There is a room later for expansion of your experiences beyond the minimum required for the camp. Work experience (other than vet/animal): Please list your last two jobs. Include a brief description of duties and the months experience. Denote if the job was 20 or more hours/week. List no more than two jobs. Community Service: Please list community service projects and a 1 phrase description of the activity. Include hours of service per project. List no more than 100 hours of service.

3 Extracurricular Activities/ Academic Clubs: Please list up to three extracurricular activities that are DIFFERENT from the jobs/community service given above (unless, for example, you listed Thanksgiving holiday dinner: 4 hours: NHS NHS does other things besides volunteer work. You may list it below.) Include in your listing any awards you have received for your activities. Include any leadership position. Other Animal Experience: Please list any other animal experience you have had and an estimation of the hours worked. Include a brief description. This should be animal experience other than that you are claiming under veterinary/animal experience.

4 Essay Subject Essay should be typed, double-spaced, and 1 page. Describe the area of veterinary medicine in which you would like to specialize and why. Please include a description of at least one talent you possess that contributes to your desire to complete a professional program in veterinary medicine.

5 Verification of Volunteer Hours Recommendation letters MUST state the number of hours you have worked for the vet or participated in extracurricular activities. Please stress this to your veterinarian or supervisor. A signed copy of volunteer hours should be attached to the recommendation letters. To do this, please use the form below. Relatives should not write letters of recommendation under any circumstances. If you are working in your parents clinic, the associate vet, vet technicians or business administrator should verify your hours and write the letter of recommendation. For FFA animals raised: estimate the number of hours/ years and have your FFA teacher or supervisor sign below.
