Briefly describe your animal programs: The Humane Society of Broward County (HSBC) placed 9,881 pets into loving new homes between October 1, 2010 and September 30, Adoptions: HSBC receives thousands of animals who can no longer be cared for by their owners. It is the goal of the Society to provide permanent, loving homes for all healthy, adoptable animals surrendered. Our adoption fee includes 2 of 7 9/5/12 2:52 PM

3 saying/neutering for every animal, including puppies and kittens, before going home; Preliminary vaccinations and de-worming; Overall wellness check; Heartworm test for dogs and Feline Leukemia test for cats; Microchip and registration; Limited 14-day health plans; Free pet behavior counseling. Senior Scholarship Program: HSBC is able to waive part of the adoption fee for those individuals who are 60+ years of age and are looking to adopt a pet that qualifies for the program. Potential adopters must still meet all the HSBC adoption criteria. This program connects senior citizens in need with animals who need love. Education: HSBC believes we should begin educating our children at a very young age about respect and kindness to all living creatures. The Humane Society s Education Department reaches 150,000 adults and children through their wide variety of curriculums, events, partnerships and school programs. Education programs include: Pet Clubs with the Boys and Girls Clubs of Broward County; Partnerships with the Girl Scouts of Broward County whereby the No Bones about It patch can be earned; Partnership with Women In Distress; Humane Science Education Curriculum in all Broward County Schools K - 5th grades; 12 FREE school presentations available for all grade levels; Annual Be Kind to Animals Week poster contest; Character Education Curriculum in all Broward County School middle schools; PHAT Teen Club where teens earn service hours while helping homeless animals; Pet Press Newspaper for Kids; Wags & Tales Reading Program; Animal Assisted Therapy Program with 90 dogs visiting 120 facilities; Pet loss and grieving support group; Adult animal adoption awareness days; Good Dog training program for shelter dogs waiting to be adopted; Low Cost Puppy Preschool Classes; Low cost positive dog training classes; FREE Pet Behavior Helpline; FREE Bow Wow Basics Class; FREE Meow Meow Basics Class. If not all, describe your sterilization policies and procedures for assuring sterilization after adoption: Give additional background information on your organization's programs as they relate to this application and the qualifications of the personnel who will be in charge of this program. Over the course of fiscal year 2011 HSBC clinic staff performed spay/neuter surgery on a total of 14,671 dogs, cats and rabbits (another 6,292 had surgery performed outside of our shelter, facilitated by HSBC staff, by local vets). HSBC spay/neuter programs are as follows: Fix Your Kitten: during the summer of 2011 HSBC offered half price spays and neuters for kittens and nearly 450 youngsters were sterilized. Because of the overwhelming success of the program the shelter now has another offer to encourage pet owners to get their kitten scheduled for this important surgery. Prices are normally $40.00 for females and $25.00 for males but if a kitten is between the ages of 8-12 weeks the cost will be only $15.00; Free Pit Bull Sterilization: Sadly they are one of the most misunderstood breeds and thousands end up in shelters all across America every year. In an effort to tackle the problem in our community HSBC is offering FREE spaying and neutering for pit bulls 8 weeks to 6 months of age through December of 2012; Spay/Neuter Incentive Program: HSBC will spay/neuter - for $10.00 each - the mother & father of incoming litters of puppies and kittens. HSBC has one full-time veterinarian, per diem vets on staff and 13 vet techs. There are 4 employees whose job it is to schedule these thousands of surgeries - HSBC receives approximately 500 phone calls and 400 s per week requesting our low-cost spay/neuter service. We have successfully shown that we have the qualifications and ability to continue to carry out our spay/neuter programs. If you currently have a program for sterilization of cats and/or dogs, describe your current level of funding and productivity and why additional resources are needed? The $430,000 clinic budget is supported by general donations, fundraising events, low-cost fee services and grants which all helped to fund the 14,000+ surgeries performed in FY 11. Additional funding is needed to support the enormous continuing need for affordable spay/neuter surgery and for HSBC to have the means by which to perform as many more surgeries as possible. The numbers above seem to reflect an accurate representation of the target area animal population numbers. Please explain what you believe are the most substantial sources of dog and cat overpopulation in the target area: HSBC has seen substantial sources of animal overpopulation in our target area from people who are having financial difficulties and cannot afford basic pet care. HSBC has been focusing spay/neuter efforts on the pit bull breed of dog and cats. What kinds of spay/neuter services are currently available in the target area and in what ways are these resources currently insufficient to meet community needs? HSBC provides in-house low-cost spay/neuter services thru various programs: SNIP - we will spay/neuter mother & father of incoming litters of puppies and kittens for $10.00 each; we are continuing our half-price offer and will spay/neuter kittens between the ages of 8-12 weeks for $15.00 each (Prices are normally $40.00 for females and $25.00 for males); we provide free spay/neuter service for Pit Bull dogs; and at the end of April we are targeting barn and feral cats and will be taking our mobile clinic unit to a rural farm area of Broward County. Insufficient funding is due to the continuing economic difficulties and the large free-roaming cat population in the area. Additionally, owners of Pit Bulls, generally, are resistant to having their dog spayed or neutered. We hope the incentive of free surgery will be the encouragement needed to bring them in. In the past, when this free service was offered, we had 87 Pit Bulls undergo surgery. Florida Animal Friend is highly supportive of proposals that are focused on animal populations that are identified as substantial sources of dog or cat overpopulation rather than being diluted over