2 Why Should I Purchase a Puppy Wellness Plan? Financial Flexibility: Owning a dog is a significant financial commitment, and often times a large portion of the medical costs occur within the first year of the dog s life. A wellness plan will allow you to provide your dog with recommended and necessary care while spreading the cost evenly over the entire year rather than paying large sums at one time. Preventive Care: Many common illnesses and injuries in dogs can be prevented with proper vaccinations, parasite control, spaying/neutering, and early detection of problems. Potential problems can be identified at an early age with frequent veterinary visits as a puppy and can help your pet reach its full health potential. Valuable Discounts: By purchasing a plan, you are not only getting a discount on all the standard recommended puppy care, but also a discount on any additional and uneseen treatment or testing your dog may need during the first year. For instance, if your puppy is vomiting after eating a toy, you will get 10% off the x-rays that may be necessary. Recommended preventive care with discounted fees and a flexible payment plan allows everyone to give their puppy the best start possible!

How Does the Plan Work? W hen you purchase a plan your puppy, the initial membership fee of $55 (includes the cost of bookkeeping and billing) and the first month s fee will be due.

3 How Does the Plan Work? W hen you purchase a plan your puppy, the initial membership fee of $55 (includes the cost of bookkeeping and billing) and the first month s fee will be due. You may pay this initial amount using any method, and then fill out paperwork to allow us to deduct future monthly fees from a bank account. On the 10th day of each month the next 11 months, your monthly fee will be deducted directly from your bank account. Discounts may not be combined with other discounts. If the patient dies, switches owners, or if you move away, you may either pay regular price the services already provided, or you may continue to finish the payments the year, whichever is less expensive. The plan may be cancelled at any time, any reason. However, all services that have been provided will be charged retroactively at our regular prices should you choose to cancel your plan. Unpaid accounts will be warded to a collection agency with additional fees assessed. The puppy plan expires a year from the date of purchase. We highly recommend continuing your dog s wellness plan coverage on one of our Adult Wellness Plans. In this way, together, we can continue to keep your pet in optimal health in a cost-effective way. (applies only to exam -- diagnostics/treatment will receive a 10% discount) What s Not Covered: (For the following, however, you will receive a 10% discount off the total bill) Monthly heartworm prevention after the initial dose (recommended) Any flea/tick prevention after the initial dose Vaccines that are not considered core vaccines, such as Lyme Disease Diagnostic testing any medical conditions found during the exam, such as ear infections or skin conditions, and any treatment those conditions Treatment non-routine intestinal parasites, such as giardia and coccidia Annual adult vaccines that start at 16 months of age unless pet wellness plan is renewed as an adult plan (recommended).
