ATTENTION EXHIBITORS The safety of the dog is our primary concern. By entering this trial, exhibitors acknowledge that they are familiar with the rules and regulations of this sport and of AKC and that their dogs are familiar with and are able to perform all obstacles safely. Exhibitors should follow their veterinarian s recommendation to assure their dogs are free of internal and external parasites, any communicable diseases, and have appropriate vaccinations. Proof of current vaccinations, including rabies, must be available at the trial if requested. All dogs must be held on leash or crated at all times except when in the ring or in the designated agility warm up area. Dogs must be exercised in marked areas only. Dogs are not required to be at the trial until the scheduled time of judging. Exhibitors are responsible for being at ringside when it is their turn to run. Judges need not wait for dogs who are holding up the class. An exercise area will be provided. All handlers must pick up after their dogs. Anyone not cleaning up after their dogs or exercising their dogs in prohibited areas will be asked to leave. In such case, no refund of any fees paid will be made. It is expressly understood that exhibitors alone are responsible for the behavior of their dogs and/or children. Any exhibitor whose dogs and/or children create unnecessary disturbances or engage in unsafe or disruptive behavior may, at the discretion of the Trial Committee, be asked to leave the trial site. In such case, no refund of any fees paid will be made. The Club assumes NO responsibility for any loss, damage, or injury sustained by exhibitors, handlers, or any of their dogs or property and further assumes no responsibility for injury to children. Owners/handlers enter at their own risk and agree to assume responsibility for any damage caused by them, their dogs, or their children. The club reserves the right to remove any animal in distress from a parked vehicle or crate, and all persons attending this agility trial hereby waive any claim for damages against the club for such removal. Entry fees will not be refunded in the event that a dog is absent, disqualified, excused, or barred from the competition by action of the Trial Committee, except that a refund of entry fees minus $5 per run, for bitches in season will be made (a signed statement from a veterinarian must be received by the Trial Secretary on or before the day of the trial before judging starts). No entry fee will be refunded if the trial cannot open or be completed by reason of riots, civil disturbances, fire, an act of God, public emergency, act of a public enemy, or any other cause beyond the control of the organizing committee. OBSTACLES: Obstacles will conform to the requirements in the current edition of the regulations for AKC Agility Trials. TENTATIVE RUNNING ORDER: Master/Excellent JWW, Master/Excellent Standard, Open Standard, Novice Standard, Open JWW, Novice JWW. Dogs will run Tall to Small both days. WAIT LIST: The club is offering a wait list to fill openings created by withdrawn entries after the trail s closing date/time. A maximum of 50 entries will be placed on the club s wait list. All entries, including wait listed entries, must be received by the Trial Secretary prior to closing at 8:00 p.m., September 1, 2018. Any entries withdrawn that are received by 8:00 p.m., on Monday, September 10, 2018 will be replaced by a wait list entry if any are available. If a withdrawn entry is replaced by a wait listed entry, a full refund will be issued for the withdrawn entry. Any wait listed entry that does not move off the wait list into the trial by 8:00 p.m. on September 10, 2018 will not be charged. If you wish to be placed on the wait list, please mark the top of the entry form. HEIGHT DIVISIONS SIMPLIFIED HEIGHT CARD PROCESS When filling out the entry form, there is a new section at the top of the form for exhibitors to designate whether your dog needs to be measured. Please check the box if your dog needs to be measured so that the Trial Secretary knows to include your dog on the Dogs To Be Measured List. It is no longer required that you submit a copy of your dog s jump height card with your entry form or show it during day of show check-in. Owners are responsible for entering the proper height divisions. Judges reserve the right to measure all dogs without a valid height card. Handlers may opt to run in a higher height division in the Regular program, but not in a lower division than their proper height division. Dogs entered in Preferred MUST run in their proper height division. Regular Preferred 8" 4" for dogs 11 inches and under at the withers 12" 8" for dogs 14 inches and under at the withers 16" 12" for dogs 18 inches and under at the withers 20" 16" for dogs 22 inches and under at the withers 24" 20" for dogs over 22 inches at the withers 24C" dogs under 22 may be entered at this height at owner s discretion Each dog must be entered in ONLY Regular classes OR Preferred classes for the weekend. Each dog must be entered in only one height division for the weekend. No ex-pens allowed unless they will be used to hold at least two big or three small dogs.

