1 Page1 We know many families have working adults and we don't expect anyone to be a super hero and quit a day job just to be able to take home a puppy/dog. Every puppy/dog family has different needs, and this information form helps us identify who needs what, and what homes might make the best fit. If you're a Doberman owner, we don't want to waste your time regaling things you already know; by the same token, if this is your first Doberman and/or first puppy, we want to make sure you are prepared for what life with a puppy AND a Doberman may bring. This questionnaire/application also applies to our non-puppy Dobermans that do not meet our breeding requirements, and/or, Retired Breeders we may have available. PLEASE state your preference if/where applicable below. OUR PUPPIES; -have tails docked and declaws removed by our vet between 3 and 5 days old. -receive first (6 week) and 2 nd (8 week) vaccinations, deworming and vet health certificate (including Fecal test result) prior to new home placement. ALL health records will be sent home with each puppy. -have their ears cropped by our Cropping Vet at around 8 Weeks old. (Cropping Vet name and references provided to approved applicants.) -will be available for their new/approved homes AFTER cropping sutures are removed and first ear posting is done, usually around 10 weeks old. Approved homes MUST have successful ear posting experience OR be within reasonable proximity or either Doberman Fields; OR an experienced, reputable Vet pre-approved to examine and repost the Doberman s ears until posting is no longer needed/ears are properly standing AND firm without fail. PROPER EAR POSTING AND CARE IS CRUCIAL TO THE SUCCESS OF THE EAR CROP. -will be microchipped, by our vet, and enrolled, by Doberman Fields s, the breeder, in AKC Reunite with the owner as primary contact and Doberman Fields, the breeder, as secondary contact. The first year s enrollment fee is included in the sales/contract price. ALL owners are to renew with AKC Reunite yearly, at their cost, and Doberman Fields MUST remain as secondary contact to ensure our dogs are safe and always have a way home if lost. Please take your time this with questionnaire/application. There are NO right or wrong answers. However, this IS our first and very important step in finding perfect homes for perfect Dobermans. We want you to explore our website, get familiar with us AND our Dobermans, our family and our breeding. And while NO ONE will be turned away for only the reason of being a first time Doberman owner, no Doberman will be placed within a first time Doberman home without extensive breed specific information and education from us to ensure this IS the breed for you. As well, no one will be turned away for being just a pet owner BUT we need to ensure ALL our Dobermans, regardless of their role in your life, will a perfect fit and live a loved, active, healthy and busy working lifestyle the breed is meant for, pet, show or otherwise.

4 Page4 All new homes are required to receive an AKC CGC (Canine Good Citizen Certificate) before one year after placement and this title MUST be added to the Doberman s AKC registered name. Please visit the Official AKC website for more information on this training and title. Are you familiar with the CGC training and test? Where will you take the classes and test for this certificate and title? Has ANYONE in your home, past/present/future occupants, been arrested, charged or convicted for animal cruelty or neglect? Please be as specific as possible. We will verify all answers.

6 Page6 OWNERSHIP COMMITMENT: The day puppies will be available to go home will be listed on the website. You will need to be able to take the puppy home within three to four days of this date. Will you be able to pick the dog in person up from Montgomery NY?. We do NOT ship any of our Dobermans via air carriers. We have a DOT approved, licensed transporter available. If this option is used, the expense of transport will be added to your sales/contract price. Pricing will be obtained as soon as possible to help in the decision process. Arrangements will be made by and through Doberman Fields, the breeder, with the transporter company, and the purchaser WILL be provided with a copy of the transporters name, address, references, insurance, quote after approval but prior to finalizing contract and sales amount. ALL prices are charged by the transporter and no additional money is added on by/for Doberman Fields. Billing/Invoicing from the transporter will be included with the Take Home Binder at the time of the dog s arrival to its new home. Will you need transport services for the Doberman?. Do you agree to give us follow up reports, photos and records from time to time? (At LEAST once a year?) Do you agree to return the Doberman to us, Doberman Fields, the breeder, in case you EVER need to rehome this Doberman at any time for any reason? WHAT TO EXPECT NEXT Completing this application is a step in the process to acquiring a puppy from Doberman Fields', but it does not guarantee you will get a puppy from this or any future litter. We, the breeder, reserves the right to refuse any placement at any time at our discretion. APPROVAL AND DEPOSIT: Applications are processed in the order they are received. If your home is approved, you will be notified within 7-10 days of our approval and you will be placed on our waiting list. We breed infrequently, so we will only accept deposits when puppies are actually available. Within a week after whelping, approved homes will receive an requesting a $ non-refundable commitment deposit on a puppy. s will be sent out

