Chairperson s Report Maureen Wood Maureen thanked the co-ordinators for their ongoing work. The Open day at the Waitakere Hospital proved to be very successful, we will continue with this initiative. A highlight over the last year is the 24/7 operation of the ED at Waitakere Hospital. Current reviews on the operation of the ED will be continuing, with WHL being included in that review. Pamphlets produced by WHL were distributed to educate the public on when to approach the ED, rather than visiting doctor or medical centres. Thanks given to Tracy McIntyre in continuing the operations of WHL, through a difficult period. Maureen Wood moved that her report be accepted. Seconded by Linda Cooper All in Favour Carried 4. Treasurer s Report Gary Sutcliffe Gary spoke to the financial report for year end 30 June Savings made in co-ordinators salaries through a period where staff had left and the Executive helped fill gaps, co-ordinator kept costs down, thanks given to Tracy McIntyre for prudent management of operating costs. Suggestion by Member to increase subscription cost to $7.50, discussion would go to Executive for decision. Gary Sutcliffe moved that the financial report be accepted. Seconded by Pam Johnson All in Favour Carried 5. Co-coordinator s Report Tracy McIntyre/Tracy Kirkley The co-coordinators tabled their report which reflected a number of goals and outcomes achieved for the community in the last year. Imelda from WDHB thanked WHL and said that they are leading the way in their Health Literacy review work and have recently won an award for their work. Tracy McIntyre and Tracy Kirkley took their report as read and asked for questions. Maureen thanked both. Maureen Wood moved that the coordinators report be accepted. Seconded by Grace Ryu All in Favour Carried 6. Election of new members to the Executive Committee. Nominations were received for the position of Community Representatives on the Executive committee as follows: Gary Sutcliffe, Kay Lindley, Grace Ryu, Linda Cooper, Awa Hudson, and David Lui. All community reps present briefly introduced themselves to the members. No election was necessary. Thanks given to Bonnie Dowding for her contribution to the Executive team, Bonnie is standing down. Presentations: Guest Speaker Robin Youngson spoke about his recently published book Time to Care Robin shared some of his research on why compassionate caring is so important in the care and wellbeing of patients and also the staff who work in the health sector. Robin played a short movie Hearts in Healthcare -champions for compassionate care. Robin shared his personal story of his daughters time spent in hospital after severe neck injuries. Simple acts of kindness from various staff made all the difference in their experiences at this time. Clinical research shows that when professionals connect compassion into the way they treat patients the benefits are both far and wide for the patient and the carer. Robin has begun a network for those who are like minded about bringing care and compassion back into the practices in healthcare. Discussion and comments from the members. 7 Chairperson s Report 2013 Another very busy, interesting and rewarding year as Waitakere Health Link continues to be involved in the health and community sector. WHL completely reviewed the constitution during the last year and a Special General Meeting was held to ratify the changes. Thank you to all those members who attended that meeting. Congratulations to our co-ordinators who entered two posters in the WDHB Health Excellence Awards and with the Keeping it Simple poster won the Excellence in Integrated Care Poster. WHL supported a request for funds from the Three Harbours Health Foundation for a Sim man. This was accepted and the SimMan Essential purchased. He will be used at the Waitakere Hospital Simulation Centre to teach the core skills of airway. Breathing, cardiac and circulation management, he has multiple pulses and visibly breathes and stops too, blinks his eyes and can realistically bleed. Our executive committee was invited to watch a demonstration which was amazing. The NGO forums and NGO Open Days at Waitakere Hospital are continuing and with the feedback from all involved are still proving to be very worthwhile. With the identification of a high prevalence of Rheumatic fever in the West the coordinators have been supporting the call for an equitable share of the funding for the school based throat swabbing programme. This was rolled out in two West schools in May. Other highlights this year have been the launching of Health West Youth Health Hub, a very worthwhile initiative consisting of several programmes; Specialist Youth health clinic School based health services and health clinics Your choice programme and Young Dads programme. Another was the opening of the Integrated Family Health Centre (Totara Health) in New Lynn, an amazing centre for general practice health with the added bonus of some secondary health services as well. For them to also have the ability to have on site diagnostic services which are affordable to