The inspection of January 05, 2009 is as follows: 21.21 (a) - Maintenance The previous unsatisfactory on December 10, 2008 has been corrected. The owner fixed the broken wires in the outside kennel area. The medium-sized hole which was approximately 14 x 6 was filled and repaired. 21.26 Ventilation The previous unsatisfactory on December 10, 2008 has been corrected. Sufficient air flow was available to the animals being housed inside the home. 21.29 (b) Sanitation The previous unsatisfactory on December 10, 2008 has been corrected. The owner has cleaned the primary enclosures and sanitized underneath the piping. 21.29 (c) Housekeeping/pests The previous unsatisfactory on December 10, 2008 has been corrected. The owner cleaned the cobwebs throughout the barn and picked up the open bags that were throughout the lower kennel area. Additional Information: An accumulation of food and hair was found outside of the primary enclosures inside of the home on January 05, 2009 This Warden directs the kennel owner to take the following action(s) : The buildings and grounds of the kennel shall be maintained and kept cleaned. The kennel operator is responsible for taking any and all actions necessary to come into compliance. The inspection of January 22, 2009 is as follows: 21.21 (a) Maintenance The previous unsatisfactory on December 10, 2008 has been corrected. The owner fixed the broken wires in the outside kennel area. The medium-sized hole which was approximately 14 x 6 was filled and repaired. During the January 22, 2009 inspection, broken fencing was found in outside play area. The wires from the fencing were sticking inwards which may cause injury to the dogs in the kennel. A piece of shredded aluminum capping had jagged edges which may also cause injury. Holes were found throughout the kennels perimeter which may not contain the dogs in the kennel. This warden directs the kennel owner take the following action (s):

Make all the necessary repairs to the fencing in order to create a safe condition for the dogs. Kennel owner must remove the aluminum capping which had jagged edges. Kennel owner must fill all holes that were found throughout the perimeter. *Warning Issued 21.21 (b) Interior Building Surfaces Rusted piping was found throughout the outside and inside of the kennel. This warden directs the kennel owner take the following action (s): Make repairs to all rusted piping so that they are water resistant and can be readily sanitized. *Warning Issued 21.21 (c) Drainage An accumulation of ice was found inside several outside pens. This warden directs the kennel owner take the following action (s): Adequate drains must be provided to rapidly eliminate excess water. *Citation to Follow 21.23 (a) Space There was a German Shepard being housed inside a traveling crate which did not allow the dog to stand erect. This warden directs the kennel owner take the following action (s): Kennel owner must remove the dog from the enclosure and make sure that all primary enclosures shall be constructed to allow each dog to stand erect. *Warning Issued 21.28 (c) Food/water receptacles Chewed water buckets were found in the kennel area. This warden directs the kennel owner take the following action (s): Remove all chewed water receptacles and replace them with durable receptacles that can be maintained and kept clean. *Warning Issued 21.29 (c) Housekeeping/ pest An accumulation of dog hair, dog food and cobwebs were found in the inside of the kennel. This warden directs the kennel owner take the following action (s): Kennel owner must clean and remove all cobwebs, dog food and dog hair from kennel area in order to maintain the kennel area clean. *Citation to follow 21.41 (a) Records kept Kennel records were not accurate during the inspection on January 22, 2009. This warden directs the kennel owner take the following action (s): All records must be completed on dogs within the kennel and dogs that have left the kennel. *Citation to follow 21.30 Condition of Dog

St. Bernard s were observed in outside area that needed immediate grooming. This warden directs the kennel owner take the following action (s): Immediately have the dogs groomed to prevent the dogs from harboring infectious and contagious diseases. *Warning Issued Recheck to be done.

