Dogs @ Play Daycare Requirements To ensure the health and safety of your pet and of our other guests, we require that all of our clients comply with the following rules and regulations. Age: All dogs must be at least 16 weeks or older. Puppies and shelter dogs must have been at home for 2 weeks prior to coming to daycare. Sex: All dogs over the age of 7 months must be neutered or spayed. Vaccinations: All dogs must have up-to-date vaccinations. Owners must submit written proof (easily provided by all vets) that their dogs have current C5 vaccinations. Health: All dogs must be in good health. Owners will need to certify that their dog is in good health and has not been ill in the last 30 days. On admission all dogs must be free from any illness condition that could potentially be communicated to other dogs. Once admitted, please alert Dogs @ Play Day Care staff to any change in your dog s health that requires medication, even if we will not be required to administer it. Behaviour: Owners will need to certify that their dog has not harmed or shown aggression or threatening behaviour towards any persons or other dogs. Application: All dogs must have a complete, up-to-date and approved application on file prior to attending daycare. There is a $25.00 non-refundable application-screening fee. Rates: Due and payable in advance or on the day of each service. Days and Hours: For your convenience Dogs @ Play Daycare is open Monday - Friday from 7 am to 7 pm. Dogs can be dropped from 7.00am (no earlier) and must be picked up by 7pm. A late pickup will be charged at $10 every fifteen minutes or there part of. After hours contact number is: 0412 400 572 Reservations: Required. Cancellations with less than 24 hours notice and no-shows will be charged full fees. What to Bring: Your dog must be have a collar with ID tag and be walked on a lead at all times to and from the centre. All items should be clearly labelled with your dog s name. Please do not bring personal items like toys or bones as this is a cage free environment, we cannot guarantee their exclusive use. We have water bowls, treats and toys. Feeding: We are able to provide a Royal Canin Dry Dog Food or BARF Frozen Patty meal for your dog at an additional small charge of $2 per day. Alternatively you may bring your own dog food if required. Medication: Please clearly label medications and include written instructions with your dog s name, type of medication, dosage, and frequency. Please include only enough medication for the length of your dog s stay. Please alert us in advance if you know your dog will require medication. Dogs @ Play Day Care reserves the right to refuse a dog for daycare if they display signs of a communicable illness or are taking medication for the same. WAIVER AND RELEASE OF LIABILITY In consideration of being permitted to have my dog(s) attend daycare at : 1. I acknowledge that: (a) In admitting my dog(s), is relying solely on my representations that my dog(s) is in good health and has not harmed or shown aggressive or threatening behaviour towards any person, animal or thing; (b) is a cage free environment and I accept that there are inherent risks associated with such an environment, including (but not limited to): (i) Transfer of communicable illness, such as kennel cough (ii) Injuries, including broken nails, abrasions, cuts and sore pads; and (iii) Behavioural problems; (c) Although carefully screen all applicants, occasionally it is discovered that this is not an appropriate environment for every dog. retains the discretion to temporarily or permanently remove my dog(s) from the daycare at any time; (d) I have read and understood the Dogs@ Play Waiver and Release of Liability and the Dogs @ Play Daycare Rules and Regulations. 2. I agree that: (a) I am solely responsible for any loss, damage or harm caused or contributed by my dog(s) while it is attending daycare at ; (b) While my dog(s) is in the care and custody of and in the case that I cannot be contacted and/or in the event of an emergency,, its agents and/or representatives may seek immediate veterinary care for my dog. (c) I am responsible for all costs associated with clause 1(c) and clause 2(b), and will provide Dogs @ Play Ptd Ltd with my credit card details so that it may be used for the payment of such costs; (d) I will abide by the rules set out in the Dogs@ Play Waiver and Release of Liability and the Dogs @ Play Daycare Rules and Regulations, accepting all terms, conditions and statements contained within. 3. I release, and its employees and agents, from all liability, and indemnify Dogs @ Play Pty Ltd from any and all claims of every kind and nature whatsoever, arising directly and indirectly, from my dog(s) participation in activities or use of service, including any legal claims and medical expenses.