the community would complete the excellence of this centre. Unfortunately the funding is not there but I feel this is another area in which WHL should lobby. I attended the opening of the North Shore hospitals new Elective Surgery Centre, an awesome facility of which the WDHB are justly proud. I was involved in the Waitemata & Auckland DHBs combined disability strategy. This was a very positive forum with many people from all sectors of the disability area. I wish to acknowledge Dr. Jonathan Simon a GP in West Auckland for the excellent work put into the West Auckland Health Network which was his initiative and which involved General practices in the West. Cluster groups and locality areas were identified. A new larger group has been formed, the Locality Establishment Governance Group (LEGG). This is chaired by the new CEO of Auckland District Health Board. The group includes the two DHBs (Waitemata & Auckland) and the five PHOs in these two districts, plus many other participants. The two Health Links are also part of the group. There will be a locality approach to health services and two areas have been identified to pilot the scheme. I wish to thank Dale Bramley, WDHB CEO, for his openness with Waitakere Health Link and for listening to what we have to say and responding to comments and queries. Thank you to all the people we work with in the community, Primary health and the DHBs. To our two coordinators who continue to carry out their roles so efficiently attending many meetings, producing excellent newsletters and being watchdogs for the health issues in West, a very big thank you. Thank you to our Deputy Chair, Kay Lindley for her ongoing support to me and the coordinators. Thank you to the entire executive for continuing to support and contribute with such an extensive knowledge of community, health and ethnicity to Waitakere Health Link. 8 Treasurer s Report 2013 Income WAITAKERE HEALTH LINK Financial Report for the Year ended 30 June 2013 (All figures below are GST inclusive) DHB Funding $ 48,939 Grants $ 33,621 Interest Received $ 0 Membership Fees $ 415 Donations $ 175 Total Income $ 83,150 Total Expenditure $ 77,690 Net Cash Surplus* $ 5,460 (*Net GST paid $2,111 effective net cash surplus = $3,349) TREASURERS REPORT We carried over $15,722 from previous year which has helped to sustain us through another difficult (albeit an improved position from 2011/12) financial year. The main challenges continue to be in securing grant funding from community and gambling-based trusts such as the Lion Foundation, Lotteries Grants Board and the Trusts Community Trust. However the latter two organizations continue to be strong supporters of Waitakere Health Link. This financial year, the coordinators secured grant funding of $33,621 which was more than 90% of a fairly ambitious income budget at the beginning of the year. In terms of unbudgeted/over-budget expenditure, there were only computer-related costs associated with upgrading our equipment that exceeded budget. All other areas were either on or under budget so again a BIG thank you to the two Tracy s for their prudent management of operating costs. Overall, we finished the year with a net positive variance of about $5,500 against budget. This report has been written prior to receipt of our audited accounts for the year ended 30 June There will no doubt be some adjustments to the figures quoted above and the financial reports (including the fact that the audited Financial Reports will be GST exclusive whereby our reports and the figures above are GST inclusive). As I will not be able to attend the AGM on 10 September, I am happy to answer any questions members may have before or after the event. In the meantime, I am pleased to move that, subject to any balance day adjustments that our Accountant may make, the Annual Accounts presented at the AGM be approved. 9 Coordinators Report 2013 The biggest success of the past year would have to be winning a Waitemata Health Excellence Award in the category of Excellence in Integrated Care for our health literacy poster Keeping it Simple. Consumer Health Literacy Reference Groups The Consumer Health Literacy Reference Group continues to meet monthly and feeds back on a variety of patient information and any relevant feedback needed by Waitemata DHB steering groups. Health Link North is now providing a monthly Reference Group meeting and both groups are only just managing the workload now it has doubled. Thank you to health literacy consumers for their tireless efforts in providing feedback and intelligent input into the Health Literacy review work, of which Health Link continues to be in demand for. Ron Paterson s Quality Review Waitakere Health Link received very favorable mention in the Quality Review report prepared by Professor Ron Patterson for the Waitemata DHB. Professor Patterson highlighted the range of work we do, with mention of our very impressive health Literacy work which resulted in our Awhina Award won November Professor Patterson has recommended that the DHB continue to support and promote the work of