The following questions will help us determine which of our animals would be a good fit to your family, home, and lifestyle. Do you have any kind of veterinary/medical training? What type of foster home would you be willing to provide? Please check all that may apply. Serial Short Term: This involves an animal that needs to learn either some manners and/or how to become a proper member of a household. This foster lasts for two (2) weeks during which the animal learns how to function in a household and (hopefully) also learns some manners when dealing with people and/or other animals. Long Term: This involves animals owned by the Humane Society for more than one year. They are fostered for a period NOT to exceed six months. They are typically fairly wellbehaved and socialized. Medical Care: This involves animals with medical problems in need of a stress-free environment until the medical problem is alleviated. This may or may not include treating the animal with prescribed medicines at your own home. Mothers with Nursing Babies: These animals are kept until the babies are weaned from the mother, at which point all the animals are returned to the shelter and put up for adoption. Bottle-Fed Babies: This involves either puppies or kittens that are still nursing but either don t have a mother or the mother isn t able to provide the proper care for them. This is a very involved commitment that requires constant feeding and attention every couple hours. The animals are kept until they are old enough to be weaned, at which time all animals are returned to the shelter. Behavior Problems: This involves an animal that has a behavioral problem anywhere from being scared of human or animal contact to being overly dominant with either people or other animals. This requires a commitment from the foster parent to help that animal overcome their issue as best as they can. These animals will stay in foster until their problem has lessened or until the allotted foster time is up. This is typically a long foster period. Emergency Stand-by Fostering: These are short-term foster homes for Humane Society animals to make room for animals that are set to be euthanized by City Animal Welfare. These foster homes last until the next available space comes open at the shelter.

Signed Agreement Terms 1. All pets MUST be returned by the date set on the day fostering begins. If the animal is not returned or other adoption agreements arranged by said date, then the foster family will be sent an invoice for the full adoption price of the animal. 2. Each person fostering an animal is required to bring the animal back one time each month to the Humane Society on scheduled vet days so that the Humane Society s veterinarian can see each animal and to participate in a heartworm/shot regimen provided by the Humane Society. 3. Each person fostering an animal is required to provide the Humane Society a copy of any medical records pertaining to the fostered animal if the animal has received any form of medical attention or treatment NOT administered by the Humane Society veterinarian. The person must also agree to notify the Humane Society within 2 days of mild medical problems (including behavioral disorders). 4. Each person fostering an animal must provide shelter from all elements of either, proper food and water, and provide attention to any special needs the pet may or may not have. If at nay time the Humane Society feels that the animal is not being given the proper care it deserves, a representative of the organization can reclaim possession of the animal immediately. 5. A person fostering an animal from the Humane Society must follow ALL city ordinances and laws that provide for the welfare of the fostered animals. If these laws and ordinances are not followed, it will be the person fostering the animal s duty to pay all fines and fees associated with the animals. 6. The same person may foster a maximum of 3 animals at one time. 7. The Humane Society agrees to provide food and veterinary care expenses for the animals placed through their foster program. Any expenses accumulated by the person fostering an animal will not be reimbursed unless prior arrangements have been made with the Shelter Director. 8. The Humane Society reserves the right to inspect the premises where the fostered animal is housed. If the person fostering the animal does not cooperate with this type of evaluation and recommendations given to them at said time, then the animal will be taken back into the possession of the Humane Society immediately. 9. The person fostering the animal must give a two-week notice BEFORE returning the animal. 10. The Humane Society MUST be notified immediately in case of medical emergency, loss of animal (running away), animal biting, etc 11. The person fostering the animal also agrees to keep the animals properly groomed and exercised. 12. The person agrees to file a monthly report with the Humane Society or in the case of a short-term turn over, file a report for the animal s length of stay. 13. The person agrees to keep a rabies tag on the dog or cat at all times. The Humane Society reserves the right to alter or change these policies and procedures as deemed appropriate by the Board of Directors.