WORKER VOLUNTEER FORM We will be able to reserve entry space for 35 workers. However, volunteering to work will not guarantee entry into this trial. If you would like to volunteer, fill out the form and return with your entry. Mark WORKER on the outside of the envelope. Workers will receive free lunch, snacks, and drinks and worker vouchers. Name: E-Mail: DOGS ELIGIBLE TO ENTER This trial is open to all dogs fifteen (15) months of age or older that are registered with the American Kennel Club or that have AKC Limited Registration, Purebred Alternative Listing/Indefinite Listing Privileges (PAL/ILP) or an AKC Canine Partners listing number, or approved Foundation Stock Service (FSS) breeds, except bitches in season, aggressive dogs, or dogs suffering from any deformity, injury, or illness which may affect the dog s physical or mental performance. Refer to chapter 2 of the Registration and Discipline Rules for the listing of registerable breeds that may participate. Mixed breed dogs that have been listed with AKC Canine Partners may participate provided their listing number has been recorded on the entry form. COURSE FAMILIARIZATION AND WARM-UPS Handlers in all classes are permitted to walk the course, without a dog, prior to the start of the class. A walk through period will be held prior to each class. There will be no contact obstacle familiarization for novice dogs. A warm-up area, consisting of a regulation agility jump, shall be provided for all exhibitors. Exhibitors may not use the warm-up area to relieve their dogs. Consideration should be given to those teams who are due in the ring prior to other teams wishing to use the jump. SITE INFORMATION: The agility trial will be held at Car-Dun-Al s training facility, 10783 Wolf Drive, Huntley, IL. The training center is in the southwest corner of the second building on the right. It will be indoors on ¾ agility matting. There will be no barking dogs allowed in vehicles, they must be crated inside. Fast food restaurants are nearby. Any additional questions regarding the site and its amenities should be directed to the trial chairperson. PLEASE PICK UP AFTER YOUR DOGS! Potty areas will be clearly marked. Anyone caught not picking up after their dog(s) will be asked to leave without a refund. Directions to the site are as follows: From the east, take I-90 to the Route 47 exit. Go north on Route 47 approximately 3.7 miles to Oakcrest Drive and turn right (known as Algonquin Road to the right). Go less than.1 miles on Oakcrest to Wolf Drive and turn left. From the west, take I-90 to the Route 20 exit. Go west (left) 1.6 miles to Harmony Road. Turn right on Harmony Road and drive 3.7 miles to the stop sign, then turn left to stay on Harmony Road. Drive to the next stop sign and turn right (Main Street). Drive 1.4 miles on Main Street to Route 47 and turn left. Go.4 miles north on Route 47 to Oakcrest Drive and turn left. (Algonquin Road to the right). Drive approximately.1 miles to Wolf Drive and turn left The following hotels have graciously agreed to accept guests with dogs, however we must rely on your assistance in keeping this privilege available for future events, so please keep the hotel rooms and grounds clean. As always, please verify that your chosen hotel s policy has not changed. Qualifying ribbons will be awarded to all dogs receiving a qualifying score. Ribbons will be awarded for 1 st through 4 th place in each class in each height division. A MACH/PACH rosette and bar will be awarded to dogs completing a MACH/PACH title at these trials. If your dog may earn a MACH/PACH at these trials, please notify the trial secretary on or before the morning of the trial. All prizes must be claimed at the trial, none will be mailed. ELECTRONIC TIMING Electronic timing will be used at these trials. Online entries will NOT be accepted

AGREEMENT I (we) agree that the club holding this event has the right to refuse entry for cause which the club shall deem sufficient. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors, governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog. Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend, and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and whether or not the same may have been caused or may be alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons. I (WE) AGREE THAT ANY CAUSE OF ACTION, CONTROVERSY OR CLAIM ARISING OUT OF OR RELATED TO THE ENTRY, EXHIBITION OR ATTENDANCE AT THE EVENT BETWEEN THE AKC AND THE EVENT-GIVING CLUB (UNLESS OTHERWISE STATED IN ITS PREMIUM LIST) AND MYSELF (OURSELVES) OR AS TO THE CONSTRUCTION, INTREPRETATION AND EFFECT OF THIS AGREEMENT SHALL BE SETTLED BY ARBITRATION PURSUANT TO THE APPLICABLE RULES OF THE AMERICAN ARBITRATION ASSOCIATION. HOWEVER, PRIOR TO ARBITRATION ALL APPLICABLE AKC BYLAWS, RULES, REGULATIONS AND PROCEDURES MUST FIRST BE FOLLOWED AS SET FORTH IN THE AKC CHARTER AND BYLAWS, RULES, REGULATIONS, PUBLISHED POLICIES AND GUIDELINES.
