Anesthesia Check-off Form 5231 SW 91st Drive Gainesville, FL 32608 (352) 377-6003 The doctors and staff at Haile Plantation Animal Clinic would like to offer the most advanced medical care and services available to your pet. Laboratory work before anesthesia/surgery can make us aware of unknown medical conditions that make your pet a surgical risk or increase the chance of an adverse reaction to anesthesia. The latest technology enables us to run safe and accurate blood chemistries minutes before anesthetic induction. These tests are similar to those your own physician would run. In addition, the results of these tests serve as reference values in case your pet should become ill in the future or can even detect serious problems before they become clinical. During an anesthetic emergency, your pet s blood pressure can drop quickly, which makes it difficult for the doctors and staff to administer life saving medications. This is why we recommend placing an IV catheter before every procedure. The IV catheter allows a direct line to the blood stream which flows to the heart and brain much faster. In human medicine, the IV catheter is often a requirement instead of an option. Please read the following information and initial on the line for which you accept or decline. Please keep in mind your pet s well being at this time, as far as age and the procedure being performed. Some of these items will be highly recommended by our doctors for the maximum health benefit and safety for your pet. Is your animal on any medications, if so what kind and when was it last given? Does your pet have a medical condition that may affect anesthesia? Please read the following information and initial on the line for which you accept or decline. Some of these items will be highly recommended by our doctors for the maximum health benefit and safety for your pet.

Pre-surgical Examination If your pet has not been examined by one of the doctors on staff in the past month, a pre-surgical exam will be performed. The exam helps the doctors determine the right protocol for your pet. Cost: $30.00 after a month from initial exam $55.00 after three months from initial exam Exam is complementary if procedure is performed within one month of initial exam. Blood Profile #1 (under 2 years of age) CBC (complete blood count) assesses anemia, infection and clotting parameters. BUN (blood urea nitrogen), Creatinine helps to assess renal function and dehydration; ALT (alanine transaminase), Albumin, and Alkaline phosphatase is used to assess hepatic/liver disease, BG (blood glucose) tests for hypoglycemia and hyperglycemia, which may indicate diabetes and many other serious illnesses. This profile also tests for levels of sodium; chloride and potassium (Electrolytes) which help the heart function normally and prevents loss of fluids from the body s tissue. Accept: Decline: Cost: $86.25 Blood Profile #2 (over 2 years of age) Blood profile #2 is a more extensive organ profile which consists of a CBC (complete blood count) which assess anemia, infection, and clotting parameters. Electrolytes maintain normal heart action and balances loss of fluids from the tissues. BUN (blood urea nitrogen), Creatinine, and Phosphorus are all indicators of kidney disease. The liver enzymes consist of Albumin, Alkaline phosphatase, ALT and Bilirubin. Some other blood values such as Total Protein, Amylase, Calcium, Globulin and Cholesterol also help add in diagnosing potential disease that may postpone surgery. A urinalysis is also performed to help indicate renal issues, diabetes, or a urinary tract infection. Accept: Decline: Cost: $149.00 Intravenous Catheter An IV catheter allows the doctors and staff to induce a sedative successfully. The IV catheter aids in administering intravenous fluids and is effective in an emergency situation if medications are needed quickly. The IV catheter is placed in one of the patients forearms and is a sterile procedure. It is necessary to shave a small area to keep the site as sterile as possible.

Intravenous Fluids Intravenous fluids maintain the patient s blood pressure. They help the patient stay hydrated throughout the procedure and flush the anesthesia out of the body for a faster recovery. Accept: Decline: Cost: $29.00 Laser Surgery Haile Plantation Animal Clinic has incorporated a laser machine into the practice to help minimize bleeding and reduce the intensity of pain. Since the laser is not indicated for all surgeries the doctor will advise you whether it will be appropriate for your pet s procedure. Accept: Decline: Cost: $60.00 Pre-sedation A pre-sedative injection may be necessary if your pet is too agitated to allow placement of an IV catheter, administration of anesthesia or to draw blood for lab tests. This is at the doctor s discretion. Pain Injection This injection is given pre-operatively so by the time your pet is recovering he/she will have a more comfortable recovery. The injection will help with pain control. Slow Release Pain Injection This injection is a slow release pain medication that can be given after a procedure to provide pain control for 3 days. This injection is appropriate for cats only. Accept: Decline: Pain Medication to go Home For some medical procedures the doctors may prescribe pain medications to go home. This helps your pet have a faster recovery and keeps them comfortable throughout the healing process. Medications to go home very anywhere from $25.00-$50.00 Accept: Decline:

Antibiotics Every time an incision is made or the body cavity is opened, the potential for infection is present. Depending on the surgery or physical condition and to eliminate the risk of complications of infection, the veterinarian may recommend antibiotics. Accept: Decline: Cost: $15.00 - $40.00 Heartworm Test Heartworms in dogs and cats can cause sudden respiratory complications and cardiac arrest (especially under the stress of anesthesia). If your pet is positive for heartworms, there is a possibility that the doctor may postpone your pet s procedure. Accept: Decline: Cost: $49.00 Leukemia/AIDS Test FIV and FeLV are the leading causes of death in cats. These diseases interfere with the immune system s ability to ward off infections and may cause delayed or non-healing of surgical incisions. Accept: Decline: Cost: $49.00 Microchip A microchip is a permanent registered ID for your pet. The microchip is inserted under the subcutaneous layer of skin between the shoulder blades. Should your pet ever get lost, other animal shelters and animal hospitals can identify then instantly and your pet can be returned to you safely. Accept: Decline: Cost: $39.00 Therapeutic Laser We are now offering complementary laser therapy for all of our surgical patients. Laser therapy has been scientifically proven to decrease post surgical pain, reduces swelling and speed healing.
