TEGA K9, LLC Dog Daycare & Training Off-Leash Play & Train Application We love dogs and want your dog to love attending and participating in leash free play groups and training. No one knows your dog better than you, so we d appreciate you taking the time to fill out this application. The more we know about the dogs in our care, the better their experience will be. However, before taking the time to fill out the application it is important you read and understand the following so expectations are managed for both you as a client and for us, the care takers of your beloved dog. If you have ANY questions, please ask us. WHAT TO BRING Completed Tega K9 Off Leash Play & Training Application which can be found online at: http://www.tegak9.com/tega_k9_off_leash_play_and_training_application.pdf A copy of your dog s up-to-date vaccination records for us to keep on file We require proof of current rabies, DHPP (Distemper/Hepatitis/Parainfluenza/Parvovirus) and Bordatella (kennel cough) vaccinations. Bordatella must be given every 6 months. For dogs under 6 months you can bring a labeled snack-size baggie of your dog s food if you would like them to have puppy lunch. Your dog wearing a Quick Release collar (either Velcro or a flat squeeze-release buckle) and identification tags. No choke, prong or electric collars. 6 foot nylon or leather leashes are ideal. Policies, Procedures, Expectations and the truth regarding off leash play and the daycare environment Our goal at Tega K9, LLC is to provide a safe, fun and stimulating social environment for dogs who desire and thrive in this environment. To ensure the overall safety and health of your dog and our other guests, we expect all of our clients to comply with the following policies and understand why we do what we do. 1. Tega K9, LLC is proud to be a 100% Force Free daycare and training service. With that, we promise to never use intimidation, pain, or fear to redirect (aka correct ) your dog while in our care. We understand your dog is not able to tell us when they are uncomfortable, tired, upset tummy, etc. When we encounter a dog exhibiting undesirable behavior, we seek first to understand why and address the root of the issue to help them return to their baseline behavior. Please refer to the Hierarchy of Dog s Need to understand what we look for. 2. As a Force Free establishment. We do NOT accept dogs wearing ANY electric, choke or prong collars. Please do NOT ask us to use these, we will NOT, EVER. If you need help in an area where someone told you these devices are necessary please let us know and we will direct you to modern, science based training methods which will not induce anxiety and unnecessary stress, pain or fear in your dog. If you bring your dog in on any of these devices, we will remind you they are not permitted. If it happens again, we will refund any money for prepaid and reserved services and refuse service going forward. 3. Please understand, many dogs are NOT candidates for a daycare environment. As they age, they are less likely to enjoy the company and high energy antics of group play with adolescent dogs. If the time comes that your dog becomes cranky, irritable, etc. and begins to show signs of disinterest or frustration in our daycare program, we promise to let you know. Please do not take this personally. It may not have anything to do with what you have or have not done with your dog. We may even suggest an alternative to Tega K9 if we believe another facility or program is in your dog s best interest. 4. Some dog s make friends for life as puppies and prefer to stay with that group of friends and are not as receptive to new friends. This is okay, a lot of people are like this. We get it! We have a private Facebook group for our clients where you may reach out and make those connections for your dog s to meet outside of Tega K9 activities and events. 5. Dogs who evaluate as green or yellow dogs, may move up or down the scale at different points in their development and life time. This is only a measurement in the off leash play environment of Tega K9, LLC. Your dog may be and most likely is different at home or during one on one play. This is important to understand. If we can help in any way to preserve your dog s green status we will, it s a guarantee. However, we CAN NOT guarantee a dog s behavior or response to our guidance. 