While in my care, I will treat these dogs humanely, including but not limited to, providing adequate food, water, and shelter. I understand and accept all of the risks inherent in the care of a foster dog, and waive any claim against RDV for any potential damage, expense, costs or other consequences which may arise from my provision of foster care. I agree not to abandon or surrender the foster dog except as prescribed in the rules and guidelines established by RDV. I understand that RDV is financially responsible for the medical care of each dog in its program. I agree to obtain permission prior to seeking medical vet treatment for any foster dog in my care from the RDV Rescue Coordinator or Foster Coordinator. If I take a foster dog to the vet without obtaining permission, unless it is an emergency or life or death situation, I may not be able to receive reimbursement for those expenses and could be held accountable for any issues that result. If there are circumstances where I intend to adopt and make a permanent part of my home, I will sign an adoption contract, pay the adoption fee as determined by RDV and notify RDV within 7 days of receiving the dog.

2 Foster Contract Page 2 In addition, any durable materials such as crates, training collars, containment devices or other items provided to me by RDV for the sole purpose of assisting in my care of an RDV rescue dog, will remain the property of RDV and will be returned in substantially the same condition as offered once no longer needed for the care of an RDV rescue dog or upon my discontinuation of providing foster care. I also agree that at any time, RDV may request the return of any equipment during my tenure as a foster home. PLEASE READ BEFORE SIGNING. Contract modifications require the approval of a board member and an Addendum attached to the contract with the modified agreement. An unauthorized altered contract is automatically null and void, if another party changes the content of the contract The breed we post for dogs are our best guess, if it is a priority that an adopter needs to know the exact breed, they will need to have the dog tested or accept that it is our best guess and not an exact science. INDICATE X Please Print Above: First and Last Name Including Wife, Husband, Partner ( Adopter ) agrees that in consideration of the fee of $ paid to Rescue Dog Village (also known as RDV) Rescue Dog Village is allowing Adopter to adopt this dog. This fee, less $50 is refundable provided the Adopter personally returns (we do not provide transportation) the dog to RESCUE DOG VILLAGE within fourteen (14) days of adoption IF Adopter provides at least 48 hours notice prior to returning the dog. After 14 days the adoption fee is non-refundable and considered a donation. Should the Adopter alter the appearance of the dog in any way or return the dog in poorer health than when adopted, RESCUE DOG VILLAGE has the right to refuse a refund or adjust the refund accordingly. The Adopter, from the date of this contract, agrees to release and indemnify RESCUE DOG VILLAGE from any and all legal and financial claims, known or unknown, now or hereafter arising in connection with the animal. This dog is adopted as is and Adopter assumes all responsibility for treatment of any existing condition or condition which may arise. RESCUE DOG VILLAGE makes no warranties regarding the health, behavior/temperament, age or breed of the dog. If the dog is returned any paid and/or unpaid expenses incurred by the Adopter while in possession of the dog are not transferable to RESCUE DOG VILLAGE but are the sole responsibility of the Adopter. Adopter agrees to take the animal to a licensed veterinarian within fourteen (14) days of this contract. The RDV Rescue. (RESCUE DOG VILLAGE) makes no representations or warranties regarding the health of the dog. INDICATE X

4 Adoption Contract Page 4 ALL vet expenses incurred while dog is in Adopter s care, shall be the sole responsibility of the Adopter. Adopter agrees the dog is to be a family companion and shall provide the animal with a safe home; a quality diet; regular veterinary care; monthly preventatives; adequate exercise; appropriate training and attention. The Adopter agrees to have the dog leashed AT ALL TIMES when not in a securely fenced area and shall provide a safe means of exercise and a safe area in which to relieve itself. The animal shall never be tied to a fixed object nor attached to an overhead cable runner or any variation of such. The Adopter shall not endanger the animal by transporting in an open vehicle or leaving a choke collar on when unattended The Adopter shall provide a collar and identification tag for the animal to wear at all times when allowed to go outdoors. If, for any reason, the Adopter cannot keep the dog, the Adopter shall return it to RESCUE DOG VILLAGE. The animal shall not be given away, sold or exchanged without the prior written consent of RESCUE DOG VILLAGE In the event of the Adopter s death, the dog must be returned to RESCUE DOG VILLAGE or be placed in an RESCUE DOG VILLAGE approved home. Dogs placed through RESCUE DOG VILLAGE will reside in the house, at the Adopter s address. The dog shall not be kept in the garage or in the basement exclusively. In no event will dogs placed by RESCUE DOG VILLAGE be left outside while Adopter is not at home. If the dog has not been spayed or neutered by Rescue Dog Village and the dog is under 6 months, the Adopter agrees to have the dog spayed or neutered within 1 month of the dog turning 6 months old. The Adopter agrees to send proof of this to RESCUE DOG VILLAGE. RESCUE DOG VILLAGE reserves the right to contact the Adopter s veterinarian for confirmation. 5 Adoption Contract Page 5 ACKNOWLEDGEMENT I/we, the Adopter(s) acknowledge having read and agree to abide by each of the rules for adoption set forth herein. Agrees to a home visit at 3 months and upon request of Rescue Dog Village thereafter for the life of the dog. Adopter agrees to indemnify and hold harmless RESCUE DOG VILLAGE - RESCUE DOG VILLAGE Rescue from any and all damages suffered by Adopter and any third party resulting from the adoption of this pet. RESCUE DOG VILLAGE retains superior title to this pet, limited to and for the express purpose of assuring the pet s well being. If Adopter breaches any part of this agreement, RESCUE DOG VILLAGE reserves the right to immediately take possession of the pet and Adopter agrees to immediately surrender the pet without need for legal action by RESCUE DOG VILLAGE. In the event RESCUE DOG VILLAGE must take legal action related to this agreement, Adopter agrees to pay all attorneys fees and expenses. Rescue Dog Village reserves the right to enforce this contract in order to protect the welfare of the Dog. Adopter understands that RDV may examine and/or make inquiries about the Dog at any time. Adopter also consents to RDV speaking with their Veterinarian to verify the care given. Refusal to cooperate with inquiries of the Dog shall be considered a breach of contract.
