MANAGEMENT STRATEGIES FOR ORTHOPEDIC IMPLANT-ASSOCIATED INFECTIONS 5-6 March 2018 Berlin Germany Endorsed by:

Dear colleagues, It is our great pleasure to invite you to the first workshops on orthopedic implant-associated infections, jointly organized with the Aesculap Academy and PRO-IMPLANT Foundation, which will be held in Berlin, Germany. During this advanced workshop, evidence-based and up-to-date knowledge of diagnosis, treatment and prevention of infections will be critically reviewed and discussed. Infections associated with prosthetic joints, fracture-fixation devices (plates, nails, wires, pins), spinal hardware will be addressed. In addition, management of septic arthritis, vertebral osteomyelitis and complicated soft tissue infections will be discussed. PREFACE Particular emphasis will be dedicated to the interdisciplinary approach, open discussions and practical advice on how to deal with challenging clinical situations. In addition, new diagnostic tests, emerging surgical techniques, local antibiotic treatment and future implant types will be reviewed. After completion of the workshop, all relevant aspects of orthopedic infections with and without implants will be addressed. We are very pleased to welcome you to this unique scientific opportunity and hope to offer you plenty of scientific, dynamic and fruitful discussions. With best regards, Andrej Trampuz, MD Berlin, Germany

PROGRAM MONDAY, 5 MARCH 2018 8.00 am 6.30 pm Registration Introduction of the course and participants Epidemiology Epidemiology of implant-associated infections Types of registries Background on biofilm Principles of biofilm formation Microbiology Role of host and implant Pathogenesis, classification and definition of infections Early and delayed (low-grade) postoperative infection Hematogenous infection: search for primary focus Definition criteria for implant-associated infections Diagnosis Preoperative (blood tests, joint aspiration) Intraoperative (periprosthetic tissue culture and histopathology, sonication) Imaging options New and investigational diagnostic tests (biomarkers, molecular tests, D-lactate, microcalorimetry) Principals of treatment algorithm Rationale for choice of different treatment approaches Surgical treatment Retention vs. removal/exchange of the implant 1- vs. 2-stage exchange of the prosthesis Timing of reimplantation in 2-stage exchange Use of fixation devices and temporary spacers Cemented and uncemented approach Antimicrobial treatment Systemic therapy (choice of antibiotic with biofilm activity, drug-drug interactions, toxicity) Local antimicrobial therapy (bone cement spacers, drug delivery systems, spacers, bone substitutes and bone grafts) Long-term antimicrobial suppression (indications, cycling, pitfalls)

PROGRAM Therapy in patients with antibiotic allergy, renal insufficiency, immunosuppression Special situations Infections after fracture fixation Septic arthritis (with and without cruciate ligaments) Vertebral osteomyelitis (with and without spinal hardware) Soft tissue coverage (vacuum-assisted coverage, flap) TUESDAY, 6 MARCH 2018 8.00 am 2.45 pm Prevention of infection Skin/nose decolonization Pre-, intra- and postoperative prophylaxis Search for potential primary foci Case discussions When to revise a surgical site hematoma and wound draining Culture-negative infections: To treat or not to treat Difficult surgical cases (multiple previous revisions, compromised soft tissue, insufficient bone stock, persistent chronic infections) Most common errors in: Diagnosis Surgery Antimicrobial treatment Wrap-up & Summary Summary of the course Unresolved issues and potential solutions Closing remarks & evaluation of the course CASE DISCUSSIONS Participants are encouraged to actively participate in case discussions by presenting interesting cases (maximum 5 slides, duration of 5 minutes).
