PAWSITIVELY PERFECT Boarding/Daycare Agreement GENERAL TERMS: Pawsitively Perfect will exercise reasonable care for the safety of your pet and to keep the boarding premises safe and secure. Pawsitively Perfect s liability for any boarded pet will not exceed $25.00. The pet owner is solely responsible for any property damage or bodily injury inflicted by the boarded animal. Pawsitively Perfect cannot guarantee against accidents. We will not be liable for loss or damages to your pet s belongings. VACCINATIONS/HEALTH: All animals must be current on vaccinations for Rabies, DHLPP and Bordetella (kennel cough). The owner must provide the veterinarian records for these vaccines prior to boarding/daycare. All animals accepted for boarding/daycare must be free of fleas and ticks; owner will be charged a $90.00 fee for any animal that is dropped off with parasites. Owner hereby represents that this pet has not been exposed to Rabies, Bordetella, Distemper, or Parvo within the last 30 days. BOARDING REQUIREMENTS: Pets are to be dropped off with nylon or leather collars (choke collars or harnesses will be removed). Pets are fed twice daily. Owners are to provide usual food. Dry food is best in large, labeled Ziplock bags labeled with your pet s name. Canned food or any type of wet or refrigerated will be given upon request (owner must provide). If pet is trained to use potty pads, these must be provided by the owners. Owners may bring treats, toys, and bedding. All items brought in must be labeled with your pet s name. We provide stainless steel dishes for food and water. For sanitation sake, please do not bring your pet s dishes. MEDICATION AND TREATMENT AUTHORIZATION: Prescription medications must be provided in the original container with the veterinarian s instructions. Medications will be administered as prescribed for a charge of $2.00 per dose. Pawsitively Perfect is authorized by the owner to seek veterinary advice and care at their discretion and at the owner s sole expense. Pawsitively Perfect will attempt to contact the owner before obtaining this care if possible; however, this document serves as full authorization to obtain veterinary care for the pet at the owner s expense whether or not owner was able to be contacted. PAYMENT TERMS: Owner agrees to pay current boarding price (as well as the price for any additional special services offered) in advance. No cash refunds will be issued for any reason. Boarding credit will be issued if the cancelation is at least 48 hours prior to the pets reserved boarding stay. There will be no credit issued due to cancelations for Holiday stays. An additional fee will be applied for holidays (the holiday rate is applied to the day before, day of, and day after the holiday) and will be included in total boarding charges paid in advance. There will be a $45 deposit per dog for all boarding bookings. This fee is non-refundable when there is a no-show or cancellations made less than 48 hours of your scheduled stay. No credit will be issued for early pick-ups. We close at 6:00pm. Late pick-up of pets (any time AFTER 6:00pm) will be charged an extra$10 fee. Access to pet will be denied and pet will not be released until all charges are paid in full. I understand that I will be charged a daily rate for boarding, not a nightly rate like a hotel. We charge a daily rate because we offer all day pick up/drop off times for your convenience. I also understand that I must pay all boarding fees in advance and that I may not post-date a check. Furthermore, I understand that boarding and daycare are separate services and cannot be combined for a reduced rate. If you drop your dog for daycare you will drop off and pick up during day care hours and be charged a daycare fee. If you bring your dog for boarding you will be charged boarding rates for each day they are here regardless of when you pick them up. This contract shall be legal and binding hereupon and will include any and/or all boarding/daycare services henceforth. This contract contains the entire agreement and shall be binding upon heirs, personal representatives, executors, administrators, or any other persons acting on behalf of either party. Any controversy arising from this agreement will be settled in arbitration according to the Montana Arbitration Laws with the expense of such paid by the owner regardless of prevailing party. Waiver, Medical Power of Attorney, Authorization and Acknowledgements 2016-2017 I, the undersigned owner of a dog(s) named, do hereby release, indemnify and forever hold harmless Nottwoweepeetaukee LLC, DBA Pawsitively Perfect, its employees, officers, agents, subcontractors, customers, visitors, and guests from any and all liabilities (including attorney s fees) arising out of or connected with injuries, illness, loss damage or other harm to myself, my dog(s), my property, and my guests and invitees which arise in any way out of services and /or products provided by or as a consequence of my association with Pawsitively Perfect. I acknowledge that dogs are unpredictable in behavior, and I understand that injuries to my pet, my guests, and myself might reasonably be foreseen to result from my dog s attendance and participation with Pawsitively Perfect. I am fully aware that Pawsitively Perfect encourages off-leash socialization and interaction of dogs in its custody and care and accordingly I assume any and all risks related to Pawsitively Perfect s performance of its services, including but not limited to bites, bodily injury, illness and disease, theft, falls, collisions with vehicles, natural disasters and death. I also agree to assume all liabilities, costs, damages, claims and expenses that may occur as a result of my pet s actions. If, in my absence, my dog should be injured, become ill or suffer an ailment or is otherwise deemed by Pawsitively Perfect to require immediate medical attention, Pawsitively Perfect, in its sole discretion, is authorized to make all health care decisions on my behalf, and may utilize the service of a licensed veterinarian to administer medicine or give other requisite attention to my pet(s) at my sole expense. I hereby indemnify Pawsitively Perfect from any action, claim, demand or lawsuit (including attorney s fees) arising out of or connected with the charges made by any veterinarian caring for my dog(s). I hereby certify and represent that my dog(s) is in good health and has not been ill with any communicable condition in the last 30 days. I also represent that my pet is current on its vaccinations for Rabies, Bordetella, and DHLP-PVO. I further certify that my dog has not harmed or shown aggressive or threatening behavior towards any person or any other dog or animal. I understand that Pawsitively Perfect reserves the right to refuse use of our facilities or to rescind attendance rights for dogs who, in Pawsitively Perfect s sole determination, act aggressively, are undisciplined, evidence inappropriate behavior or who might otherwise be a danger to themselves, other animals or people. I certify and represent that all of the information provided to Pawsitively Perfect in writing or otherwise relating to my dog and myself is true and correct. I agree that my dog may appear on Pawsitively Perfect s website, facebook, and advertising from time to time. I specifically represent that I am sole owner or agent of my pet(s), and that I am authorized to sign this contract. I will immediately notify Pawsitively Perfect of any changes to what I have represented above. By signing below, I acknowledge that I have read, understand, and accept the terms outlined above in the grooming or daycare/boarding contract. To the best of my knowledge, the information that I have provided in the application materials is both accurate and true. I accept sole and exclusive responsibility for these and all other risks and release Pawsitively Perfect of all liability, no matter the cause.