The kennel operator is responsible for taking any and all actions necessary to come into compliance. Re-inspection of kennel took place on March 3, 2009. A total of 75 adult dogs and 23 puppies were located on the premises at the time of inspection. Kennel owner s 2009 kennel license was applied for and is pending in Harrisburg. Two (2) fire extinguishers and four (4) smoke detectors were located inside the kennel premises. Inspection was performed with Warden Megan Caskey. *** Actions directed to be taken below are not all that may be required of the kennel operator to come into compliance. The kennel operator is responsible for taking any and all actions necessary to come into compliance. The re-inspection of March 3, 2009 is as follows: 21.21(a) - Maintenance There were several broken wires in the outside kennel area near entrance. There were several holes in the outside kennel area that needed to be filled. There was a chewed dog door that needs to be repaired or replaced. This Warden directs the kennel owner take the following action(s): Remove or repair broken wires in outside kennel area near entrance. Fill all holes in the kennel area to prevent injury to the dogs. Repair or replace chewed door. (Warning issued). 21.21(b) - Water resistance There was rust inside the kennel area where the dogs are housed. Kennel owner has the paint to make the corrections but states that the weather does not allow her to paint at this time. This Warden directs the kennel owner take the following action(s): All areas that have rust which the dogs have contact with must be painted in order to be incompliance with the state regulations. (Warning issued). 21.26 Ventilation Sufficient air/ventilation is needed in the kennel area (inside the home) to prevent the strong smell of ammonia. This Warden directs the kennel owner take the following action(s): Kennel owner must turn on exhaust fans in the home in order to allow the ammonia smell to dissipate. (Warning issued). 21.29(c) Housekeeping/pest An accumulation of hair was found along the fence line. Old food was found underneath the piping in the inside kennel area. The ceiling in pool area has a large amount of mold/mildew build-up. This Warden directs the kennel owner take the following action(s): All the hair along the fence must be removed. Old food must be cleaned on a daily basis or often as necessary to prevent build-up. The ceiling in the pool area must be repaired and cleaned in order to prevent mold/mildew build-up. (Citation to follow). 21.41(a) Records kept Records of dogs in the kennel and dogs that have left the premises are not current. This Warden directs the kennel owner take the following action(s): All records must be updated in order to be in compliance with state regulations. (Warning issued). All violations were explained to kennel owner. Recheck will be done at a later date.

There were 85 adult dogs and 37 puppies located on the premises at the time of inspection. The 2009 application has been received and is pending approval. There were also several holes in various outside kennel areas that were big enough for dogs to escape from the enclosure. In the runs behind the house, there were holes in the fence. Replace the broken joint in the chain link fence with a pvc joint. Fill in the holes with stone so that the

dogs cannot escape from the enclosure. Repair the holes in the fence in the outside runs behind the house. *WARNING ISSUED* Since the previous inspection the kennel owner has removed the broken wires from the gate in the front of the house and has removed the chewed dog door. 21.21b / Act 207b There was chewed/scratched/exposed wood and rust on the chain link fences throughout the kennel. All chewed/scratched/exposed wood needs to be covered, painted, or sealed. *WARNING ISSUED* Since the previous inspection the kennel owner has started painting the rusted chain link fence. 21.21c / Act 207b In both the outside runs in the front and behind the house there was inadequate drainage. Fill in the runs with more stone to provide adequate drainage. 21.22 / Act 207b There were a large number of dogs in the outside front yard area. This warden along with the other two wardens witnessed the majority of the dogs attacking one dog in this area. The owner then went into the yard and broke up the attack and separated the dog being attacked from the others. The owner reported there were no injuries to the attacked dog. Segregate dogs that display ferocious or aggressive behavior so that they do not have access to the other dogs. *WARNING ISSUED* 21.26 / Act 207b There was a strong ammonia smell in the house area of the kennel. There were no fans running or windows open when this warden entered the house. Kennel owner must turn on fans and/or open windows to decrease the smell of ammonia and increase the ventilation in the house area of the kennel. *WARNING ISSUED* 21.29c / Act 207b There was a build up of debris and hair in between the pens in the whelping area. There was a build up of debris between the runs outside behind the house. There was a swarm of insects in the front yard where there was feces built-up in the stones. Remove the screws or cover the sharp ends so that the dogs cannot come into contact with them. Replace the

