As of 2014, this is now a requirement per the federal Animal Welfare Act and the USDA for small, hobby breeders. 5. What diet will you start your new puppy on? (We currently feed Fromm Grain Free dry. In addition, we feed meaty bones, other appropriate raw foods, and treats. There are a variety of excellent foods on the market it is best to avoid those with corn, wheat, or soy, as they can cause dietary issues and allergy issues with this breed. Completely raw feeding (with lots of research!) is also an excellent choice. BARF and PREY models are our favorites.) Do you plan to spay or neuter your dog? Do you plan to eventually try to breed your dog (once limitations are lifted following health clearances)? Do you plan to work towards some type of training title with your dog? Page 3 of 6

If your dog develops a personality that does not fit the standard (shy, aggressive, difficult to train, etc.), will you make all efforts to find a way to train and maintain the dog? If not, will you agree to return the dog to SAM Vlcaks. Legal Stuff Will you agree to never sell or give away your dog without prior consent and approval from Stacy McCrary? Should you decide to breed, Will you agree to only breed to Czechoslovakian Vlcak s who are recorded with the AKC or FCI member country, and have satisfactory health results? Will you agree to locally license your dog, receive regular and necessary vet care, and abide by all laws regarding your new dog, and prevent your Vlcak from becoming a danger or nuisance with proper supervision and control? Have you reviewed the legal status of this breed with your state, county, city, town, etc.? (While the Vlcak is considered a dog breed by every major registry and most localities, there are at least a few places in which it is banned, based on the breed's history. If there Page 4 of 6

5 are any questions (especially in places where "wolfdogs" and "wolf hybrids" are prohibited, with a vague description of what those are), it is worth an (we recommend getting approval in writing and keeping it for the future) to the governing enforcement body (such as animal control) to ensure this breed is legal. Most places have never heard of the breed, and it will be up to you to educate them. The Czechoslovakian Vlcak Club of America is currently working on materials to assist with educating local and state animal control agencies in understanding our breed.) Describe your family (that resides in home), and their individual interest in living with a Vlcak. For families with children (or older, or disabled family members), how will you handle the energetic, bossy and sometimes mouthy temperament of a Vlcak around your children and friends of your children? 3. Please describe your lifestyle, and how a Vlcak would be suitable within it. Include information such as hobbies, visitors to the home, travel, extended family members, etc. 4. What kind of experience do you have with dominant, active, large working breeds of dogs? 5. What other pets reside in or near the home, or visit with you, that will interact with the new puppy? How will you handle it if your Vlcak does not get along with these animals? What purpose do you have in purchasing a Czechoslovakian Vlcak (show, training, companion, breeding, etc.)? 6. What type of training do you plan to implement? What is your plan for socialization of your Vlcak puppy? How will you provide daily exercise and mental stimulation? 7.
