DOGS HOMES OF TASMANIA (operated by the Tasmanian Canine Defence League) PO Box 7 Lindisfarne TAS 7015 E-mail: fostercare@dogshomesoftas.com.au (Trial) Application to become a Foster Carer for the Dogs Homes of Tasmania Thank you for your interest in becoming a Dogs Homes of Tasmania foster carer. The objective of fostering is to house the dog in private care until a suitable home can be found or they are able to return to the Home. Not only do foster parents maximize the number of dogs rescued, they also help to care for dogs for whom a kennel environment is not suitable: puppies with immature immune systems, dogs recovering from major surgery, or those needing one-on-one behaviour rehabilitation or a break from the Home. Fostering can be extremely challenging as well as rewarding, and should be entered into carefully. Please see Terms and Conditions of Foster Care (attached). We are committed to providing the best possible care for our dogs in both the Dogs Home and Foster Care setting and we ask that you complete the following application with this in mind. DOGS HOMES OF TASMANIA Operated by the Tasmanian Canine Defence League TERMS AND CONDITIONS OF FOSTER CARE As a Dogs Homes of Tasmania Foster Carer, I agree: 1. That I am over 18 years of age. 2. That fostering a dog requires approval of my Foster Care Application, including a home visit by an appropriate member of the Dogs Home Foster Care team. 3. To have my Veterinarian contacted as part of my foster care application. 4. To provide my foster dog with a loving environment and foster the skills they will need to become part of their new adoptive family. 5. That I will maintain contact via phone and/or email with the Foster Care Co-ordinator to keep the Dogs Home updated on the status and progress of my foster dog. 6. To contact the Foster Care Co-ordinator in the event of a foster dog requiring non - emergency veterinary care. 7. To take my foster dog to the nearest Veterinary Surgery in the event of an emergency (life or death situation). 8. To contact the Foster Care Co-ordinator in the event I can no longer look after the dog placed in my foster care. 9. That while I will be able to have input into the adoptive home of the dog in my foster care, the final decision will be made by the Dogs Homes of Tasmania. 10. That I will be offered the first right of refusal should I wish to adopt the foster dog in my care. 11. That I will report any incident that may impact on the dog s adoptability to the Foster Care Co-ordinator. 12. To follow the prescribed Veterinary medication charts and schedules set down by the Dogs Homes of Tasmania. 13. To ensure the foster dog wears a collar with Dogs Homes of Tasmania identification tag attached at all times. 14. To ensure that I will recognize the limitations and capabilities of each individual dog and will not place them in situations or environments that may lead to excessive stress or harm to the dog, other animals or people. 15. That I will not leave the foster dog unattended with children under the age of 10 years. 16. That I will not let the foster dog off lead unless in a secure area, or, after a period of no less than 2 weeks and with confidence in the dogs recall, in a designated off lead area. 17. That I will, whenever possible, be responsible for transporting the foster dog to and from check-ups and any adoption events given my availability. 18. That I am 100% responsible for the actions of the dog while it is in my care. 19. That I will not place my foster dog in the care of another person for any reason unless with prior agreement of the Dogs Homes of Tasmania. 20. The foster dog remains the property of the Dogs Homes of Tasmania at all times and I must return the dog to the Dogs Home site when requested to do so. 21. To let the Foster Care Co-ordinator know any changes to my contact details or details pertaining to the foster application form. 22. To return equipment given by the Dogs Homes of Tasmania at the end of a fostering term. 23. To not provide any false or misleading information in the adoption application or in any communication about a foster dog. As the Dogs Homes of Tasmania Foster Care Co-ordinator I agree: 1. To provide as much information about the foster dog as possible. 2. To ensure the foster dog is free from illness and is vaccinated, microchipped and desexed prior to being fostered unless specifically agreed to by foster carer (these may be the reason fostering is required). 3. To provide equipment needed for fostering a dog from the Dogs Homes of Tasmania. 4. To provide a list of Veterinary instructions to accompany any medication required. 5. To provide guidance and support in a timely manner for any issues that arise during a fostering period. 6. To give a reasonable amount of notice prior to requiring the transport of the dog for veterinary check-ups or adoption events. 7. To ensure all potential adopters are screened for suitability prior to visiting foster carers home to minimise time and privacy issues. First written and ratified: April 2010 5

8. To accept the immediate return of any foster dog should the foster carer no longer be able to continue fostering the dog. 9. To pay the costs of all veterinary care that has received prior approval from the Dogs Homes of Tasmania (unless in the event of emergency care where the dog should be taken to the nearest veterinary surgery at the expense of the Dogs Homes of Tasmania). Please note that as a Foster Carer it is important that you are aware that the dog you bring into your home may not be housetrained, may chew inappropriate things, bark or display other undesirable behaviour. Your generosity and patience in teaching appropriate behaviour is providing this dog with crucial skills to make his/ her successful transition to a new family possible. The Foster Care Co-ordinator and support team will provide you with as much information about the dog as possible and will be available to guide and assist with any difficulties you may experience.
