1 Consent to Administer CPR/First Aid All of the staff(s) at Munchkin Academy nc. are fully trained in CPR and First Aid. n the event of an emergency involving your child, a staff will make every effort to contact the parent, however, the well-being and comfort of the child will be our first priority. n the event a staff member may need to administer CPR/First Aid, parental consent will be required. By signing this form, you are giving full permission to the staff at Munchkin Academy nc. to administer CPR/First Aid to your child. Parental consent must also be given in the event your child needs to be transported by ambulance to an emergency center for treatment. Cost of such transportation will be the responsibility of the parent/guardian and not Munchkin Academy nc. Munchkin Academy nc. will not be responsible for any incident that may occur as a result of false, misleading or missed information that is given or omitted at the time of registration by parents/guardians or anytime thereafter. 2 Photo Authorization Munchkin Academy nc. often takes photographs or videos during normal day-to-day activities, special events, or school outings. These images may appear in scrapbooks, on school bulletin boards, in newsletters or publications, on our website, or on our Facebook page and other social media sites. These images may also be used in local media or for promotional purposes. Photography of enrolled children and the use of images of enrolled children at Munchkin Academy nc. require parental permission. Give my permission to the staff at Munchkin Academy nc. to take photos/videos of my son/daughter to use in scrapbooks, on school bulletins, newspapers, publications, social media, local media and promotional purposes. 3 Participation in Holiday Activities At Munchkin Academy nc. we believe in the celebration of special events and holidays. We will be celebrating the major holidays such as Christmas, Thanksgiving, Easter and Halloween as well as special events including Valentines Day, St. Patrick s Day and more. We understand that some families do not celebrate some of these holiday s in which other arrangements may need to be made. By signing this consent form you are giving Munchkin Academy nc. permission to include your son/daughter in special events and holiday events. Give my permission for my son/daughter to participate in holiday activities and special events at Munchkin Academy nc.

4 Administration of Medication n the event that your child requires medication during the school hours, Munchkin Academy nc. staff will administer medication according to dosage and requirements printed on the label of the medication bottle. This is including Puffers and Epi Pens. We will require Epi Pen/Puffer to stay at school at all times. All medications must be in its original form and package with the child s name as well as the correct dosage and information of medication. By signing this form, you are agreeing to give the staff at Munchkin Academy nc. permission to administer medication to your child as required on the medication label. Give my permission to the staff at Munchkin Academy nc. to administer medication to my son/daughter. All medications will be in its original packaging with the correct dosage and instructions.

5 Off site Activities and Field Trip Consent n the event that a program at Munchkin Academy nc. goes on a school outing or a field trip, we will require parental permission for your child to attend the outing. For special field trips we will send home a permission request form however, we may have opportunities to take the students to the park on a nice day. By signing this consent form, you are giving the staff at Munchkin Academy nc. permission to take your son/daughter off site. Give my permission to the staff at Munchkin Academy nc. to take my son/daughter off the school premises.

6 Pick-Up and Drop Off At Munchkin Academy, the safety of our students is our number one priority. We understand that the parent/guardian of the child may not always be available to pick-up/drop off a child. We will NOT release a child to an unknown person at ANYTME. We ask that you list those people who you give us permission to release your child to. Photo D may be required upon pick-up. We will also require a written note or a phone call prior to the pick-up of your child. f someone outside of this list shows up to pick-up your child YOU as the parent will be notified and your child will stay in our care until you come to pick-them up. 7 Policies and Procedures At Munchkin Academy nc. we want what is best for the student so they may have the best Preschool/Junior Kindergarten experience possible. We have set out a number of policies and procedures that we as staff will follow throughout the school year as well as a number of policies and procedures that we ask you as parents/guardians to follow. We ask that you read ALL of Munchkin Academies policies and procedures so you fully understand them. By signing this form, you are stating you have read all the policies and procedures of Munchkin Academy nc. and you agree to abide by these policies and procedures while your child attends one of our programs.
