SOTC IS OFFERING A WAIT LIST to fill openings made by withdrawn entries after the trial s losing date of April 19, 2019, at 6:00 p.m. A maximum of ten (10) entries will be plaed on the lub s wait list. All entries, inluding wait listed entries, must be reeived by the Trial Seretary by 6:00 p.m. on April 19, Any entry that is withdrawn by 6:00 p.m. on Monday, April 29, 2019, will be replaed by a wait listed entry if any of the ten (10) are available. If a withdrawn entry is replaed by a wait listed entry, a full refund will be issued for the withdrawn entry. Any wait listed entry that does not move off the wait list into the trial by April 29, 2019 at 6:00 p.m. will reeive a full refund.

For the first 48 hours the trial is open, limits are set at 520 paper and 180 on-line entries. ELIGIBILITY & CLASSES DOGS ELIGIBLE TO ENTER: This trial is open to all dogs fifteen (15) months of age or older that are registered with the Amerian Kennel Club or that have AKC Limited Registration, Purebred Alternative Listing/Indefinite Listing Privileges (PAL/ILP) or an AKC Canine Partners listing number, or approved Foundation Stok Servie (FSS) breeds. Bithes in season, aggressive dogs, dogs that are blind or deaf or suffering from any deformity, injury, or illness whih may affet their physial or mental performane, are not eligible to partiipate. Mixed breed dogs that have been listed with AKC Canine Partners may partiipate provided their listing number has been reorded on the entry form. All weaves will have 24" spaing. All ontats are rubberized. The new breakaway tire will be used. HEIGHT DIVISIONS: Judges or VMO will measure any dogs that do not have valid height ards. 4 AKC AGILITY MOVE-UP FORM SOTC Agility Trials May 3, 4 & 5, 2019 Move-ups are subjet to the availability of runs in the higher lass. ALL move-up requests: must be reeived by the Trial Seretary no later than 6:00 pm, Monday, April 29, Day-to-day move-ups: must be reeived by the Trial Seretary no later than 1/2 hour after the last run on the previous day. USE THIS FORM TO MOVE UP TO A HIGHER AGILITY CLASS ONLY It is the option of the lub to allow day-to-day move-ups. Exhibitors should refer to a lub s premium list to verify the lub s poliy for moving a dog up. If offered, dogs shall be moved up to a higher lass at the request of the owner, as a result of the dog qualifying for an agility title. The request for moving up must be made to the Trial Seretary aording to the lub s published poliy. All move-up entries are subjet to availability of the lass and payment of any differene in entry fee. Dogs so moved-up will be entered at the end of the running order. Refer to Chapter 1, Setion 20 of the Regulations for Agility Trials for additional move-up requirements. BIS: Any bithes in season with a Veterinary ertifiate, will be awarded a refund, less $2.50 proessing fee per lass. Please notify seretary prior to the start of the trial. INJURED DOGS an get a 50% refund of entry fees. Required doumentation must be supplied prior to the start of the trial. SICK DOGS an get a 50% refund of entry fees. Any dog diagnosed, prior to the first day of the trial with kennel ough, K9 influenza or any other ontagious disease and any other dog domiiled at the same residene, even if they are not yet affeted, will reeive a 50% refund if requested. Required doumentation must be supplied prior to the start of the trial. No entry should be sent whih requires a delivery signature. Entries reeived prior to the opening date will be returned to sender. Exhibitors are responsible for any errors in making out their entry forms. No entry fees shall be refunded in event of suh errors or anellations of entries after the trial loses. Aknowledgement of entries will be made as soon as possible by the seretary if you inlude an address or stamped self-addressed envelope. All move-ups will be aknowledged by the seretary via . Dogs entered in the Preferred lasses must ompete at the required jump height and may not ompete in a jump height division higher or lower than their proper height division. All dogs that have not been offiially measured for an Agility Jump Height Card may still ompete; however, they must be measured by a Judge of reord, an Agility Field Representative, or a Volunteer Measuring Offiial, prior to running. If measured by the Judge of reord, that measurement will be valid for this trial or bak-to-bak trials only. If a measurement is neessary, it is the exhibitor s responsibility to have their dog(s) measured prior to running. Dogs listed in the AKC Canine Partners SM program may be eligible to enter all-breed AKC agility trials at the lub s option. These dogs should be listed as All Amerian Dog and must inlude their AKC number on the entry form and hek the AKC No. box. Junior Handlers should enter their Junior Handler number on the front of this form. I (we) agree that the lub holding this event has the right to refuse this entry for ause whih the lub shall deem suffiient. I (we) ertify and represent that the dog entered is not a hazard to persons or other dogs. In onsideration of the aeptane of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving lub, their members, diretors, governors, offiers, agents, superintendents or event seretary and the owner and/or lessor of the premises and any provider of servies that are neessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any laim for loss or injury whih may be alleged to have been aused diretly or indiretly to any person or thing by the at of this dog while in or about the event premises or grounds or near any entrane thereto, and I (we) personally assume all responsibility and liability for any suh laim; and I (we) further agree to hold the aforementioned parties harmless from any laim for loss, injury or damage to this dog. INSTRUCTIONS Handlers whose dogs measure into the 8 through 20 regular jump height divisions may opt to enter in a higher or 24-hoie height division for all Regular titling lasses, but not in a division lower than their proper height division. Dogs who measure into the 24 regular jump height division may not enter 24-hoie. Dogs entered in the Preferred lasses must ompete at the required jump height and may not ompete in a jump height division higher or lower than their proper height division. All dogs that have not been offiially measured for an Agility Jump Height Card may still ompete; however, they must be measured by a Judge of reord, an Agility Field Representative, or a Volunteer Measuring Offiial, prior to running. If measured by the Judge of reord, that measurement will be valid for this trial or bak-to-bak trials only. If a measurement is neessary, it is the exhibitor s responsibility to have their dog(s) measured prior to running. Dogs listed in the AKC Canine Partners SM program may be eligible to enter all-breed AKC agility trials at the lub s option.