the Health Link Groups. Rheumatic Fever Waitakere Health Link highlighted the alarming statistics from a Waitemata DHB agenda in August We continued to bring the figures to the forefront by informing local Ministers of Parliament, Ministry of Health, and Health Providers in Ranui. The result of that was the Ministry of Health released more funding to address this disease. NGO Open Days at Waitakere Hospital We continue to provide the NGO Open Day at Waitakere Hospital which continues to be well attended and attracts new organizations in the west. Feedback from organizations and hospital staff is always positive and indicates the need to continue providing these opportunities. NGO Health Network Forums These forums are possible with the support from Procare Health Limited, and continue to grow in support with well attended forums and relevant health sector topics of Smoking and Related Diseases in August 2012 and Locality Planning in March We had representatives from Auckland Council and Unitary Planners attend, bringing together those outside the Health Sector who were in the planning process. The Foundation Sim man was the result of Waitakere health Link requesting the funding from the Three Harbours Health Foundation to purchase this fabulous new technology for the Awhina Campus. Sim Man has been a valuable addition to the training practices for Awhina students and hospital staff. Sim man has met the Prime Minister, local MPs and various dignitaries from MOH who have been impressed by his array of abilities. His realistic reactions to various simulations help the Awhina trainees in their learning s. Sim Man is based at the Waitakere campus only. A true Westie! Babies out West Brochure This brochure continues to be in strong demand and stocks have run out. We have finally secured funding for the reprint and should be launched for the Toddler Day Out in March Waitakere Community Networks WHL continues its involvement with other health and social service organizations and networks in the Waitakere community, sharing information and contributing to network meetings, including: WAVES Waitakere Anti-violence Essential Services, CSG Collaboration Strategy Group, NOW New Out West, GGE Greater Glen Eden, Community Waitakere, TANI The Asian Network Trust, Massey Community Network, Ranui Community Network, Community Houses and Hubs, Youth Connections, Middle Eastern Workshops, Auckland Transport, Auckland Council, MSD, Westgate Town Planning, Auckland Disability Law, Health west and Westfono. 10 Waitemata District Health Board Imelda King, Community Engagement Coordinator continues to increase access for the consumer and community voice into the Waitemata District Health Board, and provides access to staff and a mutual dissemination of information. While we continue to attend Waitemata District Health Board meetings to keep updated on current issues and highlight gaps in services, we also: Meeting with Waitemata District Health Board CEO We continue to meet with Dale Bramley at regular intervals to discuss issues and projects. This continues to provide our organization direct access to WDHB at the highest level to address issues that are operational in nature. Dale thinks very highly of the Health link organizations, and we have his full support in our advocacy work. Consumer Participation on Steering/Advisory Groups Waitakere Health Link has participated or been represented by consumers on the following groups: Funding and Planning and Community Engagement Forums, Bowel Screening Pilot Steering Group, Smoke free Site Advisory Group, Awhina Health Excellence Awards, West Locality Planning, Locality Establishment Governance Group, Chapel Project, Car Parking Policy Group, Patient and Family Centered Forum, Rheumatic Fever Group, ED Urgent Care Group, Child Health, Diabetes Work stream and External Wayfinding working to provide direct input into health care planning. Consumer Participation in Workshops Waitakere Health Link has participated or been represented by consumers on the following workshops: Funding and Planning Workshop, ADHB Primary Care Planning workshop, Health Quality and Safety Commission and ADHB Health Link Central workshop. Consumer Participation on Focus Groups and Education Waitakere Health Link has provided focus groups for the Eligibility Service, Pharmacy Long Term Conditions, Home Based Services, Disability Strategy and supporting consumers in education for Long Term Conditions, Albany School of Health and Social Services, Massey University and Feedback on Chronic Conditions, health science lecturer, AUT. Information Gathering from WDHB Services We continue to invite WDHB staff to attend Waitakere Health Link executive meetings to update members on the latest initiatives and projects in health. The members received updates from: Waitemata District Health Board on: Access for Mental Health Patients at Waitakere Hospital, Funding and Planning Manager, Waitakere Maternity, Awhina Emergency Department survey and Primary Care: West Auckland Localities Operations Group, Procare PHO. We continue to keep the