Beef cattle (<700 lb) for control of active infection of anaplasmosis caused by Anaplasma marginale at 350 mg/h/d. Beef cattle (>700 lb) for control of active infection of anaplasmosis caused by Anaplasma marginale at 0.5 mg/lb bodyweight/d. Beef cattle and Nonlactating Dairy cattle as an aid in control of active infection of anaplasmosis caused by Anaplasma marginale delivered in a free-choice feed (only approved formulas allowed) Calves, Beef and Nonlactating Dairy cattle for treatment of bacterial enteritis caused by Escherichia coli and bacterial pneumonia caused by Pasteurella multocida in a complete feed at 500 to 4000 g/ton to provide 10 mg/lb bodyweight/d or in a top dress at 4000 to g/ton to provide 10 mg/lb bodyweight/d for not more than 5 days. Swine for reduction in the incidence of cervical lymphadenitis caused by Group E Streptococci at 50 to 100 g/ton. Breeding swine for control of leptospirosis (reduce abortions) caused by Leptospira pomona at 400 g/ton up to 14 days. Chlortetracycline & Sulfamethazine: Tylosin: VFD REQUIRED Beef cattle as an aid in maintenance of weight gains in the presence of respiratory disease such as shipping fever at 350 mg/h/d of each. Illegal to use in dairy cattle at any age. Withdraw 5 days before slaughter. VFD REQUIRED Beef Cattle for reduction of incidence of liver abscesses associated with Fusobacterium necrophorum and Arcanobacterium pyogenes at a rate of 8 to 10 g/ton. Swine for control of swine dysentery associated with Brachyspira hyodysenteriae at 100 g/ton at least 21 days then 40 g/ton until market at 45 to 90 lbs of weight gain on average. 2
3 Pyrantel tartrate: Swine aid in the prevention of migration and establishment of large roundworm Ascaris suum infections and nodular worm Oesophagostomum infections at 96 g/ton. Swine for removal and control of large roundworm Ascaris suum infections at 96 g/ton for 3 days. Swine (<200 lb) for removal and control of large roundworm Ascaris suum infections and nodular worm Oesophagostomum infections at 800 g/ton as a single therapeutic treatment at the rate of 1 lb/40 lb bodyweight. Swine (>200 lb) for removal and control of large roundworm Ascaris suum infections and nodular worm Oesophagostomum infections at 800 g/ton as a single therapeutic treatment at the rate of 5 lb/h/d. DO NOT MIX with Bentonite. WITHDRAW 24 hours before slaughter. Bacitracin Methylene Disalicylate: Growing/Finishing Swine for increased rate of weight gain and improved feed efficiency at a rate of 10 to 30 g/ton. Growing/Finishing Swine (40 to 250 lb) for control of swine dysentery associated with Brachyspira hyodysenteriae at 250 g/ton to provide 5 to 7 mg/lb. Pregnant Sows for control of clostridial enteritis caused by Clostridium perfringens in suckling piglets when fed to sows from 14 days before to 21 days after farrowing on premises with a history of clostridial scours at a rate of 250 g/ton. Broiler and replacement Chickens for increased rate of weight gain and improved feed efficiency at a rate of 4 to 50 g/ton. Broiler and replacement Chickens as an aid in prevention of necrotic enteritis caused or complicated by Clostridium spp. at a rate of 50 g/ton. Broiler and replacement Chickens as an aid in control of necrotic enteritis caused or complicated by Clostridium spp. at a rate of 100 to 200 g/ton for 5 to 7 days or as long as clinical signs persist and then reduce to prevention level of 50 g/ton. Laying Hens for increased egg production and improved feed efficiency during first 7 months of production at a rate of 10 to 25 g/ton. Feedlot Beef cattle for reduction of number of liver condemnations due to abscesses at a rate of 70 mg/h/d fed continuously. Feedlot Beef cattle for reduction of number of liver condemnations due to abscesses at a rate of 250 mg/h/d for 5 out of 30 days. Lasalocid: Feedlot cattle for improved feed efficiency, feed continuously at a rate of 10 to 30 g/ton to provide 100 to 360 mg/h/d. Feedlot cattle for improved feed efficiency and increased rate of weight gain, feed continuously at a rate of 25 to 30 g/ton to provide 250 to 360 mg/h/d. Pasture cattle for increased rate of weight gain at a rate of 60 to 300 mg/h/d. Pasture cattle for increased rate of weight gain, feed continuously free choice at a rate of 60 to 300 mg/h/d. 3

