Boarding & Daycare Contract This is a contract between Ruff House Kennel and Daycare, LLC and the Pet Owner, or his agent. All pets are boarded, or otherwise handled or cared for, by Kennel staff without liability on Kennel's part for loss or damage from disease, theft, fire, death, escape, injury to the pet, or harm to persons, other pets or property by said pet(s), or for other unavoidable causes, due diligence and care having been exercised. Pet Owner, or his agent, understands and agrees that when using boarding service pet(s) picked up after 1:00 p.m., but before 5:00 p.m. on the date of the termination of the agreed upon boarding period shall be charged for a full day's daycare, and thereafter shall be charged for additional days of boarding as of 1:00 p.m. each subsequent day the pet is not picked up. Pet Owner, or his agent, understands and agrees that when using daycare service pet(s) picked up less than four (4) hours after drop off shall be charged a half day daycare rate. Pet(s) left longer than four (4) hours shall be charged a full day daycare rate. Pet(s) left after 6:00 p.m. shall be charged according to boarding rates. If Owner s pet(s) become ill, or if the state of the animal's health otherwise requires professional medical attention, Kennel, in its sole discretion, may engage the services of a veterinarian of its choosing, a n d /or administer medicine, or give other necessary attention to the animal, and the expenses resulting therefrom thereof shall be paid by Owner or his agent. Owner hereby releases Kennel from any and all liability and waives any claim for damages that Owner may now have, or may have in the future, against Kennel for harm or injury caused to Owner s animal by any other animal on Kennel premises No boarding or daycare pet will be released until all charges are paid in full, by cash or check only. Owner, or his agent, shall be and remain liable for all boarding costs, as well as any other charges incurred in the care and maintenance of Owner s Pet. Unless otherwise required by applicable law, if Owner fails to pick up his Pet by the designated time, all services will stop, with the exception of medication administration necessary to ensure Pet health and safety and basic boarding services (food, water, relief time and shelter). Daycare Pets may, at the Kennel s

discretion, be converted to boarding services if the Pet has not been picked up within by the agreed upon time and you shall pay the expenses designated in this agreement. Kennel will attempt to contact Pet Owner by telephone and/or in writing using the information provided by Owner, advising you that if the Pet is not picked up within a reasonable time period, the Pet will be deemed abandoned, and that Kennel will deliver the Pet to a third party adoption partner, Animal Control or other similar government agency. Owner understands that he may lose ownership of his Pet under these circumstances. If Owner fails to pick-up his Pet for any reason, OWNER RELEASES RUFF HOUSE KENNELL AND DAYCARE, LLC FROM ALL FURTHER LIABILITY AND RESPONSIBILITY FOR HIS PET. Owner shall remain liable to Kennel for all unpaid Charges, including without limit the court costs and reasonable attorneys fees incurred in the collection of the Charges. Any personal items brought with your Pet should have a name clearly visible on the item. Kennel is not responsible for loss or damage to any personal item or toy left with your Pet. I hereby authorize Ruff House Kennel and Daycare, LLC to depict in perpetuity the likeness, image, name and other indicia of rights of publicity of myself and my Pet ( Images ) in photographic or other works appearing in any and all media for purposes of promoting, advertising, or marketing current or future events, and I agree that such Images may be used for that purpose without compensation. Owner, or his agent, agrees to pay for reasonable attorney's fees incurred by Kennel for costs associated with the collection of any boarding, grooming, daycare, medical, veterinarian or other charges that shall arise during the boarding period or thereafter. It is understood by Kennel and Owner, or his agent that all provisions of this Contract shall be binding upon both parties hereunto for the current boarding period, as well as all subsequent boarding periods.