community informed through our website, Facebook, bi-monthly newsletter and e- news and the media. We engaged with a local High School Rutherford College for the logo design project. This resulted in our new look. Our thanks to Carolyn Shaw and Sky Kim - Art teacher and Year 11 student. Our Thank yous We are grateful to the funding bodies mentioned earlier in the report that enable our organization to remain autonomous and help us to promote the consumer voice. To our champions, Maureen and Kay. Huge thanks for their ongoing commitment in health advocacy across many sectors. They continue to ensure Waitakere Health Link remains autonomous and continues to be the rock in the ever changing tides of healthcare. We would also like to thank the Executive Committee for ongoing support of our roles and to Waitakere Health Link. Our thanks to WDHB board member Sandra Coney in her governance role and Imelda Quilty-King who encourages community engagement from within the WDHB. 11 Reports from Entitled Members Being on the Waitakere Health Link Executive has made me appreciate how lucky we are out West to have such a dedicated team speaking up for health consumers needs and providing a community voice to the Waitemata DHB. The Health Link has done terrific work in health literacy, increased awareness of NGOs to hospital staff and advocated for funds to buy the invaluable Sim Man for training. Waitakere Health Link provides a great model that does not exist in most other parts of Auckland. Sandra Coney, Waitemata District Health Board Member The power of a collective community voice should never be under-estimated in terms of its ability to influence and effect change and this is perfectly demonstrated by the achievements of Waitakere Health Link. Over the years, this important group has acted on our behalf to make sure the health sector has been responsive to our community s needs. It has insisted that tangible results have come from strategic goals in such a way that the health and wellbeing of our people is represented at the decision-making table. There have been successes - the recent addition of elective surgery at Waitakere Hospital is among them - but vigilance will be needed to keep and expand existing services, and I will continue to push for our hospital to be given the regional status it needs in order meet the health needs of rapidly growing west Auckland. Vanessa Neeson, Chairman, Henderson-Massey Local Board, Auckland Council Primary Care developments 2013/14 is a year of progress for the development of primary care and integration across services. This is being demonstrated by the amazing levels of engagement with Waitakere Health Link s NGO forums which are supported and funded by ProCare. The most recent forum included an excellent and engaging session on locality developments in the Waitemata and Auckland districts. Both Waitemata and Auckland DHBs are focusing on developing localities to support the integration of services across primary care. The sector is already making progress as evidenced by the opening of the amazing New Lynn Health Centre earlier in the year. This will further improve health care and support for the local community, particularly in regards to national health targets such as heart and diabetes checks. For example, across the ProCare network an additional 63,983 eligible patients were screened for cardiovascular and diabetes risk in the period between January and July 2013, with 151 practices achieving or exceeding the Ministry of Health s screening target of 75% of eligible patients. The network has undertaken just under 136,000 interventions across national health targets since December 2012, helping more than 105,000 patients gain a better understanding of their health. ProCare is committed to achieving great results in these important population and preventative health programmes. We have been working towards a person/patient centred approach, as opposed to a disease centred one, since this allows us to be more efficient with scarce time resource available to our health workforce. It also tends to target the individuals overdue for the most screens and immunizations. This will translate into more potential health gain for the population through earlier identification of disease or risk factors. 12 Reports from Elected Community Members I have really enjoyed being on the board of Waitakere Health Link over the past year and applaud our continuing relevance to the sector. I love the way Health Link touches lives by keeping people informed and promoting the issues the community in practical and inspirational ways. Our NGO forums and hospital open days are a great opportunity to bring a huge variety of services together to showcase the wonderful health support that is in our community. I am looking forward to serving another year with this wonderful organization. Kay Lindley It has been my privilege to be a part of Waitakere Health Link for the last 3 years. WHL has been a strong Health advocate for West Auckland. The West Auckland community includes diverse groups of ethnic people