Dear Sir/Madam, Dog Registration Form Thank you for your letter/phone call regarding re-homing your dog. Would you please complete the enclosed form and return it to the office along with a covering letter, two good quality photographs (these can be e-mailed if more convenient to enquiries@dogaidsociety.com) of your dog and copy of the dog s vaccination card if your dog is vaccinated. If you feel your case is particularly urgent, please provide details in the space provided on the registration form. Once we have these details we will display your dog on our website and put his/her details on our re-homing list. We will also check our waiting list for any suitable homes. Once a potential new owner has been found we will carry out a home visit to ensure they match the dog s requirements. You will then be asked to bring the dog into our office on an agreed date and time. A few weeks later we will carry out a second visit to ensure the dog has settled in his/her new home. You should also be aware that the Society will neuter the dog once it has settled, and by signing over ownership to us, under normal circumstances we are unable to return the dog to you. However, If your dog proves to be aggressive to people once re-homed, we will have to return him/her to you, as unfortunately we do not have facilities to retrain aggressive dogs. We cannot re-home dogs who are aggressive to people. We also please ask that you inform us immediately if you re-home your dog privately. This saves the Society from arranging unnecessary home visits. After your dog is re-homed, you are welcome to keep in touch with the Society and we can provide updates on how he/she is settling in his/her new home. If you would like to register your dog with the Society please return the enclosed form. Please do not hesitate to contact us at the above number if you have any queries. If you would like to receive this form via post please call us on 0131 668 3633. Yours faithfully, Clare Rodger, Administrator To find out how we use and protect any information you give us in this form, please see our privacy policy on our website www.dogaidsociety.com or contact us (see details below) and we will send you a copy of it. DOG AID SOCIETY OF SCOTLAND 60 Blackford Avenue Edinburgh, EH9 3ER Tel: 0131-668 3633 Re-homing your Dog Making the decision to re-home your dog is not easy. It may be due to a change of circumstances or simply that you can no longer give your dog the care and attention that they need. The Society offers a re-homing service that prioritises the dog s needs. We do not have kennels so we will ask you to continue to care for the dog in the meantime until a loving new home can be found. Many dogs can find it stressful being left behind in kennels; this can lead to behavioural problems developing which can make finding the right home more difficult. You would also have the uncertainty about how long your dog would have to wait in kennels until they are re-homed. Your dog would much prefer to live in its current house until they can be placed in their new home. This is how the Dog Aid Society re-homes all dogs. 1. The owner is asked to complete a questionnaire about the dog s personality, current circumstances and needs. 2. Once the Society receives the forms the Welfare Officer will be in touch to arrange an assessment of the dog. If suitable for re-homing the dog s information is added to our website where potential new owners can view and apply for the dog. 3. When a suitable application comes in, the Welfare Officer meets the potential new owners in their home to ensure they are friendly, have relevant experience, can financially care for the dog and are going to offer a long term, stable home. 4. The owner will bring the dog to the office with all their belongings and in the same day they are taken to their new home after a full health check at the vet. 5. They will settle with the new owner for two to six weeks depending on the dog and then the Welfare Officer will visit them again to make sure they are settling, happy and wagging their tail! 6. The Society can let the owner know how they are getting on and always encourage updates and photographs from the new owners. Here are some happy dogs that have been re-homed by the Society. Dougall out for a lovely walk Benji snuggled up with his new brother! Poppy is the love of her new owner s life! Your dog s feelings and welfare are very important to us. We only place dogs in exceptional homes. For more information please call 0131 668 3633.
