3 Abaxis partnered with Veterinary Metrics to help improve your Practice Economy CASE STUDY: Implementing an Effective Wellness Testing Program Partnering with Vet Metrics truly made a difference in building our Wellness Tes ng Program. They made it easy for my staff to understand the importance of the program and effec vely communicate it to our clients. We plan to con nue our engagement with Vet Metrics to further reinforce the message in new and innova ve ways. Dr. John Talmadge Bigger Road Veterinary Clinic The Bigger Road Veterinary Clinic is one of Veterinary Metrics, Inc s most sa sfied customers and for good reason. Their partnership with Vet Metrics recently helped shape the founda on of the prac ce s Wellness Tes ng Program with data and marke ng services so effec ve that on and annualized basis the Bigger Road Veterinary Clinic earned over $200,000 in incremental Wellness Tes ng Revenue. Step 1: Planning & Analy cs Vet Metrics conducted a comprehensive Prac ce Performance Analysis on client/pet data and found that the Bigger Road pet popula on was disparately aged versus the average vet prac ce. The analysis also iden fied declining numbers of new client acquisi ons. The prac ce enlisted Veterinary Metrics to target both service areas. Step 2: Staff Development Vet Metrics immediately expressed the importance of staff development and its role in the success of a prac ce. It is cri cal to ensure that all layers of staff have a clear understanding of the preven ve health significance of wellness tes ng and that they know how a wellness program flows through all touch points of the client/pet visit. Step 3: Internal Study and Addi onal Planning To further illustrate this point, all staff members pets received age appropriate wellness tests. Results showed that 25% of their pets had some hidden abnormality. Once the test results were shared the en re prac ce began to grasp the importance of wellness tes ng. 4 Implementing an Effective Wellness Testing Program Step 4: Launch Prepara on Bigger Road was able to lean on their Veterinary Metrics analysis and planning to develop a powerful, structured approach to their roll out. The prac ce methodically followed Vet Metrics wellness tes ng implementa on module throughout the introduc on of this program. Step 5: Educa on Process for Clients At the launch of the Wellness Tes ng Program, the en re staff made a point to communicate the importance of the program during visits by all pets mee ng the program criteria. Scripts provided by Vet Metrics that focused on benefit statements were refined to further tailor the approach to their clients and op mize compliance. Conclusion Results speak louder than words. In this case, the prac ce has accomplished the following through April of this year: Jan-April 2008 (versus 2007) Wellness Tes ng Performance in the prac ce has generated $67,000 in incremental wellness tes ng revenue and achieved 34% wellness tes ng conversions (percentage of wellness exams also receiving wellness tes ng). Based on historical Veterinary Metrics data, your prac ce may see similar results depending on the type of programs and services you elect to pursue. 8 Diagnostic Case Study Diagnostic Case Study: VetScan Aids in Cadillac s Diagnosis and Recovery Contributing Authors: Barry Littell, DVM and Deb Littell, DVM Riverside Animal Hospital, Kearney, Nebraska Case Study winner! Cadillac the Airedale Lab Cadillac, a 13 year old, 77 pound, neutered male, Airedale/Lab mix was presented for a routine dental cleaning and vaccinations on February 23, Pre-anesthetic blood work is required in our practice for patients more than 6 years old. The minimum requirement is Complete Blood Count and a routine wellness screening using the VetScan Prep Profile II which includes the following analytes: ALP, ALT, BUN, CRE, GLU, TP. Cadillac s owner agreed to a VetScan CDP chemistry profile which includes the following analytes: ALB, ALP, ALT, AMY, BUN, CA, CRE, GLOB, GLU, K+, NA+, PHOS, TBIL, TP as well as a complete CBC. Physical examination revealed a dental grade of 2/4, and a body condition score of 3/5. A firm, round abdominal mass was also palpated and a splenic mass was suspected. The results of Cadillac s CBC on our VetScan HMT was within normal limits. The Comprehensive Diagnostic Profile test, performed on our VetScan VS2 chemistry analyzer was also within normal limits for a senior dog. Abdominal x-rays and an ultra sound were performed to verify a very large mass of Cadillac s spleen. Next, a fine needle aspirate of the spleen was sent to Colorado State University (CSU) for their cytological interpretation. I suspected a hemangiosarcoma visualizing several vacuolated cells, and recommended a Spleenectomy. CSU s FNA interpretation showed possible hematoma. On March 3, 2009 Cadillac weighed 76 pounds, and his pre-anesthetic exam was unchanged and he appeared stable to undergo his Spleenectomy. He was pre-medicated with 100mg of Tramadol orally, and 3.1ml Cefazolin IV. Cadillac was given IV induction anesthesia and entubated/maintained on isoflurane/02. Comprehensive Diagnostic Profile: At 10:52 am, my team performed an abdominal exploratory surgery, we identified the spleen with a large round mass on the tail and while I suspected a second mass of the spleenic head from the ultrasound. Pre-surgically we found a large mass of what appeared to be mesenteric lymph node located at the base of the mesentery. This mass was isolated and removed, as well as the spleen. With further exploration, we found no other masses and we flushed Cadillac s abdomen with one liter of warm Physiological Saline Solution. We performed a routine 3 layer closure, ending the surgery at 1:14 pm. Thirteen mg of Torbutrol was given SQ for pain management. The patients recovery was uneventful and all tissues were submitted April / May
