fresh graduates. Training ECF vaccinatrs culd take advantage f this. Training services shuld be demand driven, and are part f the cntinuus prfessinal develpment (CPD) fr animal health service prviders (AHSP) Prject indicatrs Nils Teufel f ILRI presented n the indicatrs used t track achievements (see presentatin here) Participants nted that n the vaccinatin recrd frm, there is need t include a clumn n time f recnstitutin and amunt f OTC injected. They als recmmended that a clumn n gender f wner f the livestck shuld be included. Participants further nted that there is need t crrect the training manual t reflect crrect time perid within which the vaccinatin must be dne frm time f 1 st vaccinatin (amend t read 4 hurs instead f 6 hurs) Baseline survey f vaccinatrs Esther Kihr presented the findings frm the baseline survey cnducted in December 2015 (see presentatin here) 8

9 Participants nted that the vaccinatin perid was at peak between January and July, the calving perid. It was highlighted that vaccinatin trends are highly influenced by availability f vaccine in the market. Knwledge Attitude and Practices (KAP) survey Nils Teufel presented preliminary findings frm the KAP survey undertaken by vaccinatrs during the vaccinatr s training in the Lake Zne and suthern highlands. The survey sught t address the fllwing questins; Which knwledge des the training imprve mst? Which type f vaccinatrs have been trained? Hw are training, business activities and persnal characteristics linked t increases in ITM applicatin by trained vaccinatrs? Results shwed mst f the trainees were gvernment emplyees (88%), and 69% f the trainees wned a business. It was nted that the trainings led mre t imprved technical knwledge cmpared t business skills. (Presentatin is available here) Discussin pints Participants nted that the current sample size was t small t draw any meaningful cnclusins, hwever the findings wuld be used as an indicatin f the results. Vaccine supply and recrd keeping Heshbne Tindih f GALVmed presented the state f vaccine supply and recrding keeping n vaccinatins. He reprted the current cntracts signed n vaccine supply fr the regin fr the next 12 mnths were 640,000 dses distributed as fllws: (280,000), Tanzania (275, 000) and Uganda (85, 000). He als nted that actual vaccinatins have been belw the target as shwn in the figure presented. He als nted that all the vaccinatins frm each vaccinatr under all the distributrs shuld be captured under the vaccinatr sheets and shared with GALVmed and ILRI fr cmpilatin. Als distributrs shuld submit mnthly reprts t dcument their prgress. (See presentatin here) Discussin pints The manual says vaccine is viable fr up t 6 hurs after recnstitutin but it shuld be 4 hurs. The team shuld share a scientific paper that can be used as a reference pint n the crrect time. TFDA shuld issue permits t facilitate faster acquisitin f vaccine t help the distributrs meet their targets n time Imprting mst f the required equipment fr vaccinatin takes time and this may limit the distributrs meeting their targets n time A field safety assessment is currently being undertaken by Dr.Mbwile n behalf f the gvernment fr the release f the secnd batch Farmers Representatives Tw livestck keepers, Mr. Nichlas Kidenke frm Kagera district in the lake zne regin and Mr. Giden Mwaihla frm Mbeya, participated in the meeting during which they expressed their appreciatin f the ITM technlgy and hw their adptin f the ECF vaccine use had imprved their dairy prductin and livelihds Mr. Nichlaus Giden Kidenke Mr. Kidenke is a livestck keepers representative in Rwakaremera village Kagera district in the lake zne. Livestck keepers experience a number f diseases but ECF has been the mst expensive in terms 9

