8 Orit Chai, 2017 SCIENTIFIC BIOGRAPHY After completing my DVM studies at the Koret School of Veterinary Medicine and a one-year rotating internship in small animal medicine and surgery at the Veterinary teaching hospital, I pursued my interest in neurology and entered a three-year residency program. The training in veterinary neurology and neurosurgery provided me with unique hands-on clinical experience as well as exposure to a wide range of research opportunities and facilities. The faculty has facilitated my professional growth in veterinary medicine and, specifically, in neurology and served as the perfect learning environment preparing for the neurology boards. Soon after successfully completing the board exams, I joined the Neurological Department headed by Prof. Merav Shamir and took a clinical instructor position at the Veterinary Teaching Hospital. As a clinician in the field of neurology, my scientific interest and work can be divided into two main fields: 1) Identification of clinical conditions and development of new methods for the diagnosis and treatment of challenging neurological and neurosurgical conditions, and 2) the use of different imagining modalities for diagnosis, screening and prognosis of neurological diseases. 1. Identification of clinical conditions and development of new methods for the diagnosis and treatment of challenging neurological and neurosurgical conditions Over the years, I have identified and described new clinical conditions and diseases in dogs and other species. These include post-anesthetic cerebellar degeneration in cats (RA#1), bite wounds involving the spine (RA#2), MRI findings in intra-spinal aberrant migration of Spirocerca lupi (CR#3), spinal empyema secondary to vertebral osteomyelitis in a cat (CR#11), polioencephalomyelopathy in a mixed breed dog resembling Leigh's disease (CR#9) and reversible paralysis after direct intrathecal morphine administration.(cr#8). I have also searched for a treatment for meningoencephalitis of unknown etiology in dogs, which is the most common inflammatory disease of the canine central nervous system (CNS). I investigated the possibility of treating these dogs with mycophenolate mofetil, in addition to steroids, and proved it beneficial in a large proportion of cases (RA#9). Another project has provided new insights into mechanisms of epilepsy, which is a common and well-described disorder in dogs and humans with around 30% of the patients refractory to antiepileptic drugs. I have collaborated with the Lily Safra Center for Brain Sciences of the Hebrew university to examine the role of the cholinergic system in the CNS of dogs with naturally occurring seizures. For that, we measured the activity of acetyl choline esterase (AChE) and its read-through isoform AChE-R in the cerebrospinal fluid (CSF) of dogs after seizure and in control groups (RA#6). A following study focused on the CNS inflammatory response in epileptic dogs by measuring the concentrations of tumor necrosis factor-α and interleukin-6 in the CSF following seizure (RA#7). Other important studies were related to spinal cord injuries. In veterinary medicine, permanent paralysis often results in euthanasia. Hence, giving the correct prognosis in cases of sever spinal cord injury is essential. In collaboration with Prof. 8

9 Shamir, we used acute intervertebral disc herniation in dogs as a model of naturally occurring spinal cord injury. We found that in these dogs, the ratio between macrophages and monocytes in the CSF samples may be an accurate prognostic indicator for regaining ambulatory status (RA#5). We then conducted a second stage of that study, in which we looked specifically at dogs that had the most severe degree of spinal cord injury, namely lost nociception sensation (RA#8), and tested the validity of our recent prognostic factor in this challenging group of dogs. In a study of Pekingese dogs suffering from intervertebral disk herniation, I showed that disc calcification on plain radiographs is a major risk factor for the occurrence of the condition (RA#10). The results of that study have established the rationale for the use of spine radiographs as a breeding screening tool to reduce the incidence of disk herniation in Pekingese dogs Lastly, I have collaborated as senior clinician in the development and establishment of several particularly challenging neurosurgical procedures in animals we have encountered during our routine work at the department. Among these, we have established a new method for intervertebral space distraction prior to stabilization combined with complete ventral slot for the treatment of disc-associated wobbler syndrome in dogs (RA#3). We described for the first time a surgical procedure, sub-occipital craniectomy, which can save captive lions affected by calvarial hyperostosis (CR#2), brain surgery and rehabilitation protocol of wild Nubian ibex that was successfully released back to nature, Taenia multiceps coenurus brain cyst removal in two wild Nubian ibex (CR#7) and, lastly, a dorsal laminectomy to relieve spinal cord compression in a captive Syrian bear (CR#12), which was the first surgery of that kind in the world. 2. Application of different imaging modalities in neurological diseases Over the years, I have developed an increased interest in neuroimaging, aiming to improve our understanding of diseases processes through advanced use of imaging techniques. I have investigated different sequences and modalities of MR images in dogs with aberrant intra-spinal migration of Spirocerca lupi. I characterized the imaging findings in these cases, thereby improving the ability to make antemortem diagnosis of the disease (CR#3). This project is now followed by a retrospective and prospective study of changes in the muscles surrounding the vertebral column that, together with the typical changes in the spinal cord, will provide exclusive imagine patterns for early diagnosis of this disease. Lately, I have examined the use of CT to evaluate head trauma patients, with aim to utilize the obtained information to determine prognosis. Based on the recorded data, I have established a unique and novel prognostic CT-based scoring index for head trauma in dogs termed Koret CT score (RA#11). I am now performing the second stage of this project, in which I plan to test this score in a double-blind prospective study. As I look into the future, my current areas of research interest are: 1. To establish the CT scoring index as an objective prognostic tool in both dogs and cats following head trauma and to further develop the score to combine both CT and clinical findings. 2. To combine MRI findings with specific CSF bio-markers and tests their specificity and sensitivity in the diagnosis of intra-spinal Spirocerca lupi. 9

To investigate the patterns of intervertebral disk calcification in Pekingese dogs with aim to determine the correlation to disk herniation and to examine the hereditability of disk calcification in this breed. My ultimate goal is to develop a selective breeding strategy based on spinal radiographs obtained at a young age. 10
