MCL membership number, NOT PLM Number Detachment Commandant must sign The applicant must sign and date Check that all required blocks are filled out prior to printing Print, obtain signatures and dates; Make copies and send the original with copies the Kennel Dog Robber via the Pack Dog Robber, where a Pack exists.

3 Devil Dog Degree Advancement Filling out the Application for Degree Advancement correctly is instrumental in assuring that your Dogs are advanced when they are qualified. SECTION I - Applicants Personal Information. Please note, when asked for your MCL Membership number, DO NOT enter the Dogs PLM number. Make sure DEVIL DOG is checked. The Pound s Dog Robber should sign the application for advancements. Make sure the Applicant lists any physical problems that might be of concern for the initiation. Don t forget to have the applicant sign and date the Application!

4 Pedigreed Devil Dog Advancement Filling out the Application for advancement to the Degree of Pedigreed Devil Dog is identical to that of Devil Dog except for the requirement to check the degree of PEDIGREED DEVIL DOG. The Pack Dog Robber must sign the application for Advancement to PDD where a Pack exists.

5 Request for Transfer Applicant must complete the top of the transfer form. The losing pound should endorse the transfer. The losing pound can only deny the transfer if the dog is indebted to the pound or has pending disciplinary charges. This transfer is subject to approval by the membership of the gaining Pound. The Gaining Pound must submit this form with a Transmittal that lists the member through the proper channels. Keep one copy of this form for the losing Pound s records and provide one copy for the gaining Pound (Losing Pack / Gaining Pack if applicable). Forward the transfer form to the Kennel Dog Robber via the Pack Dog Robber, if a Pack exists.

6 Notice of Death As soon as possible after being informed of the death of a member, the Dog Trainer (with the assistance of the Dog Robber) should complete the Notice of Death form. A copy of the Notice of Death should be made for the Dog Robber's records. Forward immediately the copies as noted on the bottom of the form to the appropriate Officers (Pack Dog Trainer, Kennel Dog Trainer and Kennel Dog Robber) and Pack Dog Robber or, where there is no Pack, forward the Death Notice to the Kennel Dog Robber

7 Charter Application Make sure all blanks are filled in where required by the Pound: Remember, when the Charter Application is submitted it must have at least 7 signatures and 5 of them must be of Dogs who are Devil Dogs or Pedigree Devil Dogs. Fill out a Transmittal form for those members signing and state their Full Names, Degrees, Dog Tag numbers and addresses for each. The application is accompanied by a 20 Big Bone ($20.00) Payment and any dues owed by the members joining the Pound. Check or Money Order Only, do not send cash.

8 Membership Dues Transmittal (New Members) Transmittals have the capacity to list six (6) Dogs per Page All requested information must be filled in completely The MCL # is NOT the Life Membership Number (PLM) Kennel Dues for New Members are 10 Big Bones ($10.00) each New Pups being initiated between 1 March and 31 August will be assessed 5 Big ($5.00) Bones Kennel Dues. Forms submitted without the necessary fees and paperwork will be returned Pack Dues are set by the individual Packs and must accompany transmittal

11 Membership Dues Transmittal (Renewals with Reinstatement) All requested information must be filled in completely The MCL # is NOT the Life Membership Number (PLM) Kennel Dues for Renewals are 10 Big Bones ($10.00) each Kennel reinstatment fee is 2 Big Bones ($2.00) a year Maximum of 3 years or 6 Big Bones ($6.00) Pack Dues and reinstatement fees are set by the individual Packs and must accompany transmittal

12 Membership Dues Transmittal (Life Membership) All requested information must be filled in completely The MCL # is NOT the Life Membership Number (PLM) Life Memberships Dues is depends on the members age Age 61 or older - $ Age 60 or younger - $ Life Memberships must go in a separate transmittal Packs and Pounds may NOT access a Life Membership dues amount

13 Installation Report Please Print Legibly, typing is preferred. If the unit has an Incorporation Number, include the Incorporation Date and Number. Fill in ALL sections with complete and accurate information. The Installing Officer MUST Sign the Installation Report. The Pound is to retain one last copy, all other copies are forwarded to the Pack (if a Pack exists). The Pack will retain the bottom copy and forward all others to the Kennel, the Kennel will send one copy to the Division Vice Chief Devil Dog.
