Owner understands and agrees that any prblems which develp with their pet(s) will be treated as deemed best by the staff and at their sle discretin f Creature Cmfrts Pet Resrt. The wner assumes full financial respnsibility fr any and all expenses invlved.(i.e. separating barders, Chicken/Rice meals fr upset stmach, destructive behavir, veterinary services and shuttle fees). 7. Owner understands and agrees that if their pet(s) becme ill r injured, r if the state f the pet(s) health therwise requires prfessinal attentin, Creature Cmfrts Pet Resrt, in its sle discretin, may engage the services f a veterinarian r administer medicine r give ther requisite attentin t the pet(s), and the expenses theref shall be paid by the wner. 8. Owner understands that they will be charged a replacement fee fr destryed bedding and/r rm destructin. 10. I understand that phts, vide, r ther media may be taken f wners and their pets while in ur resrt and that they may be used in publicatin. Owner understands and agrees that dg daycare is scial play with ther dgs frm different husehlds. Please be aware that dgs in grups are at higher risks f incidents. While play is supervised, dgs may becme sre, dirty, hurt, sick, scratched, bit, and even rarely death. Dgs may als be put in time ut crates r runs during the day and fr rest perids. Owner fully accepts and understands these risks. 12. I understand that Creature Cmfrts reserves the right t refuse r revke admittance t any pet that displays an illness r behavir deemed inapprpriate r des nt meet the health requirements set frth in this agreement. Althugh it is the practice f Creature Cmfrts t have caretakers supervising and interacting with the guests, there may be ccasins whereby yur dg(s) may be utside in a fenced area withut supervisin fr a brief time. 14. Owner understands that Creature Cmfrts Pet Resrt is staffed 24 hurs a day, but that breaks and r gaps in staffing may ccur.(i.e. lunch breaks) The phne is nt answered after lbby hurs. Any cntrversy r claim arising ut f r relating t this agreement, r the breach theref, r as the result f any claim r cntrversy invlving the alleged negligence by any party t this agreement, shall be settled in accrdance with the rules f the American Arbitratin Assciatin, and judgment upn the award rendered by an arbitratr may be entered in any curt having jurisdictin theref. Plices Creature Cmfrts Pet Resrt strives t prvide the best care fr all ur guests. The fllwing plicies are t ensure ur guests receive the best care pssible in a healthy and safe envirnment. ADMISSION: All pets must be in gd general health. We reserve the right t deny admittance t any pets lacking prf f vaccinatins, and/r pets displaying signs f untreated r ptentially cntagius cnditins, and/r aggressive behavir. Althugh we have skilled staff members, ur barding caretakers may nt be able t handle geriatric pets needing significant medical care. Prir t participating in any scial activity (e.g. Grup Play), dgs must be temperament tested and apprved. Apprved dgs may have scial privileges revked if their behavir changes and they becme aggressive r dminant. Females in heat may nt participate in grup play. Creature Cmfrts Pet Resrt fllws a rigrus prgram f flea and tick eliminatin. All pets arriving at Creature Cmfrts Pet Resrt with fleas and/r ticks will be treated at the wner s expense. Creature Cmfrts Pet Resrt reserves the right t charge daily handling fees fr illness, excessively difficult r aggressive pets requiring additinal staff and/r additinal time in rder that we may deliver prper care fr yur pet. VACCINATIONS: All dgs and cats must be current n all vaccinatins. DOGS - We require DHPP (Distemper/Parv) vaccinatin, rabies, and a yearly Brdetella (r canine cugh). CATS are required t be up t date n Rabies and FVRCP (Feline Distemper) and have a negative leukemia status. We will cnsider accepting an alternate vaccinatin prtcl and/r tiers with review frm ne f ur wners and ur veterinarian. All custmers are strngly encuraged t fax r deliver current vaccinatin recrds several days prir t their check- in date. Puppies will be accepted as lng as we receive dcumentatin f their first tw sets f puppy vaccinatins. Puppies/dgs that are nt fully vaccinated fr health/age restrictins will nly be allwed t bard in a separate area and extra fees will apply. MEDICATIONS: All medicatin/supplements supplied by Owners must be clearly labeled in riginal cntainers with the pet s name, name f medicatin, dsage, and reasn fr medicatin. We will nly accept the exact dsages required fr administratin during yur pets stay. There is a fee fr injectable ($2 per injectin), detailed times, eye, ear,liquid, (3+ pills) r large/multiple cmbs($4-8daily) als if yur pet is difficult t administer/special medicatin (varies). Ask fr details. BELONGINGS: The Resrt will make every effrt t return items left with each pet, hwever, these items may nt be returned if lst r may be returned in pr cnditin. Please be advised that visiting with yur pet while it is in ur care, may cause anxiety and cnfusin. Owners and authrized peple may visit during lbby hurs nly. 4 EMERGENCIES: In case f an emergency we will attempt t cntact yu r yur listed emergency cntact. We will use yur veterinarian, r the lcal 24- hur veterinary emergency hspital at ur sle discretin. We may suggest yu attach a signed nte that details yur wishes/limits in case we need t seek veterinary care, therwise we will d everything pssible t preserve the life f yur pet. (i.e. blat, bleeding, seizures) Yu will be respnsible fr all csts invlved. ROOMS/RUNS: Rms may be reserved up t 1 year in advance. Depsits are required. Althugh we d ur best t hnr rm requests we d nt guarantee rms/runs. Occasinally variables may require us t mve yur pet t a different rm. Only pets frm the same husehld can share rms. We reserve the right t limit amunt f pets staying