1 SENS TERMS & CONDITIONS SELANGOR NEUTERING SUBSIDY - POLICY, TERMS & CONDITIONS The Selangor Neutering Subsidy is funded by the Selangor State Government to assist with reducing stray cats and dogs by offering a neutering subsidy to qualified applicants. This is a pilot project with a fixed fund managed by PAWS Animal Welfare Society ( PAWS ). Subject to the conditions, policy and terms herein, the fund subsidises the cost of neutering cats and dogs. Once the fund is exhausted, the subsidy ends. QUALIFICATION CONDITIONS The subsidy is only available to natural persons making application and fulfilling all the following conditions: 1. Only available for the actual cost of the neutering procedure of cats and dogs only (collectively the animals ) performed by a veterinarian in a veterinarian clinic (as defined, approved and/or licensed under the Veterinary Surgeons Act, 1974) located in Selangor. 2. The subsidy is available up to a maximum of RM60-00 per cat and RM per dog only for neutering; all other cost and expense do not qualify. 3. The applicant must be born in Selangor OR be a registered resident of Selangor (as per NRIC) and must make the application personally. 4. Each person is entitled to apply for subsidy for a maximum of 5 animals only per week. Provided that there shall be one Application Form per animal and one Vet Receipt per animal. 5. The following do not qualify for the subsidy: Pre and post neutering cost and expenses Neutering of pregnant animals The subsidy is not available for any animal receiving any monetary assistance, donation, subsidy or such like (whether in whole or in part) for the neutering of the said animal from any other source The subsidy is not available to third party applications or applications by corporate entities, government or semi government entities, statutory entities, societies or such like Page 1 of 6

2 The subsidy does not apply to any GST or any other tax impositions, bank charges, fees or impositions of whatever kind, including for interbank GIRO transactions, or such like 6. The Application Form provided at selangorneuteringsubsidy.wordpress.com/form must be used. The Application Form must be completed in BLOCK CAPITAL LETTERS by the applicant and the relevant veterinarian An Application Form which is incomplete or which contain any alteration, cancellation, erasure, incorrect information or may have been tampered with is subject to rejection The completed Application Form with all required attachments must be received by PAWS within 14 days after the date on the Vet Receipt Save for bank account particulars, by applying for the subsidy, the applicant grants consent and agreement to the publication of all or any of the applicant s information and/or particulars in the Application Form, the Vet Receipt and/or attachments thereto, on selangorneuteringsubsidy.wordpress.com and/or on the Selangor Neutering Subsidy s Facebook page. 7. The applicant and application must also fully satisfy all the other conditions set out below. PROCEDURE 8. Print Application Form Print out the Application Form from: selangorneuteringsubsidy.wordpress.com/form 9. Complete the Application Form and attach Collage & Vet Receipt Complete in full the Application Form. Also procure the relevant veterinarian s confirmation and signature on the Application Form. Attach the following to the Application Form: Page 2 of 6

3 9.1. Attach the original copy of the Vet Receipt Attach the original copy of the veterinarian receipt ( Vet Receipt ) to the Application Form. *** For specifications see below 9.2. Attach the Collage of photographs Take photographs of the animals before and after neutering. Combine the photographs into a collage ( Collage ) and attach to the Application Form. *** For specifications see below 9.3. Attach photocopy of NRIC Attach a photocopy of the applicant s NRIC. The photocopy of the NRIC may be crossed neatly and marked For Subsidy Only without concealing or hiding any information thereon. All attachments must be free from any alteration, cancellation, erasure, incorrect information and shall not be otherwise tampered with. 10. Post the Application Form with attachments to PAWS Post the fully completed Application Form together with all required attachments to: SELANGOR NEUTERING SUBSIDY PAWS Animal Welfare Society Pilmoor Estate, Subang Airport Road Subang, Selangor, Malaysia Applicants are advised to use Express Post. In the absence of actual proof of delivery, applications shall not be deemed delivered to PAWS and the date of delivery as recorded by PAWS shall be deemed binding, effective and final on the applicant. 11. Specifications for Vet Receipt The Vet Receipt attached to the Application Form must satisfy the following conditions: Page 3 of 6

4 11.1. Original copy of Vet Receipt The Vet Receipt must be the original copy and must: Be issued to and show the full name of the applicant (as per NRIC) Clearly particularise the following: (a) (b) (c) The date when the Vet Receipt was issued The charges/fees for the neutering procedure only for the specified animal The address and official stamp of the veterinary clinic where the neutering was performed Confirm that all charges and fees have been fully paid and received Be signed by the veterinarian who carried out neutering on the specified animal and signing on the Application Form. Specifications for Collage The Collage of photographs must consist of photographs taken before and after the neutering of the animal and must satisfy the following conditions: Page 4 of 6

5 12.1. The Collage must be clear and sufficient for PAWS to confirm that the same animal appears in all photographs i.e. photographs should show the face and/or distinct markings of the animal The after photograph/s must show the animal in an awakened state and clearly show the surgical incision for neutering. The Collage may be created using Moldiv app for smartphones, picmonkey for PCs or other similar applications/programmes. Sample Collage Page 5 of 6

Absolute discretion PAWS shall be entitled at its sole discretion be entitled to approve or reject any application without obligation to assign any reason for such decision. 14. Amend, change and/or vary PAWS shall be entitled at its sole discretion be entitled from time to time and at any time to howsoever amend, change and/or vary the format of the Application Form, the procedure, terms and conditions set out in this document. Applicants bear the sole responsibility of keeping themselves apprised of all and any amendments, changes and/or variations. Page 6 of 6
