Adoption Forms for Dogs and Puppies Please Fill Out Applicable Forms COMPLETELY NOTE: If you don t have access to a printer please email ourlastdaydogs@gmail.com to have adoption forms mailed to you. 2 copies of each form MUST be filled out at the time of adoption. 1 copy for the adopter and 1 copy to be mail into LDDR within 3 days of the adoption. $50 LAST DAY DOG RESCUE, INC. Dog Dog - Mother Puppy ADOPTION CONTRACT, RELEASE AND SCHEDULE A CANINE Last Day Dog Rescue, Inc. (Last Day), in consideration of the sum of, the receipt of which is hereby acknowledged, releases the full responsibility and ownership of (the Rescue Dog ) (LDDR Rescue ID) to the undersigned Adopter. The adopter agrees to abide by the terms and conditions of adoption as follows (the Agreement ): 1. Indemnification and Hold Harmless. The undersigned Adopter hereby agrees to release, indemnify, and covenant to hold harmless Last Day, its officers, members, agents and other representatives, from any claims, damages, injuries, costs or actions incurred as a result of this adoption or caused by the actions of the Rescue Dog. 2. Transfer of Ownership. **INITIAL HERE: ** The Adopter will NOT transfer ownership of the Rescue Dog to any other person, shelter, boarding/kennel or humane society. If for any reason the adoption is not satisfactory, the Rescue Dog will be returned to Last Day (Note: there will be no refund if the Rescue Dog is returned). If returning the Rescue Dog, the Adopter must allow Last Day up to 10 business days to find placement for the Rescue Dog into a new living situation. The Adopter agrees to notify Last Day with his or her new address upon moving and to notify Last Day if the Rescue Dog is lost or stolen. Transferring ownership of the Rescue Dog to a new permanent address without Last Day s involvement/permission will incur a penalty of $1000.00. 3. Health. The Rescue Dog will be placed on a yearly inoculation and parasite check program as prescribed by a licensed veterinarian in order to assure/maintain the health of the Rescue Dog. The Rescue Dog will receive a rabies vaccination (either 1 year or 3 year) and will be tested for heartworm and heartworm preventative will be administered as prescribed by a licensed veterinarian. If the Rescue Dog is not sterilized upon adoption, a separate spay/neuter contract will be completed which will specify the date by which the Adopter will have to submit proof (copy of vet receipt or letter from vet) that sterilization has taken place. Last Day does not warrant the health of the Rescue Dog. 4. Control. The Rescue Dog will not be allowed to run loose off the Adopter s property, except if involved in a bonafide sporting event and the Rescue dog will be under voice control or on a lead when not within the boundaries of the Adopter s property. 5. Training. Last Day strongly recommends that the Rescue Dog and the Adopter enroll in an obedience or American Kennel Club Canine Good Citizen class. The Adopter hereby agrees NOT to train the Rescue Dog for protection, guard, or attack purposes. 6. Care. The Adopter accepts full responsibility for maintaining a reasonable level of care for the Rescue Dog, including food, water, grooming, adequate sanitation, exercise, attention, safety, and shelter. The Rescue Dog will be treated with affection and kindness and never be subject to abuse, animal research. The Rescue Dog will not be subject to dog fighting or use as a guard dog for any agency, corporation or organization. The Rescue Dog will NOT be kept as an outdoor animal. 7. Reservation of Rights. Last Day reserves the right to an ongoing review of this adoption and to follow-up on any complaints or reports to protect the welfare of the Rescue Dog. 8. Remedies Upon Default. Should the Adopter breach any of the agreements contained in this contract or have misrepresented any facts to Last Day to obtain the Rescue Dog, Last Day shall have the right to rescind this contract and demand return of the Rescue Dog. Should the Adopter fail to return the Rescue Dog, Last Day may at its option, a) peaceably enter the Adopter s property to take possession of the Rescue Dog or b) seek an order from a court of competent jurisdiction for return of the Rescue Dog and payment of damages and will retain the sum paid for the Rescue Dog pursuant to this agreement. 9. Attorney s Fees. The Adopter agrees to pay actual attorney s fees and other costs incurred by Last Day to obtain the remedy available under Paragraph 8 above. 10. Severability. If any provision of this Agreement is held illegal or unenforceable in a judicial proceeding, such provision shall be severed and shall be inoperative, and the remainder of this Agreement shall remain operative and binding on Last Day and Adopter 11. Microchip: Last Day microchips all their dogs. A Last Day representative will add your name/contact info to the Rescue Dog s microchip registration. Notify Last Day of any moves so the microchip information can be updated. Microchip # Must get dog licensed in your city within 30 business days of receiving Rabies Certificate. Tagging collar is not optional and MUST be done immediately. YOU MUST ESTABLISH YOUR DOG WITH A LICENSED VET WITHIN 2-4 WEEKS OF ADOPTION. Puppies will need to be followed up with vet per Puppy Addendum. Obedience classes/training is strongly recommended for your dog or puppy. ACCEPTED AND AGREED TO AS OF THIS DAY OF YEAR. SCHEDULE A I understand that dogs and children can be unpredictable. Extreme caution and strict supervision should be exercised at all times when child(ren) and adopted dog are together. You understand we have limited knowledge of this dog and can only tell you what we have observed while in our care or what information, if any, the shelter may have given us. At any time a child could provoke a dog to react in a negative way and cause a reaction by that dog when defending itself or its possessions if feeling threatened. Extreme caution and strict supervision must be used when they are within the same area. Here is a simple list to help you improve your supervision skills: Watch for loose canine body language. Good dog body language is loose, relaxed, and wiggly. Look for curves in your dog s body when he is around a child. Stiffening and freezing in a dog are not good. If you see your dog tighten his body, or if he moves from panting to holding his breath (he stops panting), you should intervene. These are early signs that your dog is not comfortable. Watch for inappropriate human behavior. Intervene if your child climbs on or attempts to ride your dog. Intervene if your child pulls the ears, yanks the tail, lifts the jowls or otherwise pokes and prods the dog. Don t marvel that your dog has the patience of Job if he is willing to tolerate these antics. And please don t videotape it for YouTube! Be thankful your dog has good bite inhibition and intervene before it s too late. Watch for these three really easy to see stress signals in your dog. All of them indicate you should intervene and separate the child and dog: o Yawning outside the context of waking up o Half-moon eye this means you can see the whites on the outer edges of your dog s eyes. o Lip licking outside the context of eating food Watch for avoidance behaviors. If your dog moves away from a child, intervene to prevent the child from following the dog. A dog that chooses to move away is making a great choice. He s saying, I don t really want to be bothered, so I ll go away. However, when you fail to support his great choice and allow your child to continue to follow him, it s likely the dog s next choice will be, Since I can t get away, I ll growl or snap at this kid to get the child to move away. Please don t cause your dog to make that choice. Listen for growling. I can t believe how many times I ve heard parents say, Oh, he growled all the time but we never thought he would bite. Dog behavior, including aggression, is on a continuum. For dogs, growling is an early warning sign of aggression. Heed it. If growling doesn t work, the dog may escalate to snapping or biting. Growling is a clue that you should intervene between the dog and the child. By your signature on this document, which is an attachment to the Adoption Contract, you understand the risks and any possibilities thereof regarding children and dogs and release any liability against Last Day Dog Rescue, Inc. and its representatives that you may incur and understand any danger that could exist with children and dogs. TRIAL ADOPTION ADDENDUM **Must have board approval for a trial adoption** I understand that I am taking (Adopter) (Dog s Name/LDDR Rescue ID) on a Trial Adoption. HEARTWORM POSITIVE ADDENDUM HEARTWORM TREATMENT I understand that the Dog I am adopting (Dog s Name/LDDR Rescue ID) has been tested for Heartworm and found to be positive. He/She may be in the process of treatment or currently recovering from treatment. LDDR's pre-treatment protocol consists of 30 days of antibiotic to kill the bacteria that the microfilaria (immature heartworms) live on; followed by two injections of Immiticide 24 hours apart. Dogs may be sore, lethargic, may not want to eat for a few days, and may have loose stool after treatment. They MUST be kept relatively quiet (no roughhousing, jumping, running, or any other activity which may raise their heart rate) for 6-8 weeks after heartworm treatment. If LDDR is to treat, the protocol will be established by LDDR and our licensed advisory veterinarian. If at any point the adopter disagrees with the protocol, adopter MUST take the dog to their personal veterinarian and will be responsible for the cost of treatment. While your adopted dog is in the care of LDDR volunteers and LDDR vets, we do everything possible to ensure your dog s safety and comfort. However, we will be held harmless if any dog develops complications as a result of heartworm disease or its treatment; or if any unforeseen incident occurs while the dog is in our care. ADOPTER INITIAL: Adoption coordinator chooses one or more of the following as appropriate, at the time of adoption: Dog has been treated on and needs to be retested on (6 months after treatment). The cost of this retesting will be covered when LDDR's Heartworm Treatment Team is notified and the dog is taken to the APPROVED LDDR VET. If the dog tests positive again, LDDR will cover additional treatment costs when taken to the APPROVED LDDR VET. Dog is on the pre-treatment protocol with injection treatment to be done by the approved LDDR vet at the home of an LDDR volunteer on a date to be determined. DATE PRE-MEDS STARTED Dog has not yet been treated or gone through the 30-day pre-treatment protocol. DATE PRE-MEDS STARTED Dog will be treated by adopter, at adopter's expense, and proof of treatment will be provided within 60 days of adoption. Failure to provide adequate/appropriate and recommended care for HW positive dogs as per LDDR's contract will result in liquated damages of $1,000 for breach of contract. SPAY/NEUTER Adopted dog is sterilized Adopted dog is NOT sterilized If the dog is not sterilized, a $50 spay/neuter deposit and a completed Sterilization Agreement form (attached to this adoption contract) will be required. Spay/neuter surgery can be done once your dog tests negative for heartworm 6 months post-treatment. This cost will be covered if LDDR's Heartworm Treatment Team is notified, and the dog is taken to the APPROVED LDDR VET. If the Adopter prefers to take the dog to their own vet, they may do so at their own expense. Proof of spay/neuter MUST be provided to LDDR within 60 days of negative heartworm test (retesting is to be done 6 months post-treatment). By your signature on this document, which is an attachment to the Adoption Contract, you understand that your adopted dog has heartworm and will need treatment. You agree to deliver him/her to location requested and pick him/her up from same for treatment. You agree to follow-up 6 months post-treatment with a heartworm retest. You further agree to obtain annual heartworm tests at your own veterinarian and to maintain your dog on heartworm preventative monthly, year-round, for the entire life of your dog.