7 Page7 in the order approved applications were received. Be prepared to make this deposit within 48 hours of the notification . There will be no second notice or reminders sent out PLEASE NOTE: When you make a deposit, you are committing to take a puppy from the designated litter. Approved applicants whose deposit is not received within 7 working days after our request is SENT will forfeit their place in line for preference/pick and may jeopardize their approval for purchase. Upcoming breeding, as well as litter expected and whelping news will be announced and constantly updated on our website, so please plan accordingly. MEETING THE PARENTS: It is strongly encouraged that you try to make arrangements to meet the Sire and Dam of the litter, the rest of the pack and pets, as well as our family once it has been strongly established that both parties of this possible sale are reasonably sure of their choice in the breed, the breeder, and the purchaser. Being a Hobby Breeder this is our home, family, and environment and NOT a commercial breeding kennel or business. Arrangements to the mutual convenience of BOTH parties will be necessary. And while we know this type of visit is not always possible, it is encouraged for several reasons. We, as your breeder, will be there for the life time of the Doberman. You, as the purchaser, will have the breeder s Doberman for its lifetime will be added to the Doberman Fields Family. Getting to meet the people, the Dobermans, seeing the temperaments of the Dobermans, reviewing health testing, pedigrees, and spending time in a meet and greet/questions and answers for BOTH parties can and does ensure we are ALL making the right decision for the Doberman s future happiness. YOUR PICK OF PUPPY: Puppies will be assigned in request AND deposit order -by the first or second preference of sex and color of the depositor as indicated on the application, as well as matched up BY THE BREEDER to the individual needs/wants of the family and the temperament of the puppy. At any time after your commitment deposit payment is made, should you elect to not take a puppy from the litter, your deposit is forfeited, non-refundable, and applied to the costs of finding the puppy another home. It is not forwarded to an upcoming litter as we do not have regular litters. GOING HOME: We cannot hold puppies for extended times; it is critical for their social development that the litter departs within a reasonable timeframe, around 10 to 12 weeks of age. If you have a big vacation planned or have events that would conflict with a young puppy, please know puppies cannot be boarded in a kennel until they have received their full immunizations and their rabies vaccination which cannot be done until 3 months of age. CONTRACT: We have a contract and will not place a puppy without a signed contract. We believe it's the professional thing to do to memorialize our obligations to each other on paper so there are no questions later, and for that reason it's a bit lengthy. If you have questions about specific terms, you need to contact us prior to signing/sending your deposit. SPAY/NEUTER (Alter): We require spay/neuter to be done NO SOONER THEN 18 months but by 24 months of age. This might seem hypocritical at first glance, but we have a take-back provision in our contract. Any puppy from our home always has a home here, and will never experience the fear and burden of life at a shelter. Taking back an unaltered dog into our home poses significant risk to our own dogs due to canine STDs and the possibility an unaltered dog could be pregnant. Spay surgery while pregnant is exceedingly risky, and several canine STDs

8 Page8 such as brucellosis can be transferred to humans. This poses an unacceptable risk to our family. Because we take cradle-to-grave "original home" responsibility for the puppies we created, puppies must be altered, between 18 and 24 months of age (unless you are under a breeder contract or special consideration agreement). Additionally, altering a dog of this size/breed prior to sexual growth maturity, and prior to the bone growth plates closing, can and will have long term ill effects on the dog s health and wellbeing. Altering earlier than 18 months or after 24 months will violate our contact, void all guarantees the purchaser will incur a fine from the breeder. REGISTRATION PAPERS: All our Doberman puppies will be registered for you by Doberman Fields', the breeder, in the AKC and UKC when your puppy is paid for in full (by eight weeks of age). Normal registration is included in your contract/sales price. Additional needs/requirements of the purchaser(s) will incur additional costs, which will be added onto the sales contract. These additional registration options will be available for your review upon your application approval status AND when the sales contract is sent to you. These include but are not limited to; additional fees for long name, AKC- 3 Generation Pedigree, etc. All owner chosen puppy names MUST be suffixed with Vom Doberman Fields. No exceptions. Prior to signing and returning this questionnaire/application, PLEASE feel free to us with any questions, clarifications or concerns you may have.
