Greyhound Pets of America/Lexington, KY Adoption/Foster Application Thank you for your interest in adopting or fostering a retired racing Greyhound. Greyhounds make wonderful companion pets and will give you many years of love and devotion. Adopting a Greyhound is a serious responsibility requiring a longterm commitment. This application has been designed to evaluate potential adopters to ensure that each person who adopts a Greyhound is not only aware of that responsibility, but also to address the suitability and permanence of each Greyhound placed in a home. We have worked in Greyhound adoption for years and have seen many happy endings, and unfortunately we have also seen a few sad ones. We have only the best interest of the Greyhound in mind. Before signing this application, please know that Greyhounds can live 10 to 14 years, making this a serious, longterm commitment. It is very traumatic for a dog to be returned by its family once it has settled into its new home. Furthermore, note that the Greyhound breed is not for everyone. These dogs rely on us to place them into appropriate, loving, responsible homes. All adults in your household must agree to this adoption and sign the application. Are you prepared for a commitment to a dog for the remainder of its life? Do you have the financial resources to meet the medical needs of a large dog, including: high quality food, monthly heartworm preventative, monthly flea/tick preventive medications, and routine & emergency vet care? Greyhounds are sighthounds, which means their eyesight is highly developed. They are also friendly and curious, as well as filled with the desire to RUN when given the chance. A greyhound, therefore, can never be trusted off-leash unless in a secure, fenced area. A loose greyhound is usually a dead greyhound. Do you understand this requirement and agree to keep your dog safe by always leashing it unless in a securely fenced area? Heartworm is common in this part of the country and very deadly. Are you willing to keep your greyhound on a monthly preventative? Greyhounds are not a breed well-suited to living outside. For many reasons, a retired greyhound wants and needs nothing more than to live inside with its new family. They certainly enjoy time outside, but long periods alone unsupervised invite a whole host of problems. Do you understand this requirement and agree that your greyhound will be an inside member of your family? Greyhounds fresh from the track sometimes feel the need for the security of a training crate. We advocate the limited use of these crates when it appears the greyhound needs it in order to learn the rules of home life and remain safe when unsupervised. Remaining comfortable with being in a crate is also helpful in the case of vet trips, boarding kennels, illness or injury, etc. Are you willing to buy and learn to use a crate to help your dog adjust, if it seems to be required? Also, do you understand that it is unnecessary, and cruel, to crate a dog all day and at night? Adoption Requirements (initial each, which will indicate that you agree to abide by each clause) It is vital to keep your greyhound on leash whenever it is outdoors, unless it is in a fenced-in area. NEVER "trust" it not to run away. The greyhound MUST be an indoor pet, and must NEVER be chained outside, or left outside for long periods of time. You must keep a collar and ID tag on the greyhound at all times. If your greyhound should become loose, or lost, you are REQUIRED to notify GPA/LEX immediately. You are responsible for providing both routine and emergency medical care for the greyhound. You MUST notify GPA/LEX if for any reason you cannot keep your greyhound, and you MUST agree that you will return the dog to GPA/LEX. Page 8 of 10 v 3/24/2018

Adoption Fee The $350.00 (as of 1/1/2017) non-refundable adoption donation helps offset a portion of the cost of initial veterinary care and associated expenses for recently retired dogs. We spend on average over $500 per greyhound that enters our program. Each GPA/LEX dog has had the following services: physical exam, spay/neuter, dental, heartworm testing, and monthly heartworm & flea preventative, worming, vaccination updates, nail trimming. Each greyhound comes with: information packet, martingale collar, tag collar, leash, GPA/Lex identification tag, muzzle, and the lifetime support of GPA/LEX. Deposit To begin the adoption process we must receive your $50.00 deposit with your application. If your application is approved, this deposit will be applied to your adoption fee. If GPA/LEX does not approve your application, your deposit will be returned. If you withdraw your application after the home visit, your deposit is considered a non-refundable donation. The Legal Part In submitting this application, I am authorizing GPA/LEX to investigate the answers given on this form, including veterinary records. I understand that in the interest of making the best placements possible, they have the right to deny any application or ask for additional information. GPA/LEX will NOT use this information for any other purpose, in order to protect my privacy.