6. Long days at daycare are like long days working in the yard, family reunions, day at the beach with kids, etc. A LOT is going on and at the end of the day your dog will be mentally and physically tired. It is best when possible to avoid extended days at daycare. Those arriving around opening and leaving around closing may burn out with daycare quickly. So if you can avoid consistently long days, it is ideal for you and your dog long term. 7. We have specific drop off and pick up hours due to dog s high arousal status during these hours because a lot of movement and changes in group dynamics. If your dog is a little nervous, it is best they arrive as early as possible so they are less likely to be overwhelmed with a full house (playgroup) greeting them. Additionally, it is best they leave at the early end of pick up hours because new dogs are sometimes challenged with understanding the constant changes in playgroups which in turn can be stressful. Please do your best to respect these hours for your dog s benefit and to allow staff to remain in their stations with dogs during non lobby hours. 8. Owners/Guardians must give full disclosure regarding their dog s history of aggressive or threatening behavior. In many cases we can help through our training programs however, if it is not disclosed upfront we will have difficulty trusting you and your dog and it will hinder our partnership with you in caring for your dog going forward. 9. If your dog s social skills need work, we will determine upon the initial evaluation if they are at a level where we may be able to help build their confidence. However, although they may be approved initially for group play and daycare, if we do not see improvements we may suggest alternative options to group play and confidence building. We want your dog to be happy and comfortable and do not want to be a source of stress in their lives. 10. We require proof of current rabies, DHPP (Distemper/Hepatitis/Parainfluenza/Parvovirus) and Bordatella (kennel cough) vaccinations. Bordatella must be done every 6 months. We accept titers. Although dogs are vaccinated against certain viruses, bacteria, bugs, etc., it is still possible for dogs to share coughs, sniffles, eye infections and general cooties just like children at daycare. 11. All dogs must be free from any condition which could potentially jeopardize other guests or themselves. DO NOT BRING YOUR DOG TO DAYCARE if they are not well. 12. We recommend all dogs be on an oral flea treatment program. Please do not administer a topical treatment within 7 days of visiting Tega K9 and no flea or tick collars are not permitted. We do not want dog s wrestling with other dog s who have insecticide on their coat (getting it in their mouths) and do not want staff inadvertently and unknowingly touching these chemicals. 2

13. All dogs must be clean and in good health with the exception of stable, post surgical dogs that are in need of daytime human supervision (These are special prearranged services). 14. When dog s play, they wrestle and use their mouths and their feet, etc. Sometimes dog s will have scratches from other dog s and sometimes even teeth mark scratches from play with other puppies. THIS IS NORMAL! As you know, puppies have very sharp teeth and while they are learning bite inhibition from each other they may end up with superficial wounds. This is like children coming home from school or football practice with skinned knees, and elbows, bruises, etc. We would never allow dogs and puppies to bully each other however some time s dog s become overly excited and forget their manners. We are here to help guide and direct fair play however please remember the daycare environment can be like the playground at school. 15. We will place your dog in a playgroup or playgroups that best fit his / her energy level, size and age. Sometimes, our guests want to take naps with a buddy in a suite or in a nook they ve found or an unoccupied sofa or chair. If we recognize your dog or puppy is tired but is having difficulty finding a good spot to shut down, we will provide a suite for them if they are comfortable with this. However, daycare at Tega K9, LLC is NOT intended to be a place where our guests are kept confined all day with a few potty breaks. That is just not our idea of fun or daycare. 16. For the safety of all dogs and our guests, dogs must be escorted in and out of Tega K9 ON LEASH, through the appropriate enter & exit doors. This helps those dogs who may be reactive on leash safely avoid having to greet strangers while walking in or out of our facility. 17. All dogs must wear "quick release" collars. These are collars that pinch off so we may remove them quickly in the morning to get them out for a potty break and to greet their friends as well as get them ready to go home as quickly as possible. 