Briefly describe your animal programs: If your program performs adoptions, are all animals sterilized before adoption? Yes If not all, what percentage of animals are not currently sterilized before adoption? If not all, how are animals selected for sterilization before adoption? If not all, describe your sterilization policies and procedures for assuring sterilization after adoption: Give additional background information on your organization's programs as they relate to this application and the qualifications of the personnel who will be in charge of this program. Show that you have the ability to carry out this program. TLC PetSnip, Inc. is a high volume, low cost spay/neuter clinic. We opened August 2, 2013 and to date have performed over 4000 sterilization surgeries. We currently average 30 surgeries a day, 4 days a week. We were awarded a blitz grant from PetSmart Charities for 2015 and have successfully completed 2 of the 5 target groups, with the other 3 targets being scheduled in April, June and August. Executive Director and full-time volunteer, Terry Sandlak, will be in charge of this program. All of our veterinarians and staff are committed to our mission to end the killing of healthy animals as a means of population control. In addition to our Spay/Neuter services, we offer low cost vaccines, heartworm testing, Feline Leukemia/Aids testing and microchipping. If you currently have a program for sterilization of cats and/or dogs, describe your current level of funding and productivity and why additional resources are needed? Our clinic is funded mainly by the low-cost fees we charge clients and rescue groups for spay/neuter surgeries and by the lowcost vaccine clinics we offer weekly and at special events. We currently have a blitz grant from PetSmart Charities that will be completed September 2015. November 2014, we hired a full-time employee who is responsible for fundraising. We speak with residents of the Combee community on a regular basis who are unable to afford our services, despite our pricing structure being set to just cover our normal operating expenses. We've provided free spay/neuter surgeries in excess of $5000 to over 165 pets for this community during our last fiscal year. Target Population Geographical target area (name of city, county, etc.): East Lake Parker-West North Combee Area, Lakeland, FL 33801 Total human population in target area: 5722

Percent of residents living below poverty in target area: 32.8 Estimated number of pet cats in target area (human population divided by 3.3): 1734 Estimated number of pet dogs in target area (human population divided by 4.0): Estimated number of feral cats in target area (human population divided by 6.0): Number of cats admitted to animal control shelters in the target area last year (if known) Number of dogs admitted to animal control shelters in the target area last year (if known) Number of cats euthanized in animal control shelters in the target area last year (if known) Number of dogs euthanized in animal control shelters in the target area last year (if known) 1431 954 Please explain if you believe your target area animal population is significantly different than above. We believe our target area is similar to above. Polk County Animal Control did not give numbers for admitted/euthanized by cat and dog. They only gave combined numbers. The total number of cats and dogs admitted in CY 2014 was 22,200 and total number euthanized was 10,905. Please explain what you believe are the most substantial sources of dog and cat overpopulation in the target area: We believe the most substantial sources of dog and cat overpopulation in our target area is the lack of education of the importance of spay/neuter and the lack of resources to afford even low-cost spay/neuter. What kinds of spay/neuter services are currently available in the target area and in what ways are these resources currently insufficient to meet community needs? TLC PetSnip has been offering low-cost spay/neuter services since August 2013. Prior to and currently Humane Society of Polk County has a voucher program that provides services to income eligible residents of Polk County. SPCA Florida (Lakeland) also offers spay/neuter services although at a higher pricing structure. Despite these options being available in Polk County, we continue to see a staggering number of unaltered animals both at our low-cost vaccination clinics and around town during networking. Florida Animal Friend is highly supportive of proposals that are focused on animal populations that are identified as substantial sources of dog or cat overpopulation rather than being diluted over too broad of a geographic area or diverse animal populations. Describe the specific target animal population of the spay/neuter project proposed for this grant: Pets in low-income families Pit Bull / large breed dogs TNR managed colonies of feral cats Community cats (free-roaming and/or owned) Other Referrals from Polk County Animal Control TNR Managed Colony Feral Cat Program/Community Cats (Free-Roaming and/or Owned) Program Define the precise boundaries of the colony or targeted area, including estimate of square miles. What is the criteria used for determining the target area(s) and/or eligibility for this program? Describe whether the targeted area is rural, suburban, or urban. Is it commercial, residential, agricultural, or a designated special land use? Estimated number of cats in the target colony area : 0