9 Diagnostic Case Study continued to Colorado State University (CSU) for histopathology. Cadillac was maintained on IV fluids, and pain management of Tramadol 100mg BID-TID, and Butrophanol as needed. We performed another CBC on our VetScan HMT before releasing Cadillac, and the results showed a high white cell count, which was to be expected following his abdominal exploratory surgery and normal red blood cell count. Cadillac was sent home the following day on his medications. Complete Blood Count: Complete Blood Count: Complete Blood Count: CSU diagnostic laboratory reports, there are no features of malignancy within two sections of spleen examined and further sections are similar. Spleenic hematoma and extramedullary hematopoiesis is the first tissue diagnosis. The mass lesion is quite unusual and appears to be either involving a lymph node or kidney. As per history this did appear to be involving the kidney, at least grossly, as two kidneys were identified surgically. This could be arising from ectopic renal tissue. There is an extensive amount of necrosis present, but there does appear to be portions of a remnant tumor centrally. Further sections to better identify this mass confirm a well differentiated renal tubular adenocarcinoma. CSU oncologist relayed that there is not allot of good literature regarding this tumor type and only a handful have been diagnosed in veterinary medicine; and in humans chemotherapy is not highly effective. Cadillac was presented on March 9, 2009 to recheck his CBC, and his owner reported that he was having some issues getting up on his own. At this visit, Cadillac weighed 69.8 pounds. The results of Cadillac s CBC were much improved and back to normal for a senior dog. He was released on Previcox 57mg and we recommended acupuncture treatment for pain management. April / May
10 Diagnostic Case Study continued Currently, Cadillac is healing well and undergoing Acupuncture Treatments with Dr. Barry Littell. Dr. Barry also prescribed Chinese herbs, to aid in Cadillac s recovery from surgery. In addition, Cadillac has begun treatment with Piroxicam once daily and hopes that this NSAID with indirect anti-tumor activity, may discourage rapid tumor regrowth. In conclusion, our practice would not be the same without our Abaxis VetScan Chemistry and Hematology analyzers. There is not a day of the week that we do not use them. In special cases such as this one, we used the VetScan HMT three different times within a short period of time on the same patient and we used the VetScan VS2 Chemistry Analyzer as well. We are pleased with the success of our Wellness Packages, which include Complete Blood Count, Comprehensive Diagnostic Profile and T4 test. If we identify liver concerns, these patients receive therapy and monitoring with the Mammalian Liver Profile. At Riverside Animal Hospital in Nebraksa, in spite of the current economy our practice revenues continue to be bright, greatly in part to our ability to perform accurate, reliable and timely in-house diagnostics. 12 Special Look: Borneo Orangutan Survival Society Borneo Orangutan Survival Society: VetScan Helps Endangered Species Contributing Author: Barbel Kohler, Abaxis, International Sales & Marketing Manager Lone Droscher-Nielson, Founder - BOS The BOS in Borneo and Abaxis VetScan equipment is critical to the survival of the one of the most endangered species in the world, the Orangutan. In the April/May 2008 issue of VetCom we gave you a short overview of Abaxis collaboration with the Borneo Orangutan Survival Society (BOS), one of the best clinics in the world dedicated to orangutan rescue and survival. Due to the exciting feedback on that story I would like to update you on the BOS project based on my own experience from December BOS is located in Borneo, Indonesia, and provides for all aspects of orangutan care from rescue to emergency care to eventual reintroduction into the wild. Because of deforestation and poaching, the orangutan population in Borneo has gone from 13,000 to 6,000 in just 6 years. BOS is helping to stop this terrible trend and restore the orangutan to its rightful place in the Indonesian habitat. Abaxis was introduced to BOS by the World Society for the Protection of Animals (WSPA), which purchased a VS2 and HM5 to donate to the clinic. Since then, Abaxis has been happy to provide BOS with all the reagents they need free of charge, and will soon donate a second chemistry analyzer to this expanding clinic. Given that my background in laboratory and emergency medicine and a keen interest in exotic veterinary medicine I was excited about the opportunity to work with these beautiful animals. I used my vacation time to come to BOS Nyaru Menteng to train the staff in the best uses for the VS2 and HM5, as well as best medical practices in general. When I arrived, I was very impressed at the comprehensive care that BOS provides. BOS saves orangutans that are trapped in the plantations that are encroaching daily on their habitat. It nurses them back to health with all methods of treatment necessary including emergency surgery, laboratory medicine, oxygen therapy, malaria therapy and any other treatment necessary for re-introducing the orangutan into its natural habitat. BOS was grateful to receive training from an outside expert especially in the use of the VS2 and HM5, both of which are vital to their practice. Before the VS2 and HM5, BOS had to send blood samples to a laboratory which was 80 km away delaying crucial treatment decisions. It now is able to get comprehensive chemistry and hematology results at the point of care; and indeed, it has both instruments running 24 hours a day. This has led to a much more efficient practice as well as better patient care, thanks to the much faster availability of diagnostic information. When it comes to chemistry analysis, the veterinary team usually uses the Comprehensive Diagnostic Profile, which includes ALB (with a normal range of Orangutan G/L in the orangutan), ALP ( U/L), ALT April / May

They usually collect blood through venipuncture in the arm. Infusion is done through the calf. Collecting blood from infants can be extraordinarily difficult; however, given that the VS2 requires only two-drops of whole blood, the veterinary team can obtain diagnostic information for these important patients as well.