10 f treatment (e.g Parvexn and Butalex) and dipping csts. Nevertheless the livestck keepers still experienced high mrtality rates frm ECF related cmplicatins. The farmers started using ECF vaccines in May 2013, thrugh RONHEAM Internatinal Limited. Since then Mr. Kidenke has had 573 f his cattle vaccinated, 200 cws and 373 calves. Mr Kidenke nted that cases f ECF related cattle deaths n his farm have significantly reduced. He currently has mre than 2000 cattle. He mentined access t pasture, imprved fdder crp seeds and the high cst f ECF vaccine as the main challenges he faces. His presentatin in Kiswahili is available here. Mr. Giden B Mwaihla Mr. Mwaihla is a dairy farmer and businessman frm Mbeya. With 37 dairy cattle, he als sited high csts assciated with use f acaricides t cmbat ECF. Hwever, he mentined that after adpting the use f the ECF vaccine, these csts have significantly reduced. He is currently ne f the mdel farmers used by the distributrs in sensitizatin and awareness creatin t ther farmers n the imprtance f adpting use f ECF vaccine. Mr. Mwaihla appreciates that his vaccinated animals have a higher market value; in August, 2016 he sld sme f his animals fr 36,000, 000 TZS (an equivalent f abut16000 USD) and managed t purchase a 5-acre piece f land. He can nw expand his enterprise and educate his children. Discussin pints: Mre emphasis shuld be placed n sensitizing the farmers n the crrect applicatin f acaricides after the ECF vaccinatin t avid ther tick related diseases The ILRI cmmunicatin specialist will wrk with the distributrs t facilitate the dcumentatin f the farmers experiences thrugh vides. Distributrs wrk-plans Participants were divided int grups with a distributr in each f the grups, t facilitate prductin f wrk plans fr the perid Nvember June Hwever, sme extended their wrk plans up t December 2017 beynd the current prject perid. (See prpsed wrk plans here, here and here) Discussin and emerging issues: Several factrs were identified as limiting uptake f ITM in Tanzania: Number f vaccinatrs:there is need fr an institutinal framewrk t achieve increased number and mass vaccinatins Liquid nitrgen supply: It was nted that althugh liquid nitrgen plants are strategically lcated and distributed acrss the cuntry, they require repair and prper maintenance. The meeting highlighted that this culd be achieved thrugh public-private partnerships, and thrugh the recently launched livestck mdernizatin prgramme. In the meantime it was reprted that there are facilities within gvernment that can be shared with distributrs. Dr. Ruheta frm DVS prmised t will give Dr. Mwakulukwa ne liquid nitrgen tank frm the Ministry f livestck in Dar es Salaam which is currently nt in use, There is prgramme that is wrking n an imprved canister- Abdallah Twahir frm GALVmed will take this discussin frward. Updates will be prvided Registratin f the vaccine: TFDA had requested fr sme clarificatins frm CTTBD s that they culd prceed with the registratin but this infrmatin had nt been prvided Presentatin n business develpment ILRI s James Ra presented n a prpsed ITM business develpment mdel (see mdel here). Ra s presentatin fcused n the plan n identificatin f sub-distributrs and discussed the agent assessment tl. He als presented a case study f a business mdel wrking in Kenya by using cmmunity champins t mbilize farmers n behalf f the distributrs at a small fee. 10

11 Discussin pints frm presentatin Participants highlighted that the imprtant pint f interventin fr business develpment is at the vaccinatr level. It was emphasized that the prject is nt restricting the distributrs t any particular actrs, but is wrking with all actrs alng the value chain The participants nted that the business develpment tl will be very helpful in selecting subdistributrs It was further emphasized that in pursuing increased business returns, distributrs shuld be very careful nt t cmprmise the quality f the prduct alng the value chain All the distributrs shuld frward names f districts where they wuld need a sub-distributr and names f pssible agents they wuld wish t wrk with t Esther and James thrugh . Implementatin f central training by LITA A representative frm LITA delivered a presentatin n the prpsed and n-ging plans n institutinalizing central training fr the ITM vaccinatrs in Tanzania. Presentatin is available here Discussin LITA will first train their staff wh will train ther peple interested in the ITM vaccinatin curse. Training prgrams n ECF vaccinatin shuld be embedded int natinal/cuntry-wide prgrams. It was nted that curses are designed as part f the CPD prgrams, therefre they shuld be targeting thse with educatinal qualificatin as required fr accredited vaccinatrs LITA can intrduce ECF and ITM cntent t graduates but can nly ffer the ITM vaccinatr training t graduates, because nn graduates are nt allwed t vaccinate befre graduatin. Students undertaking diplma in animal health and with an interest in ECF vaccinatin can be ffered a shrt curse n ITM vaccinatins. LITA will als incrprate ITM business entrepreneurship skills, in the curriculum. Presentatins by Znal Veterinary Cuncils The Znal veterinary cuncils presented n the histry f ECF vaccinatins in Tanzania, the current prgress and available infrastructure t facilitate the success f the vaccinatins. Dr. Nng na gave a backgrund n the prgress f the ITM ECF vaccinatins in Iringa. Part f the current implementatin plans include farmer sensitizatin, training f vaccinatrs and disease surveillance. The abve activities will be carried ut at cuncil level under crdinatin f ZVC/TVLA thrugh; prvisin f transprt (mtrcycles fr field wrk), financial supprt, equipment and persnnel. This will als greatly rely n the existing pprtunities including; Livestck Field Officers, liquid nitrgen plants (Hali Prject, Sa Hill Farm, Uyle Research Institute) and liquid nitrgen agencies. The prcess will entail a Permitattach prfma invice and a letter frm the DVS. Mr. Bwana will engage the PAID prject n their plans t prvide liquid nitrgen fr the AI prgramme t see if the ITM culd benefit frm this. He articulated that if ITM is widely and successfully up-scaled, the 40% cntributin t agricultural GDP will nt nly duble, it will als have multiplying effects at husehld level leading t healthier cattle, mre milk and meat, better child nutritin increased husehld incme and increased exprts.