too broad of a geographic area or diverse animal populations. Describe the specific target animal population of the spay/neuter project proposed for this grant: Pets in low-income families Especially difficult or unique animal overpopulation sources Pit bull dogs Feral cats Special spay/neuter event Other Objectives What do you hope to accomplish with these funds (objectives should be specific and quantifiable)? Humane Society of Broward County would like to have the means by which to add additional surgery days which would result in more dogs and cats being sterilized. Funding would enable us to add those additional surgery days and would support the medical supplies and drugs which would be required. Additionally, we would like to continue with the $15.00 sterilization program for kittens, the free sterilization program for the Pit Bull breed and the new sterilization program just added whereby barn and feral cats will be targeted, How does this program increase the number of sterilization surgeries above the existing baseline? By adding additional surgery days, we anticipate an increase of 1,000 animals over our current baseline. Our hope, too, is that by having the ability to continue to offer $15.00 spay/neuter for kittens and free spay/neuter for Pit Bulls then more of those pet owners will schedule their pet for surgery. 5 What criteria will you use to determine eligibility for the program? The HSBC spay/neuter programs are available to all citizens regardless of income. Our focus and goal is to sterilize as many dogs, cats, kittens and puppies as we possibly can. How will you advertise the program? Explain how the advertising will reach the target audience. Humane Society of Broward County's Vice President of Marketing has been with the organization for 15-1/2 years and has established strong relationships with local radio, TV and print entities. She appears every Saturday morning on the local ABC station, WPLG, to promote the Humane Society of Broward County and our many programs and services. The station is the second most watched English-language station in south Florida. HSBC sterilization programs will also be marketed via: Website promotion which has information available for all of our programs. The HSBC website averages 126,042 visits each month; Facebook - which has fans; the HSBC quarterly newsletter, Paw Prints, which is mailed to approximately 20,000 local citizens; press releases; brochures that are written in Spanish and available for distribution; the HSBC shelter has approximately 200,000 visitors each year and flyers advertising our various programs are displayed throughout the building and are available for an individual to take upon leaving. How will you address barriers to full use of the program such as transportation, illiteracy, and cultural hurdles? Transportation: The Humane Society of Broward County has always required that pet owners use their own vehicles or public transportation (our shelter property is adjacent to Tri-Rail's Fort Lauderdale/Hollywood Int'l. Airport Station at Dania Beach) in order to bring their pet in for surgery at our one location in Fort Lauderdale. From time to time our clinic staff will travel with our mobile spay/neuter unit to other areas of the county which can ease the burden of long distance travel for the pet owner. Literacy: HSBC has brochures printed in Spanish that are available at our shelter; we also have several bi-lingual employees on staff who are available, along with English speaking staff, to assist pet owners. Cultural Hurdles: HSBC staff is exposed, through various training methods, to the diversity of the south Florida population. Does this project involve the transportation of animals by someone other than the client? If so, describe the vehicles, methods for confinement, personnel training, liability releases used to assure the safety of the animals and handlers. No. Veterinary Services Objectives What arrangements have you made with veterinarians to perform the surgeries? Humane Society of Broward County veterinarians and per diem veterinarians will perform the spay/neuter surgeries. Veterinary Practices Fee Range What is the fee range to be paid for spay and neuter and what is the distribution to be paid by the client vs. the grant program? Keep in mind that Florida Animal Friend grant funds may only be used for costs directly associated with sterilization surgery (including anesthesia and pain control) and not for other items such as vaccines, testing, licensing, and capital purchases. Surgeries are performed on the HSBC mobile spay/neuter clinic Tuesday - Saturday. Pet owners are instructed to bring their pet to the Humane Society of Broward County's mobile spay/neuter clinic at 8:00 A.M.; pet pick up time is 4:00-6:30 P.M. Scheduling an Appointment: Pet owners can either call or send a request for an appointment via . When their appointment is scheduled, an HSBC staff member will also provide the pet owner with all of the necessary pre-surgery information they need for their pet. Budget Total number of sterilization surgeries projected: Cats: 900 Dogs: 100 Total budget requested (Budget should not exceed $25,000): $25, Average cost/surgery projected: $25.00 Describe any expenses that are not included in the grant and how they will be paid for: Humane Society of Broward County staff veterinarian, vet techs and per diem veterinarians will be paid for out of the annual budget. Timeline All projects must be completed within 12 months of receipt of funding. Any unexpended funds must be refunded to Florida Animal Friend within 30 days of the end of the project. Requests for time extensions must be made in writing at least 30 days prior to the end of the project. Funding for accepted projects will be available after August 1, Projected start date: 10/01/12/ Projected end date: 09/30/13 Future Funding Explain how the organization plans to fund this program in the future. Having plans beyond seeking funds from other funders enhances the chances of receiving this grant. The Humane Society of Broward County's budget provides for approximately $430,000 of its general contributions and fundraising revenue for clinic program funding. 7 Applicants selected for funding are expected to publicize their grant in support of their spay/neuter program via press releases, newsletters, website links, etc. In addition, they are expected to promote the sales of license plates so that additional spay/neuter grants can be funded.
