FOI REF: 16/401 29 th September 2016 FREEDOM OF INFORMATION ACT I am responding to your request for information under the Freedom of Information Act. The answers to your specific questions are as follows: Under the Freedom of Information Act, please could you provide me with an extract from the hospital episode data for every admission where the external cause was an animal for 2015 and 2016 to date, including: The date of the admission. The age of the person injured. The specie of the animal that caused the injury. The variety/breed of the animal that caused the injury. A description of the injury. If the injury was fatal. (Yes or No) Including but not limited to incident that were logged under the following codes: W53 - Bitten by rat W54 - Bitten or struck by dog W55 - Bitten or struck by other mammals W56 - Contact with marine animal W57 - Bitten or stung by nonvenomous insect and other nonvenomous arthropods W58 - Bitten or struck by crocodile or alligator W59 - Bitten or crushed by other reptiles X20 - Contact with venomous snakes and lizards X21 - Contact with venomous spiders X23 - Contact with hornets X25 - Contact with other specified venomous arthropods X26 - Contact with venomous marine animals and plants X27 - Contact with other specified venomous animals X28 - Contact with other specified venomous plants X29 - Contact with unspecified venomous animal or plant

Please see attached excel document and note the further information provided below. Whilst I can confirm that we hold the information in relation to the age of the patient and date of admission, we are not prepared to disclose this information as the patient could be identified and this would breach the principles of the Data Protection. I am therefore applying a section 40(2) exemption under the Freedom of Information Act 2000 Personal Information of third parties. However, we have provided the age range for each patient and the month of admission in response to your Freedom of Information request. Further details regarding the variety, breed or specie of the animal that caused the injury are not held centrally. To obtain further details would require a manual review of patient records. Section 12(1) of the Act allows a public authority to refuse to comply with a request for information if the authority estimates that the cost of compliance would exceed the appropriate limit, as defined by the Freedom of Information and Data Protection (Appropriate Limit and Fees) Regulations 2004 (the Regulations). These state that this cost limit is 450 for public authorities which are not part of central government or the armed forces. The costs are calculated at 25 per hour per person regardless of the rate of pay, which means that the limit will be exceeded if the work involved would exceed 18 hours. The Trust estimates that the cost of complying with this request would significantly exceed the above limit. If I can be of any further assistance, please do not hesitate to contact me.