tgether. CHECK- IN/CHECK- OUT/FEES: Fees are set daily fr each separate service ffered. Daycare, barding, grming, and training are treated as separate services. Discunts are ffered fr cmbining services. A cmplete list f services and fees can be fund n ur website r call fr details. Check- in/ut can be scheduled anytime during ur lbby hurs. Reservatins are required. Our barding rate is charged daily and check ut is nn Mn- Sat, the daily rate will apply t all barders staying past nn r n Sundays. We understand flights get delayed/weather restrictins happen. We d ask that yu let us knw 24 hurs in advance if the reservatin time (nt date) fr pick- up r drp- ff time can nt be kept, we will d ur best t set up a new time that will wrk fr bth parties. STAFFING/LOBBY HOURS: Mnday thrugh Friday, 7:00 a.m. t 12pm and 1:30 t 7:00 p.m., Saturday 9:00 a.m. t 4:00 p.m., and Sunday 12-4 p.m. Staffing is 24 hurs, but may cntain gaps in schedules and therefre there may be perids f time when the facility is nt staffed (i.e. lunch breaks). HOLIDAYS: Hliday barding perids are defined as the 7 days befre and after each date listed. We will be clsed t the public n the fllwing hlidays: Easter, Memrial Day, Independence Day, Labr Day, Thanksgiving, Christmas Day, and New Years Day. We will clse the lbby at Nn n the fllwing days: Christmas Eve and New Year s Eve. Abslutely n check- ins r check- uts will be permitted n hlidays. The facility will be fully staffed, but the lbby will nt be available, as we will be fcused n caring fr ur guests. CANCELLATION POLICY: We require a depsit at the time f bking. We pride urselves n prviding the best care available t yur pet. Smetimes the grming experience can unveil hidden pre-existing cnditins. Creature Cmfrts Pet Resrt will always bring anything that the grmer may ntice t the wner's attentin. Creature Cmfrts cannt diagnse a cnditin but may advise yu as the wner t seek veterinary care fr yur pet. Parasites: If fleas are fund n yur pet, it is Creature Cmfrts Pet Resrt s plicy t give a flea bath at an additinal minimum cst f $ This cst cvers the special shamp; time and extra clean up t insure n fleas are active in ur resrt. A Mnthly flea treatment is recmmended fr lng-term preventin. Ticks fund will be remved fr an additinal charge. Please nte that parasites are a health hazard t yur pet as well as t humans Aggressive r Dangerus Pets: Owners MUST infrm Creature Cmfrts if yur pet(s) bite(s), has bitten, r is aggressive t peple, ther pets r specific grming prcedures. Muzzles may be used if necessary. Muzzling will nt harm yur pet, and prtects bth the pet and the grmer. Creature Cmfrts Pet Resrt reserves the right t refuse/stp services fr such pet(s) at any time befre r during the grming prcess, and charge an Aggressive Dg Fee in additin t the regular grming charge. 4

5 Mat Remval: Pets with matted cats need extra attentin during their grming sessin. Mats left in a cat nly grw tighter, and can strangle the pet's skin, r eventually tear it pen. Mats can be very difficult t remve, and may require the pet t be shaved. When necessary, remving a heavily matted cat includes risks f nicks, cuts r abrasins due t warts, mles r skin flds trapped in the mats. Heavy matting can als trap misture and urine near the pet's skin allwing mld, fungus r bacteria t grw, causing skin irritatins that existed prir t the grming prcess. After effects f mat remval prcedures can include itchiness, skin redness, self-inflicted irritatins r abrasins and failure f the hair t re-grw. Shaved pets are als prne t sunburn. There is an extra charge fr dematting. Health r Medical Prblems & Senir Pets: If yur pet has any medical cnditins, Creature Cmfrts asks t be made aware f them (sme examples are: arthritis, seizures, skin prblems, masses etc.) s that the grmer can take steps t lk ut fr certain cnditins and use prper techniques and prducts fr yur pets special needs. Grming prcedures can smetimes be stressful, especially fr a senir pet r a pet with health prblems, and can expse hidden medical prblems r aggravate a current ne during r after the grm. Because senir pets and pets with health prblems have a greater chance f injury, these pets will be grmed fr cleanliness and cmfrt, in styles that will nt add t their stress. Sme pets have behaviral prblems that we may r may nt be able t wrk thrugh. If we are unable t cntinue the grming prcess due t behaviral prblems there will be a minimum charge f $15 fr ur time and effrt. Current Vaccinatins/Veterinarian Infrmatin: By signing this cntract, wners verify their pets are current n Rabies, Brdetella, Distemper and Parv, fr canines, and Rabies fr felines. Prf f Vaccinatin r current Titer testing shall be prvided upn check-in as well as current Veterinarian infrmatin. Special circumstances will be taken int cnsideratin. Every effrt will be made t insure yur pet is grmed as safely as pssible and t yur satisfactin. There is always the pssibility an accident culd ccur. Grming equipment is sharp, even thugh we use extreme cautin and care in all situatins, pssible prblems culd ccur including cuts, nicks, rashes, scratches, quicking f nails, etc. In mst cases this can happen when a pet is wiggling r mving arund. By signing this cntract yu (r yur Agent) agree t hld Creature Cmfrts Pet Resrt, it's wners, grmers, and emplyees harmless frm any damage, lss, r claim arising frm any cnditin f the undersigned pets, either knwn r unknwn t Creature Cmfrts Pet Resrt. It is als further understd and agreed the terms f this agreement can change at any time, withut ntice, and will verwrite any and all prir signed cntracts r releases. It is further understd this clause applies t any and all pets grmed under this wner. I agree t pay any and all charges fr prcedures/services rendered at time f service. Late Pick-Ups: If yur dg is nt picked up by clsing fr the day, an additinal $50.00 per day kennel charge may als apply.