Estimated number that are currently sterilized: 0 Projected reduction after utilizing the grant: 0 For TNR program, describe the ability to maintain lifelong care for remaining cats, commitment level of volunteers/organizations, etc. Detail any public education/outreach, adoption programs, etc. Do current city/county ordinances address TNR or free-roaming cats? No Please explain what is allowed: (NOTE: FAF will not fund any program this is inconsistent with local ordinances.) For TNR program, list any groups or government agencies who support this TNR effort: Describe any effort to lessen the negative impact on local wildlife. Describe efforts that will be made to mitigate current or potential nuisance issues. Will the cats be ear-tipped? No Will the cats be microchipped? No Provide any additional information that will help the grant selection committee understand how this program will operate to achieve its goals. Objectives What do you hope to accomplish with these funds (objectives should be specific and quantifiable)? Our first objective is to complete 350 sterilizations in targeted area. In addition, we want to increase public awareness of the importance of spay/neuter and responsible pet ownership. How does this program increase the number of sterilization surgeries above the existing baseline? The majority of residents living in the Combee area can not afford basic household needs, let alone funds to pay for spay/neuter surgeries. With monies from this grant we can provide assistance to those families who want and need the service, while continuing to offer low-cost options to all of our clients. Methods What criteria will you use to determine eligibility for your program? Our first determination will be proof of residence in approved zip codes and/or letter of referral from Polk County Animal Control. Photo ID and proof of residence will be required. How will you advertise the program? Explain how the advertising will reach the target audience. Advertising will be done via our website, Facebook and local media. Word of mouth will also be utilized as will employees and volunteers going out in the Combee Community to talk with people face-to-face about the importance of Spay/Neuter. We will utilize flyers, posters and leaflets as needed as well as working directly with Polk County Animal Control.

How will you address barriers to full use of the program such as transportation, illiteracy, and cultural hurdles? We have a bi-lingual employee to help with our Spanish speaking population. We will have Spanish flyers to distribute. Our clinic is located in the same zip code as the Combee residents. Since we will also be on foot, distributing literature we will have the opportunity to speak directly with people who may not be able to fully comprehend the written literature. Does this project involve the transportation of animals by someone other than the client? If so, describe the vehicles, methods for confinement, personnel training, liability releases used to assure the safety of the animals and handlers. Veterinary Services Objectives What arrangements have you made with veterinarians to perform the surgeries? Our own TLC PetSnip Clinic will be performing all the surgeries. We have three in-house, contract veterinarians. Are they: In-house Private Vet(s) Combination Veterinary Practices Practice Name Address City State Zip Phone Lead Practice Lane Wallett, DVM 1701 E Gary Rd Lakeland Florida 33801 863-686-7647 Truman Harrell, DVM 1701 E Gary Rd Lakeland Fl 33801 863-686-7647 Tsuyoshi Watanabe, DVM 1701 E Gary Rd Lakeland Fl 33801 863-686-7647 Fee Range What is the fee range to be paid for spay and neuter and what is the distribution to be paid by the client vs. the grant program? Keep in mind that Florida Animal Friend grant funds may only be used for costs directly associated with sterilization surgery (including anesthesia and pain control) and not for other items such as vaccines, testing, licensing, and capital purchases. Amount Paid by Client Amount Paid by Project Total Amount Range for Male Cats $0.00 $30.00 $30.00 Range for Female Cats $0.00 $40.00 $40.00 Range for Male Dogs $0.00 $75.00 $75.00 Range for Female Dogs $0.00 $85.00 $85.00 Please check each item below to indicate additional services offered at the time of surgery, whether the client is required to pay for them, and if so what the fee is. Cryptorchid additional fee is $15. Parasite medication (capstar) is only required if animal has flea infestation upon intake to clinic. Is this a voucher program? No If so, how will you assure compliance with the program? For your voucher program, how have you determined the capacity of the veterinarians listed above to handle the projected capacity? Will you have the ability to report the number of vouchers issued and the percentage that result in S/N surgeries? Community Collaboration To assure the success of your program, are there any local groups (such as rescue groups, animal control agencies, TNR groups, local businesses, local media, social service agencies, etc.) other than your organization and your cooperating veterinarians who are committed to assist? Yes No Please list them and detail their level of involvement with the proposed effort. Name Level of Involvement Polk County Animal Control Press Express High Level - This group already distributes our information to clients registering animals for rabies certificates. They are invested in providing referrals to residents in need of assistance affording spay/neuter options for their pets. Medium Level - We currently have a partnership with Press Express. They do all of our printing and we provide spay/neuter services to their feral cat rescue. Other Information Provide any additional information that will help the grant selection committee understand how the program will operate to achieve its goals. We truly believe that pet overpopulation can only be fixed with affordable spay/neuter options in every community. Since opening our doors August 2013, we have completed over 4000 sterilization surgeries. We have hired a full-time employee to assist with all marketing/advertising and fundraising for TLC PetSnip, Inc. We offer weekly and monthly vaccine clinics which helps offset spay/neuter surgeries that we currently offer for no cost to our clients that need additional assistance. Budget

Total number of sterilization surgeries projected: Cats: 200 Dogs: 150 Total budget requested (Budget should not exceed $25,000): $25,000.00 Average cost/surgery projected: $72.00 Describe any expenses that are not included in the grant and how they will be paid for: Vaccines, microchips, county license will be paid for by caregiver if mandated or elected. TLC PetSnip will absorb any costs caregivers are unable to provide, other than County Rabies Tag Registration fee. These funds will be available from our Community Outreach Fund, as those resources won't be needed to cover the cost of surgery.