18. When greeting your dog at pick up, please remain calm, and only greet your dog when he or she is sitting. This will transfer to greetings of strangers on walks and at your door at home. Ideally, if you bring a few pieces of really good treats and give it to them when they OFFER a sit, they will learn this routine WAY FASTER! 19. Tega K9 guests must have a completed registration and release form on file, with medical, contact and behavioral information BEFORE they are left in our care. 20. All dogs MUST have a complete full day evaluation. Evaluations are $35 by appointment Monday-Friday. 21. Payment for daycare is due at the time of pickup or in advance. Please let us know if we can help you set up your online account. From your online account you may make reservations, payments, register for group classes, purchase gift cards, and view all account details. 22. Although uncommon, any injuries incurred while at Tega K9 will be the financial responsibility of the owner /guardian. If an injury is due to the direct negligence of staff (not between dogs), the guardian / owner agrees to discuss details with Tega K9, LLC management and come to a reasonable and fair agreement regarding financial responsibility of veterinary care. If injuries result from a fight between dogs the owner / guardian of the dog who is determined to be at fault is responsible for the veterinary bills of the injured party. If it is undetermined which dog is responsible for injuries, then all parties involved are financially responsible for the care of their own dog. (note: dogs although evaluated, can be unpredictable.) 23. Tega K9, LLC will post any contagious illnesses that exceed 1% of our population and is not responsible for veterinary bills of this nature. 24. If your dog is ill and you are unavailable to pick him / her up, we will make accommodations to take him/her to your local vet or Baxter Veterinary Clinic if your vet is not in close proximity. A $25 transportation fee will apply. 25. We are an indoor and outdoor daycare / training facility. If it rains or snows, our outdoor play yard will get wet and subsequently, your dog may get wet from the turf being rained/snowed on. We also have sand boxes for your dog s enjoyment. If these get wet, we do our best to keep dogs out of them however, some dogs are persistent and will work really hard to get in them to dig (a favorite past time for many). We cannot guarantee your dog will go home clean (but we do our best to have them going home as close to it as possible) due to our main goal of providing a fun, safe, mentally and physically stimulating play day and environment. (Please inquire about our spa bath dog treatments if ever needed). 26. Sometimes it is difficult for us to answer the phones when dogs are barking and / or when we have guests in the lobby (especially during drop off and pick up hours), please leave a voicemail message if we can not be reached with as much information as possible and we will respond as quickly as we can. Alternatives are email team@tegak9.com and texting (704) 608-3271. 27. We try to take as many photos as possible and post on Facebook when we can. Please understand that the more time we spend taking and posting photos takes away time we d like to spend focusing on and with your dog. If you have specific requests, we will always do our best to accommodate you and your dog! 28. We reserve the right to refuse service to anyone, for any reason, at any time. 29. EMERGENCY INFORMATION If your dog becomes ill or injured, whom do you want us to call first and what number? If your dog becomes ill or injured and we need to transport for immediate veterinary care, we will transport to the following Veterinary Clinics: Baxter Veterinary Clinic, Veterinary Medical Center of Fort Mill, Tega Cay Animal Hospital or Palmetto Pet Hospital. If you re not a client at any of the named vet practices, we will default to the Baxter Veterinary Clinic as they are a highly recommended veterinary clinic and we have an agreement with them to see our dogs in the event of an emergency. TEGA K9, LLC is not responsible for vet bills in the event that your dog becomes ill or injured. PAYMENT Payments must be made in full and in advance for daycare / training. Tega K9, LLC accepts cash, checks, credit, and debit cards. No Refunds for Daycare or Day Training. Any remaining balance at the end of the service day, will be charged to your card on file. 8

