The following information will help you and your family prepare for camp. Please keep this letter. This is the only letter you will receive prior to camp. Registration Deadlines: April 17, 2015 to State 4-H Office. Please submit your forms and payment check to your county extension office, one week before the by April 17 so they can electronically register you by the deadline. There is also a late payment penalty attached to all state level events. (See attachment for information). Late payment will begin April 24, 2015. Reminder: You must bring the Kentucky 4-H Dog Health form with you or you will not be allowed to attend camp. Other forms to bring or turn in to the extension office: Found on the 4-H website, dog program (right side) 1. 4-H Participant Information Form (both adults and youth staying at camp) please include copy of family medical insurance card a. Participation Form: Includes Health History, Medical Treatment and Publicity Release b. Code of Conduct (signed) 2. Pickup/Release Form (completed) 3. Dog Camp Activity Release Form Each county must have one adult attending camp with their club members (ratio is one adult to 5 children). Please contact your county extension office if this is an issue. There is no limit on the number of parents allowed to attend camp and stay on site at dog camp this year. However, it is important that all parents planning to attend understand: 1. All adults must have a background check completed through their County Extension Office as part of 4-H s Client Protection Policy (updated at least 6 weeks before attending camp) 2. All parents are adult leaders and/or chaperon. 3. All adult leaders are required to watch the children in their care, meaning those children in their cabin. 4. All adult leaders are required to help out, either in an area of their choice or where assistance is needed. 5. Please bring your own and any 4-H ers medical forms to registration. See your county 4-H agent for 4-H Participant and Code of Conduct forms. 6. There may be an option for family cabins on a limited basis, please contact State for request.

NO CHILD IS TO LEAVE THE J.M. FELTNER 4-H CAMP GROUNDS AT ANY TIME UNLESS ACCOMPANIED BY THE ADULT RESPONSIBLE FOR THE CHILD. Campers will not be permitted to leave camp at any time without prior notification and approval from State 4-H Staff or Kristin Leshney, Dog Program Volunteer Coordinator, and must be accompanied at all time by an adult leader. Off-Site Hotel: Please see off site lodging information. 1. Map/Transportation. Your parents/guardians or another adult are responsible for transporting you and your dog to and from camp. If you have transportation problems, contact your County Extension Agent for 4-H Youth Development to see if he/she is aware of other 4-H ers with whom you might car pool. Attached are maps to J. M. Feltner 4-H Camp and a map of the campgrounds. Driving time from Louisville is approximately three (3) hours and from Lexington, about 1½ hours. Online map: http://www.ca.uky.edu/agcollege/4h/oldsite/camp/feltner.htm 2. Arrival: Time Check-in is between 5:00 and 7:00 pm (Eastern Daylight Savings Time) on Friday May 16 in the Dining Hall. (See the map of the camp for the dining hall s location.) Snack will be from 7:00 8:00 p.m. (light snack foods) and meeting will follow. Before you unload your luggage and equipment, you MUST come to the dining hall to check in and receive your cabin assignment, and have your dog flea/tick checked and obedience tested. The Parent/Leader needs to remain at State 4-H Dog Camp until check-in of the 4-H ers and their dogs is complete. Flea Check: During check-in, members of the State 4-H Dog Advisory Committee will be checking for illness, parasites, ticks, and fleas. If any dogs are found with fleas/ticks, chaperons will be advised to get their animal(s) treated at their own expense before animals are allowed to stay at camp. Animals untreated will be sent home or must stay off camp at their own expense until the animal is properly treated. There will not be a reimbursement of money. 3. No dogs are allowed in the dining hall: DO NOT take dogs into the dining hall. This is a health code violation that must be obeyed. 4. Emergency: In case of an emergency, the telephone number at camp is (606) 864-2770. All parents MUST leave a phone number so we can reach you in case of emergencies. In most cases, this is on the 4-H Participant Form. If it is not, we will be asking for this information when you arrive at camp. If we do not have a number for Parent/Guardian where they can be reached in emergency situations, your child will not be able to stay at camp. All youth and adults must bring a completed 4-H Participant and Code of Conduct form to registration. Please see your county extension agent for forms. 5. Departure time: Camp will end Sunday, May 17 by 11:30 am (ET). Parents should arrive at 11:00 am to help with packing. No one may leave the camp until all the cabins have been cleaned, passed inspection by the 4-H Camp Manager, and all poop has been scooped! PARENTS, it is very important that you be at camp to pick up your child ON TIME. Everyone should be on the road by 12:15 pm. 6. Agenda includes crafts, dog agility, jumping chutes, obedience instruction, showmanship, skillathon, etc. Sign-up sheets will be available on Friday evening for special classes on Saturday afternoon, including Canine Good Citizen Testing (CGC), Showmanship, Obedience, Freestyle Obedience, and Advanced and Intermediate Agility.