small section of broken fence or repair and secure the section that is currently there. Repair the hole in the fence in the runs behind the house. *WARNING ISSUED* Since the previous inspection the kennel owner has replaced the broken joint in the chain link fence, filled in all the holes with stone so that the dogs cannot escape, and repaired the holes in the fence in the section behind the house. 21.21b / Act 207b There was chewed/scratched/exposed wood and rust on the chain link fences throughout the kennel. All chewed/scratched/exposed wood needs to be covered, painted, or sealed. *WARNING ISSUED* Since the previous inspection the kennel owner has continued to paint the rusted chain link fence. 21.21c / Act 207b This previous unsatisfactory has been corrected. The owner has added more stone to areas that previously had inadequate drainage. The owner also added a rain gutter system to the small dog building behind the house. 21.22 / Act 207b This previous unsatisfactory has been corrected. The dogs were separated either by themselves or in smaller groups during this inspection. 21.26 / Act 207b This previous unsatisfactory has been corrected. The smell in the house area of the kennel has decreased significantly. 21.29c / Act 207b There was a build up of debris between the runs outside behind the house and in the lower big building of the kennel. The ceiling in the pool area continues to have a large amount of mold/mildew build-up. At the end of this inspection the kennel owner s husband stated the pool is going to be closed. The build up of debris and hair needs to be removed on a daily basis. The ceiling in the pool area needs to be repaired or replaced. If the pool is going to be closed there needs to be signs posted that it is indeed closed and the door needs to be kept locked. Paint/cover/seal all exposed wood. 21.23(a) Run Space Wardens observed dogs in primary enclosure that could not turn around with out hitting sides of kennel and the head of the dogs touched the top of the kennel. Primary enclosure shall be constructed and maintained to provide sufficient space to allow each dog to turn around freely and to stand erect, sit, and lie down in a comfortable normal position. 21.23(b) Run Dimensions- Wardens observed a dog in the primary enclosure that could not turn around without hitting sides of kennel and the head of the dog touched the top of the kennel. Each dog housed in a primary enclosure shall be provided with a minimum amount of floor space, which shall be calculated in accordance with the regulations. 21.24(a) Shelter- Wardens observed several pens housing three to four dogs without any shelter to protect them against inclement weather, preserves there body heat, and keeps them dry. All dogs shall be provided access to shelter which protects them against inclement weather, preserves their body heat, and keeps them dry; regardless of weather the shelter is temporary or permanent. 21.24(b) Bedding- Wardens observed soiled and what appeared to be old musty bedding for several of the dogs in the primary enclosure. Bedding to be kept clean. 21.28(a) Food Wardens observed feces mixed in with food. The food shall be free from contamination or disease, and shall be of sufficient quantity and nutritive value to maintain the health of the dogs.

21.28(c) Food/water receptacles Wardens observed that there was no food bowls for any of the dogs. There is to be food bowls for all dogs and kept clean. 21.28(d) Bedding Cleanliness Wardens observed dirty bedding in kennel. If bedding is used in primary enclosures, it shall be kept clean. 21.29(a) Excreta Wardens observed excreta in and under primary enclosure which had not been removed daily. Excreta shall be removed from the primary enclosure, including any floor area or ground surface beneath the primary enclosure, on a daily basis. 21.29(b) Sanitation wardens observed an accumulation of dog hair and other debris in the primary enclosure floor and along fencing, also standing urine. Primary enclosures where the dogs are housed indoors must be cleaned a minimum of once daily.specifically, owner shall keep clean area where dogs are kept. 21.29(c) housekeeping /Pests Wardens observed Feces and debris as well as cobwebs throughout the kennel. The building and grounds of the kennel shall be maintained, kept cleaned and in good repair and to prevent avian or mammalian pests 207h.2.v primary Enclosure-Shelter form elements Wardens observed that almost all primary enclosures had attached outdoor wire runs which are not adequate to enable the dogs to remain dry. Primary enclosures housing dogs must enable the dogs to remain dry and clean. 207.2.vi Primary Enclosure-Shelter Temp-Hazard Wardens observed that most of the primary enclosures do not provide shelter and protection from temperatures and weather conditions that may be uncomfortable or hazardous to any dog. Use primary enclosures that provide sufficient space to shelter all dogs housed in the primary enclosure at one time. 207. h.viii Water Potable constant Wardens observe no water in bowls in primary enclosures. Provide potable water at all times. unless otherwise directed by a veterinarian in writing that shall be kept in the kennel records. 207h.14 vi Sanitation Premises Wardens were advised by kennel owner that the primary enclosures hsd not been sanitized in several weeks. Used primary enclosures and food and water receptacles for dogs must be sanitized at least once every two

weeks using one of the methods under subparagraph (iv)and more often if necessary to prevent an accumulation of excreta,feces,hair,dirt,debris,food waste and other disease hazards. 207h.14vii Sanitation Insect and parasite control Wardens observe mice and rat dropping in the kennel area. An effective program for control of insects, external parasites affecting dos or pests must be established and maintained so as to promote the health and well - being of the dogs and reduce contamination by pests in housing facilities. 207i.1 Space Size of Primary Enclosure Not all primary enclosures met the minimum amount of floor space allowed by law. A dog housed in a primary enclosure must provided a minimum amount of floor space in the primary enclosure ; the floor space shall be calculated using the longest dog. 207h.10 Food Contamination - Wardens observed feces in food. All dogs must be provided with adequate food that is clean and free from contaminants. 207h.14.i Sanitation-Feces, waste, etc Wardens observed interior of primary enclosures with excreta,food mixed together on flooring. Food and excreta must be removed from primary enclosures at least daily or more often to prevent an acculation of excreta and food. 207i.6.v Exercise area-sanitation Wardens observed exercise with debris in them. The exercise area shall be cleaned in accordance with the requirements under subsection (h) (14). Recheck to be done at a later date