LIABILTY WAIVER / RELEASE AGREEMENTS & POLICIES 1. I understand that I am financially responsible for any damage to Tega K9, LLC facilities and equipment caused by my dog. (Please initial) 2. I understand if I do not pick up my dog by 6:30pm (for full day daycare/training, a fee of $25 for the first 15 minutes). If my dog is not picked up by 6:45pm, he/she will be tucked in for overnight care, and will not be able to be picked up until the next day. You will be charged an additional rate of $39 for the overnight care. If my dog is here for half day daycare and is not picked up by 12 noon, I will be charged the full one day rate and my dog will not be available for pick up until 2pm (afternoon hours). Tega K9, LLC hours are 6:30am - 12pm and 2pm-6:30pm, Monday-Friday. I understand Tega K9. LLC is closed to the public between 12pm-2pm for lunches (K9 s and staff), as well as needed rest & quiet time for the dog s. (Please initial) ANIMAL MEDICAL POWER OF ATTORNEY Tega K9, LLC will make every effort to contact you in the event of a medical emergency involving your pet. However, if we should be unable to reach you, we will need formal authorization to request care for your pet on your behalf. Please review the following Animal Medical Power of Attorney, and if acceptable, sign below. I, the undersigned owner, hereby designate Tega K9, LLC ("TK9 ) to act as my attorney-in-fact and to act in my name for the emergency medical benefit of my pet upon the terms and conditions outlined below. 1. Effectiveness. The Animal Medical Power of Attorney shall become effective in the case of medical emergency requiring immediate care for my pet during my absence or if deemed necessary to preserve the life or well being of my pet. 2. Powers. By the execution of this Animal Medical Power of Attorney, it is my intention that my attorney-in-fact shall have authority to make all emergency healthcare decisions for my pet to the same extent I would, including but without limitation, the following: to employ and discharge medical personnel; to execute documents; to provide written consents/releases for treatment; to obtain and administer prescribed medications; and to incur reasonable and necessary fees and costs in carrying out the powers and duties under this document that shall be reimbursed by me upon demand by TK9. 3. Indemnification. I shall hold harmless and indemnify my attorney-in-fact from all liability for acts done in good faith. ASSUMPTION OF RISK AND INDEMNIFICATION 1. Assumption of Risk. I understand and acknowledge that pets can be extremely unpredictable in behavior and while TK9 performs its services, the chance of injury to my pet is possible. I assume all risks related to TK9 s services to my pet (with the exception of gross negligence of TK9), including but not limited to: illness, bodily injury; death; theft; bites; collisions with vehicles; natural disasters; the unavailability of emergency medical care; or the negligence or deliberate acts of third parties. 2. Release of Liability. I agree not to sue and to release from liability TK9, its officers, contractors, owners, agents, employees and other persons or entities involved with the services offered by TK9, from all actions claims or demands for injury, loss or damage regardless of the cause. 3. Indemnification. I understand and acknowledge that pets can be extremely unpredictable in behavior and may cause damage to third parties for which TK9 could be held liable. I agree to bear any and all damages, losses, liabilities, demands and expenses, including legal and professional fees TK9 may incur as a result of any damage caused by my pet, and I agree to defend, and hold TK9 harmless for any liability thereon. IT IS THE INTENTION OF THE PARTIES TO THIS AGREEMENT THAT THE FOREGOING RELEASES SHALL BE EFFECTIVE AS A BAR TO ALL ACTIONS, FEES, DAMAGES, LOSSES, CLAIMS, LIABILITIES, DEMANDS OR DEBTS WHATSOEVER, OF ANY NATURE OR KIND, KNOWN OR UNKNOWN, SUSPECTED OR UNSUSPECTED, ARISING OUT OF THE PERFORMANCE OF TK9 s SERVICES. THE PARTIES TO THIS AGREEMENT EXPRESSLY CONSENT THAT THIS RELEASE SHALL BE GIVEN FULL FORCE AND EFFECT IN ACCORDANCE WITH EACH AND ALL OF ITS EXPRESS TERMS AND PROVISIONS. I understand that this Agreement contains an Animal Medical Power of Attorney, release of liability and contract between TK9 and me and I am signing this agreement of my own free will. If any part of this Agreement is deemed unenforceable, all parts shall be given full affect to the extent possible. If there is a dispute between the parties relating to this Agreement, the party substantially prevailing will be entitled to recover all costs and expenses of any subsequent proceedings (including trial, appellate and arbitration proceedings), including the attorney fees incurred therein. This Agreement contains the complete understanding of the parties with respect to the subject matter hereof and supersedes all prior presentations and understandings, whether oral or written. This Agreement may be modified only by writing and signed by both parties.
