Converse Premium Labs LLC 2019 Health Guarantee/Purchase Agreement Terms and Conditions These Purchase Terms and Conditions are for the purchase of a Converse Premium Labs puppy ( Puppy ) The sire and dame have been checked by a veterinarian and found to be healthy and are current on all vaccinations. The dames and the sires of Converse Premium Labs have been DNA tested and are free of any terminal diseases or abnormalities. Both the sire and dame are American Kennel Club ("AKC") registered. Bryan and Gina Converse are the sellers of the Puppy and hereinafter will be referred to as "Breeder". Breeder Responsibilities / Disclosures The Breeder reserves the right to refuse to sell to any person or family that they feel may not provide a good and responsible home for the Puppy. The Puppy is not sold with any known health problems. The dewclaws are removed when the Puppy was 3-5 days old. The Puppy will be dewormed prior to going home with Buyer. (Consult your veterinarian for future worming needs) Puppy shots were given at 6 weeks old. Shot records will be provided with at the pickup/delivery of your Puppy. Consult with your veterinarian for your Puppy s future shot needs After the Puppy was weaned from the mother it was fed only a premium dry puppy food. The Breeder currently uses Taste of the Wild Prairie Grain-Free Roasted Venison & Bison Dry Puppy food and finds that this provides the necessary nutrition for proper growth, joint and bone development for these dogs. It can be purchased online at Chewy.com or at a local Tractor Supply or Rural King Store. All precautions have been taken to ensure the Puppy is clean, healthy and no outside contact with other dogs or people (except the Breeder s Family
and the veterinarian staff and employees of Baker House Animal Hospital, Morrow, Ohio) during the first critical several weeks after birth The Puppy was imprinted (handled by the Breeder) from birth and is very comfortable being picked up and handled The Breeder will provide the Buyer with the necessary paperwork to register the Puppy with the AKC The CPL litters are AKC "Limited Registration" is defined by the AKC as "the dog is not to be used for breeding and entry in dog events is restricted" per the Breeder s discretion. CPL provides Clear Registration at an additional fee which gives the buyer the full ability to breed their puppy at proper age if they so desire The Puppy comes with a health clearance from the veterinarian from Baker House Animal Hospital Morrow, Ohio. If the Puppy is being shipped, it will be shipped when the Puppy is 8 weeks old The dames and the sires of Converse Premium Labs have been DNA tested and are free of any terminal diseases or abnormalities. They have been thoroughly checked by Dr. Craig of Baker House Animal Hospital in Morrow, Ohio and there are no known physical issues. Payment Options: Cash Venmo Checks; with proper ID. (The Breeder does not accept postdated checks) When shipping a puppy payments are expected to be paid in full prior to the shipping of puppy
Payment Terms: The current price per puppy $900.00*. A minimum, non-refundable deposit of $350.00 is required to the reserve the Puppy. The balance is to be paid in full at the time of picking up the Puppy. Make all checks payable to Bryan or Gina Converse with Converse Premium Labs in the memo of the check. Checks will be permitted with appropriate identification. Payment options are available with approval from Converse Premium Labs. There will be no refunds on deposits; however, your deposit may be applied to another litter unless there has been a prior arrangement or commitment from the Breeders to the Buyer. (Ex: The Buyers preferences which has been approved by the Breeder is not available the buyer can then select another, non-committed puppy from the same litter, or put the deposit towards the next litter or per prior agreement with the Breeder the seller may be refunded the deposit (*$900.00 is for a Limited Registration puppy. This means that the purchaser will not be able to sell any puppies bread from their dog that will be recognized by the American Kennel Club. C le a r R e gistra tion is a va ila ble for a n a dditiona l $250.00which then allows the purchaser your bread puppies that will be recognized by the American Kennel Club.) Buyer s Responsibilities: Love your new family member and provide a good home. The Puppy will return the love many times over Socialize your Puppy with other people outside your family. In order to help maintain your Puppy s health, your Puppy requires
regular exercise (consult with your veterinarian) Feed your Puppy a premium large breed, meat or poultry based puppy food Make sure that your Puppy receives the entire set of shots, (consult with your veterinarian) Current Rabies vaccine when due (consult with your veterinarian) If your Puppy is one with sensitive skin and eyes, it requires special care. Consult your veterinarian to determine what precautions you should take to protect your Puppy s skin and eyes You should get obedience training for your Puppy starting at 4-6 months. Obedience training will help you when the Puppy matures and becomes too big to handle Pay all shipping costs. Normal shipping costs are dependent upon the location of where the puppy will be shipped but the cost will include an approved crate for safely transporting the puppy. Return / Guarantee Policy: If there is a health problem with the Puppy, you must provide the Breeder a written notification of the specific health issue and the health issue diagnosis must be confirmed, signed by a veterinarian and notarized. (You must also provide evidence of regular checkups, current vaccinations and receipts from using a quality nongrained based food). The refund of purchase price or replacement puppy is valid for up to 3 months of the date of balance payment of the puppy. Cancer Guarantee - Cancer can be a factor in all breeds. If the Buyer should have that misfortune within one year of the time of pick up that terminal cancer has been found in a CPL puppy, a replacement puppy or a predetermined monetary compensation will
be provided. A veterinarian confirmation of the cancer required. (This guarantee is optional by the breeder and must be predetermined and agreed upon by the Breeder and prior to the puppy leaving the care of the Breeder and taken custody by the Buyer). If you have changed your mind and don t want to provide a home for the Puppy, it will be the Buyer s responsibility to find the Puppy a new home. (The Breeder may be willing attempt to assist you in selling the Puppy but cannot and will not guarantee the Puppy will be sold or it will be sold at the asking price.) Deposits are non-refundable. In the event that there are not enough puppies in the litter to complete the number of orders. Buyer can apply their deposit to another litter. If there has been a preapproved agreement by the Breeder and the buyer prior to the deposit being accepted the deposit will be refunded. CPL s only acceptable food is a Large Breed Puppy or Adult food that is certified to be non-grain or filler based but rather meat, poultry, or fish based. Proof of purchased of acceptable food is required for any refund or replacement of the puppy. Replacement guarantee during shipment: If the Puppy passes away for some reason during shipping, another puppy will be given to the Buyer at no charge and shipped to the Buyer at no charge. An affidavit from the shipping company or carrier will be required to support the Buyer s claim.