Daily Lasalocid intake in excess of 200 mg/h/d has not been shown to be more effective than 200 mg/h/d. Diflubenzuron: Beef and Dairy cattle as an insect growth regulator, which prevents the development of house, stable, face and horn flies in the manure at a rate of 0.10 mg/kg bodyweight. Amprolium: Beef and Dairy calves as an aid in the prevention of coccidiosis caused by Eimeria bovis and Eimeria zuernii during periods of exposure or when experience indicates likely hazard at a rate of to g/ton to provide 5 mg/kg bodyweight/d for 21 days. Beef and Dairy calves as an aid in treatment of coccidiosis caused by Eimeria bovis and Eimeria zuernii at a rate of to g/ton to provide 10 mg/kg bodyweight/d for 5 days. S-Methoprene: Beef and Dairy cattle for insect growth regulator for continuous feeding during the fly season to prevent the development of horn flies in the manure at a rate of 22.7 to 45.4 mg/100 lb bodyweight/mo. Neo-Terramycin: VFD REQUIRED Calves (<250 lb) for treatment of bacterial enteritis caused by Escherichia coli and treatment and control of colibacillosis caused by Escherichia coli at a rate of 10 mg/lb bodyweight/d for 7 to 14 days. Cattle for prevention and treatment of early stages of shipping fever complex at a rate of 0.5 to 2.0 g/h/d for 3 to 5 days before and after arrival in feedlots. Growing cattle (>400 lb) for reduction of liver abscesses at a rate of 75 mg/h/d. Calves, Beef and Nonlactating Dairy cattle for treatment of bacterial enteritis caused by Escherichia coli, bacterial pneumonia from shipping fever caused by Pasteurella multocida and treatment and control of colibacillosis caused by Escherichia coli at a rate of 10 mg/lb bodyweight/d for 7 to 14 days. For use in dry feeds only (mix in dry milk replacer before addition of water). Swine for treatment of bacterial enteritis caused by Escherichia coli and Salmonella choleraesuis, treatment of bacterial pneumonia cause by Pasteurella multocida and treatment and control of colibacillosis caused by Escherichia coli at a rate of 10 mg/lb bodyweight/d. 4

Swine for control and prevention of development of house flies in manure at a rate of 0.05 g/100 lb bodyweight/d. Monensin: Feedlot cattle for improved feed efficiency at a rate of 5 to 40 g/ton to provide 50 to 480 mg/h/d. Feedlot cattle for prevention and control of coccidiosis caused by Eimeria bovis and Eimeria zuernii at a rate of 0.14 to 0.42 mg/lb bodyweight/d up to 480 mg/h/d. Pasture cattle for increased rate of weight gain at a rate of 25 to 400 g/ton to provide between 50 and 200 mg/h/d or 400 mg/h/d every other day after the fifth day. Pasture cattle and dairy replacement heifers for prevention and control of coccidiosis caused by Eimeria bovis and Eimeria zuernii at a rate of 0.14 to 0.42 mg/lb bodyweight/d up to 200 mg/h/d and less than 100 mg/h/d during first 5 days. Brood cows for improved feed efficiency at a rate of 25 to 400 g/ton to provide between 50 and 200 mg/h/d. Brood cows for prevention and control of coccidiosis caused by Eimeria bovis and Eimeria zuernii at a rate of 0.14 to 0.42 mg/lb bodyweight/d up to 200 mg/h/d and less than 100 mg/h/d during first 5 days. Calves (except Veal) for prevention and control of coccidiosis caused by Eimeria bovis and Eimeria zuernii at a rate of 0.14 to 1.00 mg/lb bodyweight/d up to 200 mg/h/d. Component-fed Nonlactating Dairy cows for increased milk production efficiency at a rate of 11 to 400 g/ton to provide 115 to 410 mg/h/d. Dairy cows for increased milk production efficiency at a rate of 11 to 22 g/ton in a TMR. Component-fed Dairy cows for increased milk production efficiency at a rate of 11 to 400 g/ton to provide 185 to 660 mg/h/d. Fenbendazole: Swine for the removal of lungworms, gastrointestinal worms, roundworms, whipworms and kidneyworms at a rate of 9 mg/kg bodyweight divided over 3 to 12 days. Cattle for the removal and control of lungworms, barberpole worms, brown stomach worms, small stomach worms, hookworms, thread-necked intestinal worms, small intestinal worms, bankrupt worms and nodular worms at a rate of 2.27 mg/lb bodyweight (WITHDRAW 13 days before slaughter). Carbadox: Swine for increased rate of weight gain and improvement of feed efficiency at a rate of 10 to 25 g/ton. Swine for control of dysentery and bacterial swine enteritis and increased rate of weight gain and improvement of feed efficiency at a rate of 50 g/ton. 5
