Safety News Risk Control Improvement highlight: CHS Nutrition, Great Falls, MT An issue that we see come up many times is how to get information to our employees in an effective efficient way. We can t always stop what we are doing and gather together in a room every time we need to share something. Sean and Matt at our feedmill in Great Falls recently completed an initiative that helps address this issue. They installed Electronic monitors in key areas of the facilities, such as the warehouse, lunch room scale and control room, that can be used to convey various types of information. Some examples of the types of items that they are displaying are safety messaging, weekly briefs, scheduling information, production numbers, even weather forecast info. w this is not a solution for every location and would certainly fit better in some over others, but it is a great example of coming up with a way to address an issue we all face. Thank you to the team at Great Falls for taking the initiative to increase safety awareness with employees. Page 1 of 10

6 Injury/Illness Incidents: (September 16 th October 15 th ) Incident Description Medical OSHA While loading and moving rail cars, employee stepped into a badger hole and twisted his knee. Medical Treatment Employee bumped left elbow while descending ladder into boot pit. While entering truck, employee's foot slipped off top step & fell backwards, landing on elbow. Employee was raising and lowing equipment for repair, when he got a rope burn on his hands. Employee was stung by a bee and had an allergic Reaction (he had epi pen with him). ER Treated And Released Other Case Driver was passing farm truck going south on highway, he ended up driving in the ditch and field. Hospitalized < 4 Hours Other Case Employee was repairing a drag when he hurt his wrist. Employee was filling cylinder and felt something pull or burning in right shoulder-arm. Employee was putting brake pads together & smashed finger between rollers. While cutting straps off seed pallet, employee lacerated left thumb. Employee felt rib pop out and back in, rib was sore afterwards. Employee stepped on the edge of the sidewalk and twisted his ankle. Employee reached for bag on upper racking, back foot caught on plastic employee fell on back and bottom, hitting elbow. Chaff in right eye. Other Case Employee was climbing down fuel tank ladder with fuel hose and pulled his groin. Driver pulled muscle in chest rolling tarps open on hopper trailers. Employee was blowing out the hopper and the lid closed on his head giving him a small cut. Employee was vacuuming water when he turned and hurt his back. Employee attempted to lift a junk mower at the scrap yard, and hurt his back. Employee sustained head injury after contact with metal attachment on pay loader. Helping unload tires and slipped on wet pavement, landing on his hands and straining his little finger. Page 6 of 10
7 Incident Description Medical OSHA Employee sustained lower left leg injury from contact with cab step, while exiting and slipping off vehicle cab steps. Employee slipped climbing off a propane truck and twisted his knee. Metal rack leg landed on top of employee's foot. ER Treated And Released Restricted Time Coming down stairs and hit knee on handrail mount. Employee sustained knee injury after attempting to climb steps on to equipment. Employee cut his hand while moving material. Fertilizer was splattered in employee's eye. Employees were spraying a field when a fire started. Employee received minor burns to hand. Employee felt ill and had increased heart rate and supervisor took him to ER. ER Treated And Released Bin sweep got hung up and employee was trying to help it and his glove got caught in wheel and it ran over his hand. Other Case Employee was stung by bee while in the office. He is normally allergic so went to ER. ER Treated And Released Other Case Employee standing on the front tire of a semi cleaning windshield slipped and hit his knee on rim. Lost Time Employee was working on a farm tire and while airing it up it blew off the bead of rim. Hospitalized < 4 Hours Lost Time Employee was closing bottoms on a train when he slipped and fell, hitting his elbow. Employee was moving a pipe and got a sharp pain in his lower left part of his back. Other Case Employee tripped over boxes placed in work area during an office clean-up event. ER Treated And Released Employee was getting out of the yard truck when he felt something pull in his left lower back. Other Case Working on a truck, removing a foot valve and the part penetrated through the right thumb slightly and made a cut. When lifting a fertilizer tank onto its side employee lost footing and tank came down on fingers breaking tip of one. Other Case Employee sustained skin abrasion on hand while operating a powered angle grinder. Shoveling wet heavy snow off the loading dock. Inhaled grain dust. Employee fell off forklift while performing an unnecessary task, fracturing his collar bone. Lost Time Page 7 of 10
8 Incident Description Medical OSHA Particle of debris entered left eye while cutting a piece of plexi glass. Employee suffered minor burns to her arm. Walking in yard had a nail go through shoe into left foot. Walking in shop & felt sharp pain in lower back. Had just been lifting approx. 10lbs. Medical Treatment Other Case Employee tripped on foot of manager's chair in the office and fell down. Employee was climbing down ladder, missed last step and fell. Employee was working from a ladder, manipulating a tarp, when he slipped and fell. medical treatment. Employee strained right shoulder while moving soybean seed Pro-boxes. Employee was filling a nurse tank, when he got sprayed in the face with anhydrous. He was wearing all his PPE. ER Treated And Released Other Case Employee was closing the boxcar door and cut the back of his left hand. Employee hit his head on a pipe/spout clamp resulting in a bleeding injury. Employee was throwing garbage into dumpster. When he turned, foot slipped into washout and felt a slight pop in knee. Livestock Gates fell on foot. ER Treated And Released Lost Time During normal operation, an employee strained a tendon in his right foot when climbing a ladder. Employee strained rotator cuff at work. ER Treated And Released Employee had foreign object in eye. ER Treated And Released Employee strained back lifting & pulling 40 lb. Bags out of a company pickup truck. Lost Time Page 8 of 10

We are compared to other carriers that are similar in the number of miles driven and number of power units. This month will not contain the breakdown of CSA violations, as RAIR (the third-party company we use for our CSA data) is unable to access our roadside inspection information. Page 9 of 10
