Adoption Fees: Adoption fees help defray the costs of all rescued animals Food, Heartworm Testing and Prevention, Flea and Tick Prevention, Spaying/Neutering, Vaccinations, Microchipping, Worming, and any other medical expenses that may be necessary i.e., arthritis medication for the older animals, eye ointment and salves, antibiotics for those who come with wounds or upper respiratory infections, Heartworm Treatment for HW+ dogs, et al. According to the estimated* age of this dog, his/her adoption fee is $. $300 Great Pyrenees Puppies (Less Than 1 Year Old)* $250 Great Pyrenees (1 to 2 Years Old)* $200 Great Pyrenees (2+ to 5 Years Old)* *as ascertained by examining veterinarian $150 Great Pyrenees (5+ to 8 Years Old)* $100 Great Pyrenees (8+ Years Old)* Each Wayside Rescue and Sanctuary Pyr will have received the following if age permits: Spayed/Neutered Vaccinated against Rabies (certificate provided) Vaccinated against Distemper, Hepatitis, Parainfluenza, and Parvovirus (DHPP) prior to transferring to WSRS or if Pyr comes with out-dated or no vaccination records Pyr will have received a 7-Way Vaccination that also includes Adenovirus Cough (Kennel Cough) and two (of four) types of Leptospirosis within two days of transferring to WSRS Re-Vaccinated with 7-Way as a Booster within two weeks of transferring into the WSRS Tested for Heartworms (HW) Negative Positive* Provided with Monthly HW, Flea and Tick Prevention, with Brand, Dose and Last Date Given (See Medical Records) Heartworm Positive Dogs will have been on the Advantage Multi/Doxycycline Protocol (See Attached Information Sheet) Microchipped * If this Pyr has tested Positive for Heartworms, treatment will be discussed at length and this adopter may wish to complete a Foster to Adopt Contract rather than this Adoption Contract. Temperament and Health (Initial) While the Rescuer is of the opinion that this Pyr already possesses the temperament and personality suitable for a family pet, the history of this animal is unknown; therefore, the Rescuer is unable to give any guarantees of any kind of temperament or personality except what has been observed in Foster Care. (Rescuer Initials) Disclosure Wayside Rescue and Sanctuary Rescuers will disclose any observations that may help the transition of this Rescue Dog into your family. (Rescuer Initials) This Rescue Dog has been seen by a certified veterinarian and has been deemed to be healthy to that vet s knowledge without administering expensive diagnostic tests. Any knowledge contrary to this statement is listed below and any medical treatment given to this dog is documented by veterinarian record(s). (Initial) The Adopter agrees to have this Pyr examined by a licensed veterinarian within seven (7) days of adoption. (Initial) For whatever reason, the Adopter has the right to return this Pyr to Wayside Rescue and Sanctuary within 30 days of adoption for a 50% Refund. (Initial) If at any time in the future you are unable to care for or keep this Pyr, the Pyr must be returned to Wayside Rescue and Sanctuary or the Adopter must get approval from WSRS to give this Pyr to a third party. If returning the Pyr to WSRS, I agree to notify WSRS immediately and to make arrangements to return the animal to the organization s adoption center and to abide by relinquishment procedures. (Initial) This Pyr may NOT be surrendered to a shelter or pound. (Initial) If at any time Wayside Rescue and Sanctuary is notified that this Pyr is not being treated as indicated in this contract, a WSRS designee may investigate the charge. Level of Care The Great Pyrenees/Mix you are adopting is a Companion Animal requiring a specified minimum Level of Care. Level of Care required for all Great Pyrenees/Mix Companions (Pyr) adopted from Wayside Rescue and Sanctuary: (Initial) The Pyr will be a member of my/our family and will be treated as such. 1. The Pyr will not be relegated to the outdoors, but will live in the house as a companion animal with the adopter/adopter and family; a. The Pyr will not be chained to a doghouse, or chained outside to any type of object or outbuilding for any reason, b. The Pyr is not to live in a garage, doghouse, or any type of outbuilding. 2. An outside shelter, such as a doghouse, is not acceptable as residence, but can be used only when needed in extreme heat, cold, or rain when the dog is let out into the yard for short periods of time; 3. The Pyr will never knowingly be placed in a dangerous situation, such as attack proximity to an aggressive dog, in the street without a human attached to it with a leash, objected to fireworks, in the back of a pick-up truck without being in a proper and restrained carrier, etc.; 4. The Pyr shall not remain in any vehicle in extreme heat, with windows down, or unattended for any amount of time; 5. At all times, the Pyr will have access to fresh water indoors and out of doors; 6. The Pyr will be provided a nutritious dog food and the appropriate amount of food for his/her size, weight and age; 7. The Pyr will have suitable outdoor exercise as dictated by age and health; 8. The Pyr will have optimal time for daily family interaction. Wayside Rescue and Sanctuary Page 4

The Pyr may not under any circumstances be crated or otherwise confined for more than 8 hours if an adult and no more than 3 hours if a puppy in a 24 hour period; 2. If crated, the crate shall be large enough for the Pyr to lie comfortably on its side; 3. Fresh water will be easily accessible. (Initial) Control of your Pyr 1. The Pyr will not be allowed off leash except within adequate fencing; 2. The Pyr will never be allowed to roam free unsupervised; 3. The Pyr is not to be left with small children at any time when unattended by an adult--the Pyr is not to be used as a babysitter. (Initial) Identification 1. The Pyr will wear an appropriate collar or harness with an ID tag and Rabies tag when outside of its home. The identification tag will include the Adopter s name and phone number. (Initial) Veterinary Care 1. The Adopted Pyr must be Licensed in accordance with the laws of the jurisdiction in which the Adopter resides. Some Counties/Cities allow 3-year Rabies Vaccinations after the initial 1-year Vaccination, others require annual Rabies Vaccinations. 2. The Adopted Pyr must receive Annual Vaccinations Distemper, Parvo, et al. 3. The Pyr must remain on Heartworm Preventative all year round. 4.
