Entries from Obedience and Rally may not be combined for discounted second entry. Obedience Trial $20.00 for the first entry of a dog (includes $.50 AKC recording fee and $1.50 AKC event fee) per day; $12.00 for each additional entry of the same dog in regular or non-regular Obedience classes Rally Trial $20.00 for the first entry of a dog (includes $3.00 recording fee) per day; $12.00 for each additional entry of the same dog in Rally classes. American Kennel Club Certification Permission is granted by the American Kennel Club for the holding of this event under American Kennel Club Rules and Regulations. James P. Crowley, Secretary No entry fees will be refunded if the trial cannot open or be completed by reason of riots, civil disturbances, fire, an act of God, public emergency, act of a public enemy, or any other causes beyond the control of the organizing committee. Entry fees shall not be refunded in the event that a dog is absent, disqualified, excused by a veterinarian or judge, or barred from competition by action of the Trial Committee.

Countryside Agility Training Center is located in the Lakeside Commons Mall. This is an old mall located next to Lake Edinboro. From the South - Take I79 North to Exit #166 (Edinboro Rt. 6N) turn right (East) and go 2.17 miles to Lakeside Commons (on left ). Entrance is just after the cement bridge. If you get to Taco Bell you have missed entrance. The training center is all the way around back of the building right on the lake, Suite 600. Countryside Agility Signs are in the window. Unloading for crating will be marked. Please do not enter with dogs at front of mall! From East and West - Take I90 East or West to Rt. 79 South. Get off at Exit #166 (Edinboro Rt. 6N) and turn left (east) and go 2.17 miles to Lakeside Commons (on left). Entrance is just after the cement bridge. In the case of a tie, time will be used if within the same class. If 2 teams in different classes have identical scores, the team with the most stations/second will be declared the winner. All Ribbons, Trophies and Placement Prizes will be awarded by the conclusion of the Trial. The winning dog and/or handler must be present.

Please mail checks or money orders with entries. Trial Secretary assumes no responsibility for cash sent through the mail. Telegraph, Telephone, and Unsigned Entries Cannot be Accepted. Entries not on current, official AKC entry forms, or photocopies of entry forms without agreement and rules on the reverse side of the official AKC entry form are NOT ACCEPTABLE and will be returned. Entries without fees will not be accepted. Acknowledgment of entries by written confirmation letter will be made as soon as possible after trial closes. If a legible address is provided, an confirmation of entry will be made as soon as possible as entries are received and processed. Post Dated Checks and Returned checks do not constitute a valid entry fee. The Trial Secretary will add a collection fee of $25 to the amount of each returned check. No Refund for Duplicate Entries. Every effort will be made to find duplicate entries; however, if the entry has been processed, no refund will be made. Declination of Entries. No entry shall be made and no entry shall be accepted which specifies any conditions as to its acceptance. may decline any entries and may remove any dog on account of disease, viciousness, or other cause, and no one shall have any claim against NSCA or any official thereof. Errors on Entry Blanks. Owners are responsible for errors in the filling out of out entry forms. No entry and no entry fee will be refunded in event of such errors, or for cancellation of entries after the published closing date. Please type or print carefully on entry forms to eliminate errors. Bitches in Season will not be admitted to the Trial. Any exhibitor with an entered bitch in season on the trial date will be refunded the entry fee less $10.00 (each day) upon written certification by a veterinarian given to the Trial Secretary before the start of judging. Judges will not wait for any dog holding up a class. Owners or agents are responsible for the presence of their dogs in the judging ring when they are called to be judged. Dogs may arrive any time prior to their scheduled time of judging. Dogs not required for further judging may be excused. Exhibitors should follow their veterinarians' recommendation to assure their dogs are free of internal and external parasites, any communicable diseases, and have appropriate vaccinations. It is expressly understood that exhibitors alone are responsible for the behavior of their dogs and/or children. Any exhibitor whose dog and/or children create unnecessary disturbances or repeatedly engage in unsafe or disruptive behavior may, at the discretion of the Trial committee, be asked to leave the show site. In such cases, no refund of any fees paid will be made. Countryside Agility, National Shiba Club and its agents, employees, etc. will assume no responsibilities for any loss, damage or injury sustained by exhibitors or handlers, or to any of their dogs or property, and further assume no responsibility for injury to children.

Move-Ups Obedience and Rally move-ups will be accepted as space permits within entry limits, in accordance with Chapter 1, Section 16 of the Obedience Regulations.

5 AKC Rules, Regulations, Policies and Guidelines are available on the American Kennel Club Web site, AGREEMENT I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem sufficient. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors, governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog. The following categories of dogs may be entered and shown in Best of Breed competition: Dogs that are Champions of Record and dogs which, according to their owners= records, have completed the requirements for a championship, but whose championships are unconfirmed. The showing of unconfirmed Champions in Best of Breed competition is limited to a period of 90 days from the date of the show where the dog completed the requirements for a championship. 3. (Event Class) Consult the classification in this premium list. If the event class in which you are entering your dog is divided, then, in addition to designating the class, specify the particular division of the class in which you are entering your dog, i.e, age division, color division, weight division. 4. A dog must be entered in the name of the person who actually owned it at the time entries for an event closed. If a registered dog has been acquired by a new owner it must be entered in the name of its new owner in any event for which entries closed after the date of acquirement, regardless of whether the new owner has received the registration certificate indicating that the dog is recorded in his name. State on entry form whether transfer application has been mailed to the AKC. (For complete rules, refer to Chapter 11, Section 3.)
