Foster Application Rescue website: Gainesville Cockatiel Rescue Contact: Jennifer Hugus E-mail: betta5@yahoo.com Caring for a bird is a serious responsibility. Each potential foster parent must be willing to provide adequate housing, good nutrition, exercise, and stimulation for the bird. It is important that you educate yourself on the species you wish to foster and ask any questions on aspects of care you are uncertain about. Please fill in all the answers and e-mail this application back to: betta5@yahoo.com with the subject Foster Application. All questions must be answered to proceed. Bird Fostering agreement: I am requesting to foster a bird within my home and will take appropriate care of any bird in my care. I will give the bird(s) fresh food and water daily and set aside some time each day to interact with it/them. I will contact the rescue immediately and return the bird(s), if I can no longer care for the bird. I will not sell, give away or breed the bird(s). I understand that the bird(s) belong to Gainesville Cockatiel Rescue (Jennifer Hugus) and must be returned immediately upon request. I agree not to hold Jennifer Hugus or other representatives of Gainesville Cockatiel Rescue personally or otherwise responsible in the event that I sustain emotional, financial, personal or property damage while serving the rescue/caring for the bird (s). I agree to follow any special instructions to care for the bird (s) I am fostering. I understand that effort is made to make sure that birds are healthy before being placed in foster care but I will not hold Jennifer Hugus (Gainesville Cockatiel Rescue) responsible for any illness that appears. I will use proper quarantine procedures if I have other birds to avoid the transfer of any disease that may be carried. Any known diseases/issues will be disclosed before the bird is offered for fostering. Unless you are willing to take the foster bird to the vet and pay the vet bills yourself (without reimbursement), any bird found to be ill must be returned to Jennifer Hugus (Gainesville Cockatiel Rescue) immediately for care. I have read and understand the agreement above in its entirety and agree to the terms. Name: Household Information Signature (to be added later):
