This agreement is made by and between Mutts Galore Animal Rescue and the following individual(s), hereinafter Foster. Work Schedule Employer: City: Occupation: Days/hours you work: Adoption Events Schedule Please list if there is any reasons you will not be able to bring your foster to adotpions events. We have them every Saturday sometimes Sunday and we do understand if you can not make all of them. Foster will provide compassionate and loving care for whatever dog is taken into the home as long as necessary until the dog has been successfully placed in a permanent home. This care includes but is not limited to adequate food, water, shelter, and medication when required. Foster will be patient and tolerant during the transition period (which can last from a few days to a few months). Foster is prepared to work with the dog on basic training, such as housebreaking. Foster will keep the dog primarily inside the home; the dog will not be outside without proper adult supervision Foster will notify Mutts Galore if any medical treatment becomes necessary during the time caring for the dog and will see that the dog get necessary medical treatment as soon as he/she needs it. Foster will not hesitate to provide necessary emergency care. If the situation allows then notify Mutts Galore immediately. Foster must remain flexible and help get the animal to routine appointments if arrangements can t be made by Mutts Galore. Foster will administer monthly heartworm preventative to the dog. If Foster cannot donate the medication, Mutts Galore will provide it. Foster will immediately notify Mutts Galore of any problems or difficulties resulting from caring for the dog. Foster will make certain that the dog is wearing a collar that securely fits with a proper identification tag securely fastened to the collar with a readable name and phone number on the tag. 3
Foster agrees to keep the dog leashed outside whenever he/she is not in a securely fenced area. Foster will not tie or chain the dog in the yard for long periods of time and will never do so unless a competent adult is outside with the dog, and the dog has more than adequate moving space Foster will never leave the dog outside in a yard or elsewhere without adequate adult supervision. Foster will not allow strange people or animals, (people or animals unknown to the dog) to approach the dog without being in full control of the dog and the situation If Foster contributes money toward the care of the dog for which Foster requires reimbursement, Foster will submit any and all receipts directly to Mutts Galore within 30 days of payment. If receipts are not provided within 30 of payment, the money spent will be considered a donation to Mutts Galore, and no reimbursement will be provided. Foster will work cooperatively with Mutts Galore to find the dog a permanent, loving home as quickly as possible. If Foster decides he/she would like to adopt the foster dog, Foster understands and accepts that the procedure for adoption will be the same for him/her as it would be for any other applicant, and Foster will be asked to sign the same contract and pay the same fee to Mutts Galore that any adopter would. Foster understands and accepts that Foster may not keep the dog without the approval and consent of Mutts Galore. Foster understands and accepts that prospective adopters may wish to visit the dog before adopting him/her. Foster is willing to allow people to meet the dog either at his/her home or at a mutually convenient place. Foster understands and accepts that the dog is and shall remain the sole property of Mutts Galore. Foster further understands and acknowledges that Foster does not have any or authority to keep or place the foster dog in other homes or with other individuals. Moving the dog or placing him/her in permanent homes can only be done with the full knowledge and approval of Mutts Galore. Foster understands and accepts that Mutts Galore makes no representations or warranties concerning the health, temperament, behavior, and/or previous ownership of the dog. Foster agrees to indemnify and hold harmless Mutts Galore from any direct or consequential damages arising from this Foster Care Agreement. This includes any and all manner of actions and causes of actions, suits, debts, dues, accounts, bonds, covenants, agreements, judgments, claims, and demands whatsoever arising from or relating to the adoption, placement, and /or possession of the dog. The main purpose of Mutts Galore Animal Rescue is to find responsible and reliable homes for all dogs placed in our care. Fostering a dog is a very important decision and one that not only affects your family, but the dog you decide to foster. Millions of dogs are euthanized each year because people do not seriously consider the care and maintenance of animals. Time and attention are major factors that need to be considered when taking a dog into your home. If behavior problem arises, the foster agrees to contact Mutts Galore Animal Rescue to discuss the issues. By submitting this form, you agree to have read and understood the above information. You are certifying that the questions have been answered truthfully and in full. False information or misrepresentation will result in your disqualification to foster from Mutts Galore Animal Rescue, and / or the removal of your fostered pet.
