March 28, 2018 INVITATION TO BID BL038-18 The Gwinnett County Board of Commissioners is soliciting competitive sealed bids from qualified contractor for the Purchase of Veterinary Pharmaceuticals for the Animal Welfare and Enforcement Facility on an Annual Contract with four (4) options to renew for the Department of Community Services. Bids should be typed or submitted in ink and returned in a sealed container marked on the outside with the BL# and Company Name. Bids will be received until 2:50 P.M. local time on April 13, 2018 at the Gwinnett County Financial Services - Purchasing Division 2 nd Floor, 75 Langley Drive, Lawrenceville, Georgia 30046. Any bid received after this date and time will not be accepted. Bids will be publicly opened and read at 3:00 P.M. Apparent bid results will be available the following business day on the website www.gwinnettcounty.com. Questions regarding bids should be directed to Dana Garland, CPPB, Purchasing Associate III, at dana.garland@gwinnettcounty.com or by calling 770-822-8723, no later than April 06, 2018. Bids are legal and binding upon the bidder when submitted. All bids should be submitted in duplicate. BL038-18 Page 2 REQUIREMENTS & SPECIFICATIONS A. General: 1. Any supplier submitting a qualified quote may be deemed non-responsive. Qualifying a quote can be, but is not limited to, all or none offers and/or minimum order or delivery requirements. 2. Pricing for each UNIT PRICE PER BOX, BOTTLE, ROLL ETC. should not be extended beyond two decimal places. 3. Pricing for each LOWEST UNIT OF MEASURE PRICE should not be extended beyond four decimal places. 4. Gwinnett County Animal Welfare is eligible for shelter pricing. This should be reflected in the pricing of your bid submittal. 5. This contract does not allow for third party billing. If you bid an item and this item is awarded to you, the invoice must be billed from your company and reflect your company name. B. Delivery: 1. Deliveries will be F.O.B. Destination Freight Prepaid and Allowed and inside delivery is required. The unit price MUST include any shipping fees. Supplier shall deliver orders as specified on the issued purchase order document received and be accompanied with a packing slip. All invoices shall reflect the bid number, purchase order document number and the delivery address. It shall spell out the exact description as indicated in the bid tabulation. 2. Delivery location does not have a loading dock. Location is not accessible by any vehicle larger than a box truck. 3. All deliveries must occur between 8:00 A.M. and 4:00 P.M. Monday through Friday. 4. Delivery location is Gwinnett County Animal Welfare and Enforcement, 884 Winder Highway, Lawrenceville, GA 30045. C. Equivalent/Alternate Products: 1. The use of brand names is not for restrictive purposes or to limit competition but to assist suppliers in determining the standard of quality Gwinnett County is seeking. Gwinnett County shall be the sole determinant of acceptability of all approved equivalents. 2. When bidding an equivalent product, supplier represents: a. Equivalent product has been personally investigated and determined to be equal to or superior in all respects to that specified. b. Product identification, including manufacturer s name and address should be provided as part of bid document. c. It is the supplier s responsibility to prove equivalent products equal or exceed the quality level of the products(s) specified. D. Samples: 1. Gwinnett County reserves the right to request samples for evaluation purposes prior to award. 2. Gwinnett County reserves the right to request samples of any items on the bid schedule (including all equivalents). This does not mean all items, only certain items upon request. Samples should be provided within five (5) business days upon request. It will be the responsibility of the supplier to incur all costs associated with the samples. Failure to submit samples upon request may result in the line item(s) being deemed non-responsive

BL038-18 Page 3 or rejection of the bid. All samples are to be identified and must include the supplier s name, item number as indicated on the Bid Schedule, and bid number. 3. Samples will not be returned after the evaluation. E. Substitutions After Award: 1. Substitutions may be accepted only if the product quality is equal to or exceeds the current product specifications that were originally bid and is approved by the user department(s) prior to submitting the substituted item. Gwinnett County shall be the sole determinant of acceptability of substitutions. Item delivered without authorization will be rejected. 2. Gwinnett County reserves the right to purchase outside of this contract and the price difference may be charged to the vendor if item quoted is not available when needed. 3. If providing an approved substitution, pricing, delivery, quantity/packaging and invoicing must remain the same as the item originally bid/indicated on the successful supplier(s) bid documents.