with different health needs. WHL always represent vulnerable people s best interest in health services by working with WDHB, PHOs, local government, and NGOs. I do appreciate the WHL Chair s passion and vision, as well as the coordinators professional service delivery, and the executive members great contribution to the community Grace Ryu It has been my privilege that I have belonged to Waitakere Health Link, where I can make myself to be more sensitive on the community s health and wellbeing issues as well as contributing to the group from my work and cultural experience. I am so pleased to work together with members, learning from their expertise and their life experiences in a loving and encouraging environment. I would like to convey my sincere gratitude to our dedicated coordinators who hold good heart for people and work hard with building team work. I trust the work WHL does. It has definitely contributed to improving health and wellbeing of Waitakere community as well as for healthcare service providers to hear the voices from the community. Samuel Cho Waitakere Health Link continues to be very active in community work in Community Network forums have also been a real success demonstrating the value that Waitakere Health Link brings to the sector by facilitating forums that brings together the community and other agencies, leaders, politicians and workers. The Waitakere Hospital Open as well as WHL s work with health literacy is other examples of its value and contribution. WDHB is increasingly utilizing the expertise, knowledge and networks of WHL in its work and community consultation. David Lui As Chairman of Hospice West Auckland I have secured partial funding for the operation of a 2 bed specialist palliative care unit which is almost complete and to be opened in September. As Chairman of Ranui Action Project, I have secured long term blood collection service with Labtests in Ranui. I am a m ember of the WDHB Palliative Model of Care Governance group. 13 Members Survey Waitakere Health Link s vision is to foster collaborative relationships between communities and healthcare providers through consultation, advocacy, and monitoring. As part of our annual review process we conducted a survey of our members to ascertain how effective we are in achieving our goals and where we need to put more effort. We received a response rate of 28%. Respondents were asked to score each specific activity under the four goals on the scale according to how they felt Waitakere Health Link has performed during the past year. 1 means very unsuccessful; 2 means unsuccessful; 3 means OK - neither particularly successful, nor especially successful; 4 means successful; and 5 means very successful; DK means Don t Know. The scores for last year are in brackets. The results are below: How effective are we in achieving our goals? Goal 1: To be a vehicle for the community voice to be heard in healthcare planning and decision making Average Score Range Waitakere Health Link (WHL) represents the Waitakere community by participating in WDHB 4.6 Community Engagement and Consumer Participation Alliance, attending CPHAC meetings and other WDHB committees are required (4.8) WHL provides support for community members participating in health committees 4.2 (4.5) 3-5 WHL actively supports the community health sector representatives 4.7 (4.6) WHL actively promotes collaboration between Primary/Secondary Healthcare Organizations 4.7 and the Community Sector through regular NGO forums (4.6) WHL works collaboratively with Auckland City Council by being involved in discussions and partaking in forums. Comments Received: An excellent year s work on all of the points in this goal. More information available to the members and organizations and also more information about board members etc come election time. More needs to be done to involve city council. Congratulations on your success in effort for better health facilities in West Auckland. I am very pleased we have this local hospital with 24 hour emergency access and specialized services. Thank you. I enjoy keeping up to date via Health Link newsletter. Thanks for excellent meetings. Goal 2: To identify new health issues in the community as they arise by Average Score Range Building good relationships with other health and social services in the community to find out what is happening for them 4.5 (4.4) Providing opportunities for the community to communicate their concerns and issues through a well advertised FREEPOST address, phone number, fax number and address 4.5 (4.5) Comments Received: Our co-ordinators have built an immense network of pertinent relationships in the community and have become the health voice for the west. I heard rumbles of downsizing our outpatient services and orthopaedic (knee/hip replacements). I recommend that physio therapy for orthopaedic services still continue at Waitakere Hospital for patients. That Waitakere Health Link may be active for many years for West Auckland population. Very important. Thank you for all your time and I wish you a successful year ahead.
