Trinity Shih Tzu Purchase Contract Spay/Neuter agreement + 1 year Health Guarantee The buyer agrees to have the puppy examined by a licensed veterinarian within 48 business hours. A copy of the exam must be mailed to: Trinity Shih Tzu c/o Kellie Crnkovich 300 E 11 th Ellis, KS 67637 post marked within 72 hours of the purchase. This is mandatory to start your 1 year guarantee against congenital defects. If your puppy develops a condition that dates back to the fetal stage and this condition would severely impact the quality of life (Buyer must contact Breeder before euthanasia is performed); upon conclusive proof from a licensed veterinarian, such proof to be presented to breeder within one year of sale only, that at the time of sale the puppy had a congenital illness or disease the breeder will forward to buyer at Buyer's cost another pet puppy within a reasonable time (24 months). These conditions are listed and limited to heart, kidney, and liver from the date of the puppy's birth. No other conditions are covered in this contract, except limited Parvo/Distemper guarantee detailed below. This guarantee is NOT transferable; applies to original Buyer only. If puppy expires an autopsy must be performed (at Buyer's expense) and buyer must provide to Trinity Shih Tzu all written findings stating the defect and cause of death from a certified Vet within 72 hours of autopsy. Trinity Shih Tzu will send it to our vet for review and satisfaction of the findings. Buyer's Vet to be available for phone consult with Breeder and/or Trinity's Vet. No puppy will be replaced that fails to have the proper veterinary care. No money is refunded ever for any reason, only replacement of puppy. Trinity Shih Tzu will not be responsible for ailments resulting from or intensified due to obesity, second hand smoke, malnutrition, dehydration, starvation or lack of proper vet care. Puppy replacement guarantee will absolutely be terminated if the puppy was abused, or used for breeding purposes. Under such circumstances, all warranties are VOID. Trinity Shih Tzu does not guarantee hypoglycemia (this is up to the new owner to
prevent). Trinity Shih Tzu is NOT responsible for and guarantee does not cover what in the Shih Tzu breed is considered normal Hernias (umbilical & inguinal); stenotic nares (pinched nostrils), and under bite as they are not life threatening problems and are common in small (toy) breeds. Hernias are easily & affordable to have closed when your puppy gets spayed or neutered. Pinched nostrils are usually outgrown. Any condition that is minor, correctable or breed related is not guaranteed. We guarantee parvo virus and distemper ONLY if we are contacted within 3 days because the puppy can get the disease after it departs our home. Diagnosis and lab results must be provided immediately to Trinity Shih Tzu. Buyer will be provided a replacement puppy if puppy does not survive proper medical treatment (autopsy required). Coccidia, Giardia, misc. parasites & any other intestinal or minor illnesses should not be life threatening with prompt & proper medical care and are not covered in this guarantee. Ear mites & yeast are not covered items. It is unlikely puppy will have ear mites or yeast in ears, and both are easy to treat if found. Breeder takes every precaution to prevent these issues. Coccidia can develop quickly due to puppy stress during shipment/travel (almost can guarantee Coccidia if shipped by plane due to stress; very manageable with Vet care and diligence in treating at home per Vet instruction), changing environments, etc. Buyer to weigh puppy every day to the ounce or gram for the first two weeks and consult Breeder & Vet if puppy looses weight. At first sign of diarrhea or vomiting, Buyer must seek immediate (meaning in Vet.s hands within 2 hours) Veterinary attention! Your puppy should NOT vomit ever. Seek emergency treatment! Every case of diarrhea must be diagnosed & treated as well. Do not wait!!! Buyer agrees to allow new puppy plenty of rest & not to expose puppy to public places until after all puppy shots, & Buyer's Vet check, including all fecal testing, is completed. Parvo & Distemper and other serious diseases are easy to catch. Avoid taking your pet to park settings, pet stores, etc. until Vet authorizes these activities & instructs Buyer in proper exposure to public places (preferably after 16 weeks of age!). Buyer further agrees to supervise puppy at all times to protect from falls, attacks by other animals (even through fences), and improper handling by children. Trinity Shih Tzu is not responsible for any vet or medical bills that are incurred by the said puppy from the date the said puppy leaves with its new owner. It is the sole
responsibility of the new puppy owner to treat the puppy for all and any medical conditions that arise from the date the said puppy is in the care of the new owner and thereafter. Trinity Shih Tzu does not guarantee size, color and temperament. Nor do we guarantee fitness for breeding. Weight is only an estimate. Prices are according more to quality than to size. Prices can vary according to the more rare colors and champion pedigrees. All puppies MUST remain on a regular vet schedule, including fecal testing, to be determined by the vet and up to date on shots & worming preventatives at all times. Proof of a regular high quality diet is required. Buyer to keep a log and receipts demonstrating Buyer's diligence in providing a well thought out diet. Buyer agrees that said dog will live inside the Buyer's home and will not be housed outside or in a garage type setting for any length of time. Buyer agrees said dog will be socialized and treated as one of the family. (Buyer is requested to send a family photo of puppy with family members within 60 days of leaving Breeder's home, and ongoing photos and updates are greatly appreciated!) Once said dog has left the home of Trinity Shih Tzu, Buyer is the owner of rights for the family photo of puppy and any ongoing photos. Spay/Neuter Agreement (AKC Limited Registration Only when available): If puppy is purchased on limited registration as a pet Buyer agrees to neuter or spay the puppy by no later than puppy's sixth month. Buyer to provide Trinity Shih Tzu with a copy of the certificate of Spay / Neuter from a certified vet before the puppy reaches 7 months of age. Registration papers-limited registration papers, if desired & offered as part of your purchase will be released AFTER proof of spay or neuter is received. Buyer must request papers in conjunction with delivery of paperwork above within the puppy's 7 month time-frame. Puppies sold on limited registration may not be bred or shown in conformation shows.
AKC Full Registration papers to be given immediately. Registrations available to be specified in listing for individual Puppy. The buyer agrees that the puppy will never be placed in a shelter, rescue, or sold to a broker, pet shop, or puppy mill. If the buyer is unable to keep the puppy, the buyer is to contact Trinity Shih Tzu for the right of first refusal, should Buyer choose to place said puppy. If it is determined by any means that the puppy/dog is not being suitably cared for or that it has been mentally or physically mistreated, Breeder has the legal right to immediately take possession of the dog and its properly signed transfer papers/registration, without compensation. No other warranties or guarantees, expressed or implied, are made under this contract. Note: Due to the possibility of unforeseen circumstances, Replacement guarantee will be considered null & void if Breeder is no longer breeding during the Buyer's 1 year period. ANY Disputes regarding this purchase will be litigated in Breeder's County & State of residence at Buyer's expense.
