I understand that failure to submit the required documents allows Olive U Standard Poodles to assume I am using this puppy for breeding without permission which will result in penalties of a minimum of $5,000 for breach of contract. Spaying/neutering the puppy listed above before the date listed to be their six month birthday will void all warranties. Olive U Standard agrees to send American Kennel Club (AKC) Registration Application once proof of spay/neuter is received. 2-Year Genetic Guarantee: Olive U Standard Poodles warranties the puppy listed above for two (2) years against genetic defects. If at any time during the first two (2) years of the puppy s life a genetic defect is detected by a licensed veterinarian that would be considered a threat to the puppy s life or grossly effect the puppy s ability to maintain a suitable life for purpose of companionship, Olive U Standard Poodles agrees to replace the puppy, with a puppy equal to the specifications of the warranted puppy as color & gender when one becomes available. 3-Year Hip Guarantee: Should you decide to have your puppy s hips tested, Olive U Standard Poodles guarantees an OFA hip rating of Good or better. Should your puppy rate less than Good Olive U Standard Poodles will replace the puppy, with a puppy equal to the specifications of the warranted puppy as color, gender and quality when one becomes available. You must have your puppy s hips tested between 24-36 months of age. Any hip problems/poor ratings due to injury or abuse will not be covered by Olive U Standard Poodles. Lifetime Hip guarantee: First six (6) years full replacement on puppy of same stature (see example above), seven-nine (7-9) years five-hundred dollars ($500.00) toward replacement puppy, ten plus(10+) years or older, three-hundred ($300.00) toward replacement puppy. Lifetime Nutrition Guarantee: Olive U Standard Poodles guarantees this puppy for it s lifetime against conditions that result by mal-nutrition, example of an included condition but not limited to: Osteoporosis as result of lack of Vitamins and/or Minerals. Buyer understands that to benefit from the lifetime nutrition guarantee and lifetime guarantee against hip/elbow dysplasia, the puppy listed above must be kept strictly on Life s Abundance Agility Formula for Puppies and Adults (Life s Abundance Puppy Food is also acceptable for the first year). YOU MAY ASK TO SEE THESE ITEMS PRIOR TO PURCHASE. MAKE SURE YOU RECEIVE THESE ITEMS AT THE TIME OF PURCHASE" [2007, c. 702, 33 (AMD).] 7 4160. Notice of consumer rights 1. Written notice. A seller shall provide the purchaser a written notice of rights, signed by the seller, certifying the accuracy of the information contained in the notice. The notice must be signed by the purchaser, acknowledging that the purchaser has reviewed and understood the written notice. A signed copy must be retained by the seller and one copy given to the purchaser. animal at the time of delivery or if, within one year after receipt of the animal by the purchaser, a veterinarian states in writing that due to a hereditary or congenital defect the animal has died or has a condition that will shorten its life or will require constant treatment during its life, the animal is considered to have been unfit for sale at the time of sale.
