No telephone, email or fax entries will be accepted. Any changes to entries, including cancellations will be accepted until the closing date. Changes must be made to the Secretary by telephone, email or in writing. Cancellations received after the closing date will not be eligible for a refund of fees. Requests for move ups are permitted until Monday, April 7 th at noon. Move ups to a higher level class are permitted between trials provided that the exhibitor properly completes the Class Move Up form before the next trial begins.

Entry Forms All entry forms must be complete and fully legible. Incomplete or illegible entry forms do not constitute valid entries and will be returned. If you are going to request a jump height modification, please indicate modified height on entry. Entries will be accepted on a firstcome-first served basis. Initial confirmations will be sent out as they are received. If entry limit is reached before entries close, a wait list will be established. Exhibitors will be promptly notified if they have been placed on the wait list. All final confirmations and final judging program will be sent by e-mail. If you would prefer your final confirmation mailed, please enclose a self-addressed, stamped envelope for this purpose. If your dog has not earned an obedience or rally title you must enter the Novice A class. If you have a Novice CDSP Title you may enter Novice C. If your dog earns an obedience or rally title after you have completed this entry and submitted it, you must notify the Trial Secretary immediately. *There is no camping at the trial grounds, and motor homes are not allowed. No Saturday evening setup. No one is allowed on the property after the show and not before 8:00 AM on Sunday. General Information CDSP Registration numbers: Entries will not be accepted without a CDSP registration number. To obtain a registration number, please fill out the registration form on page 7 of this premium and mail directly to Companion Dog Sports Program. Trial conditions: These trials will be held indoors on ¾ rubber mat flooring. The building is heated & air-conditioned but please dress accordingly. The entrance is on the 2 nd floor. The building is not handicap accessible. Food: Coffee and donuts will be available in the AM and water, soda & snacks throughout the day. Please bring your own lunch. There is a restaurant, Giant Supermarket, and a pizzeria all within walking distance Crating: Minimal crating space inside, plenty outside space. Parking: Extra parking behind Giant Supermarket. Housekeeping: Dogs must be on leash while on show grounds. YOU MUST pick up after your dog and yourself. Please note: Any dogs removed from the premises for aggressive or threatening behavior are not eligible for a refund and not eligible for any placements and/or awards. Exhibitors are expected to conduct themselves in a sportsmanlike manner. Any exhibitor exhibiting unsportsmanlike conduct will be declared ineligible and asked to leave the premises. Exhibitors are expected to be ready at ringside when their number is called. Judges will not delay judging for any exhibitor not ready when called. Notice to Exhibitors Competitors, through submission of entry, acknowledge that they are knowledgeable of CDSP Competition Obedience rules and regulations including but not limited to the following rules regarding entry: This trial is open to all dogs registered with CDSP who meet the qualifications set forth within, being at least six months of age, including purebred and non-purebred, except bitches in season or dogs with disabilities, injuries or illnesses which may cause the dog pain or great discomfort, or dogs exhibiting signs of aggression that threaten the safety of other dogs or humans. All dogs must be registered with CDSP obedience to compete. Dogs must be shown by a member of the owner s immediate family: mother, father, spouse, life partner, sibling, child, grandparent, or grandchild. Safety shall always be of foremost consideration in actions and conduct by handlers at all times. Handlers, through entry at this event, accept full responsibility for themselves and the actions of their dogs. The organizing committee may refuse any entry for any reason. THERE SHALL BE NO REFUND for entries withdrawn or in the event a dog and/or handler are dismissed from competition, regardless of reason for such dismissal. There will be no refunds if the trial has to be cancelled for any reason (i.e. riots, civil disturbances, fire, act of God, public emergency, act of a public enemy, strike or any other cause beyond the control of the organizers) or for absent entries. Refunds will be made for withdrawn entries only if request is made prior to

closing date. Refunds will be made for bitches in season if request is accompanied by a note from the exhibitor s veterinarian. Returned checks do not constitute a valid entry. There will be a $35.00 service charge for returned checks. Payment of entry fees and service charges shall be made in cash or money order within 10 days of notice to exhibitor of returned check. Any unpaid fees or shortfalls must be paid prior to the start of the event or entry is cancelled without recourse by owner/handler. All dogs shall be shown in a flat buckle or snap collar, plain harness, or properly fitted martingale (limited slip) collar. No choke collar, prong collars, shock collars, no-pull harnesses, martingale leads, or head collars are allowed. All dogs must remain on leash unless they are in the ring. All competitors through entry at this event waive any and all rights relative to video broadcast of this event. Competitors shall have the right to videotape portions of this event for their personal use only. No portion of this event may be videotaped for commercial or other purposes. All video, broadcast, and telecast rights for this event are the exclusive property of the CDSP or the trial host(s). Owners with disabilities are encouraged to compete. Any necessary modifications to the exercises must be provided by the handler to the judge and approved by the judge. Upon successful completion of an exercise, the owner may choose to briefly pet or touch the dog as a reward. The owner may also choose to give the dog food as a reward. Food may NOT be used as a lure to help the dog complete an exercise. Food rewards must be contained in pockets and only be present in the owner s hand in between exercises. Any petting, touching, or feeding that significantly interrupts the flow of performance shall be scored accordingly. GENERAL AGREEMENT I (we) agree that Oakes Dog Training, Inc. has the right to refuse this entry for any cause which the organizing committee alone shall deem to be sufficient. In consideration of the acceptance of this entry, of the holding of the event, and of the opportunity to have the dog participate and/or to be judged, I (we) agree to hold the organizing committee, the hosting club, Companion Dog Sports Program, including their members, officers, directors, agents and employees, sponsors of the event, and owners of the premises upon which the event is held and their employees, harmless from any claim for loss or injury that may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or upon the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim. I (we) further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, death or otherwise, and from any claim for damage or injury to the dog, whether such loss, disappearance, appearance, theft, damage, or injury be caused or alleged to be caused by the negligence of the parties aforementioned, or by the negligence of any other person, or any other cause or causes. I (we) hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death resulting at any time there from, sustained by any person or persons, including myself (ourselves), or on account of damage to property arising out of or in consequence of my (our) participation in this event, howsoever such injuries, death, or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents or any other persons. I (we) further acknowledge that I (we) have read, understand, and will abide by the Official Rules and Regulations of Companion Dog Sports Program and the provisions for entry as set forth in the Trial Schedule of which this entry form is a part. I (we) acknowledge and represent that the information supplied by me (us) herein is correct to the best of my (our) knowledge; if any portion of this information is found to be invalid or insufficient to establish the validity of my (our) entry, such finding shall render my (our) entry invalid in its entirety, for which there shall be no refund of fees paid. Further, I (we) hereby grant to, Companion Dog Sports Program their representatives, agents, and assigns, all rights and permissions to use or appropriate my and the dog s name, biography, likeness, photograph, voice, performing persona, or other indicia or identity for broadcast, telecast, cablecast, transmission, or distribution in any format or media now known or hereafter to become known. I (we) hereby release Companion dog Sports Program & Oakes K9 Training, Inc. their representatives, agents, and assigns from any claim or cause of action for invasion of the rights of privacy, right of personality, or any similar right.