7. What to bring: Everyone We are asking everyone to bring any and all available agility equipment that you can. This would include tunnels, teeters, etc. Also, each club should bring prizes for the number of youth in your club attending for the pooper scooper contest. Example, if you have five youth attending camp you should bring five prizes, etc. For You Bedding: Twin sheets, pillow, and blankets or sleeping bag (the cabins are not heated) Towel, washcloth, and soap Toothbrush, toothpaste, comb and other personal care items Clothing for warm and COLD weather as weather can vary at this time of the year. Bring at least one change of clothing per day. Two pairs of comfortable shoes. Flip-flops may be worn in the shower (only), Jacket, sweater, or coat. NO SANDALS or FLIP-FLOPS may be worn during camp (except in showers). Rain gear, even if rain is not predicted where you live (boots for muddy weather). Flashlight Alarm clock Snacks for yourself Any Medicine with instructions for use (includes over the counter headache medicine). Copy of your parents medical card that you are on, with medical forms, emergency phone numbers of your parents/guardian Extension cord/power cord for electrical hair dryers, I-Pod or phone charging, etc. Your dog s vet papers Your own medical forms For Your Dog Pooper scooper or disposable plastic grocery or sandwich bags Dog crate-make sure your dog is familiar with the crate BEFORE coming to camp. o No soft crates please! Dog food (enough for the whole weekend) and two bowls 4-6 foot lead, flat collar, and training collar (choke chain, pinch collar, etc.) o No chain leads please! Showmanship Lead Dog treats for training Healthy dog treats 1-2 gallons of water Vet/animal papers showing updated information on your animal(s) Optional Dog first aid kit Tracking equipment Camera Bug spray Board or card games Dog dumbbell Sheet or blanket to cover the dog s crate (may help to quiet the dog) Heater and/or fans Dog videos Photographs of your dog for scrapbooking and dog poster

8. Campers will not be permitted to leave the camp at any time without prior notification and approval from Kristin Leshney. 9. To prevent accidents, campers are to walk at all times, except during organized running games. Campers are to stay away from maintenance equipment parked or being used on camp property. 10. Campers who have personal conflicts with other campers are encouraged to discuss these with their local volunteer. If it cannot be resolved, please discuss with the Cabin Leader(s) or complete a Grievance Form at the registration table in the Dining Room. 11. Abusive behavior toward any dog is not permitted. 12. All dogs are to be on leash and when not on a leash, in a crate. To reduce dog stress (and your stress too), start NOW preparing your dog for the crate. Dogs should not be in the beds with you in the evening. In past years we have been threatened that we could not use our 4-H Camps for future State 4-H Dog Camp due to muddy dog prints on camp mattresses. 13. Aggressive dogs must be isolated from other dogs or wear a muzzle. Please obtain an orange flag from the dining hall for your dog to wear during camp. 14. Electrical appliances such as hair dryers may be used in the cabins (limited), but not in the restrooms. 15. The possession and/or use by campers or volunteer of tobacco products, alcohol, drugs, fireworks, and any weapon or item construed to be a weapon (such as a knife) is strictly prohibited. 16. All adults staying at State 4-H Dog Camp need to be fully engaged with youth and dogs. You must also have participated in the 4-H Client Protection process (see your county agent for details 6-8 weeks before camp). All adults will be asked to assist with camp through the Volunteer Signup Sheet (copy attached), please sign in upon arriving and sign the Volunteer Sheet. You and your family will have a more enriching experience if you do so. This year, if you are a family unit who does not have responsibility for other campers and the parent is a different gender from their child, we may have a limited number of family cabins available. In general, we need adults (especially males) to assist with other 4-H ers who do not have chaperons of the same gender. This makes budgeting and account management difficult. To help address this concern, the following payment policy will be implemented. Late Payment Policy It is assumed that payment for all events and activities will be received (IN HAND) by close of business no later than 5 business days after the end of registration. Example: Dog Camp 4-H Dog Camp 2015 Fee is $80 due April 17, 2014 County A (1 youth/adults registered) Fee received by April 17 = $80.00 After this date late payment penalty is incurred Registration Deadline April 17 Late payment begins April 24 Payment becomes late five (5) business days after registration Late Payment begins 5/25 - $78.75 Payment 1-30 days late ($80 + 5%) = $84.00 Payment 31-60 days late ($80 + 10%) = $88.00 Payment 61+ days late ($80 + 15%) = $92.00 Billing Participants should pay all program fees associated with an activity or program in advance. Billing the county after the event is not an option. This takes time and effort away from program development. Every effort should be made to pay, in full, all fees before arriving at the event or activity. Checks are payable to: KY 4-H Foundation. Returned Checks In situations where checks are returned for insufficient funds, the county will be assessed the penalty associated with the penalized transaction. It is the responsibility of the county to collect any fees from the individuals. Cancellations Check with the event coordinators for policies and procedures concerning cancellations of registrations.
