Our foundation is designed to support Tallahassee Animal Services both with the animals in their care and preventing more animals from entering the shelter (through spay/neuter and intake prevention programs). The numbers of surgeries referenced above were free, community spays and neuters outside of Animal Services. If you currently have a program for sterilization of cats and/or dogs, describe your current level of funding and productivity and why additional resources are needed? In referencing the FY14 budget, of the $134,462 expenses, $52,235 was spent on community free spay/neuter to prevent additional animals from coming into Animal Services. The estimate of cats in the target area is probably lower considering the poverty level. Please explain what you believe are the most substantial sources of dog and cat overpopulation in the target area: The poverty index in Tallahassee maps similarly to the intakes of Animal Services however in mapping all of the intakes for the shelter in 2014, the hot spots are south of the interstate (I-10) and west of Monroe street (which bi-sects the City). This area of Tallahassee is part of the City as well as unincorporated areas of the County. Over 80% of the animals coming from this area are unaltered and strays. What kinds of spay/neuter services are currently available in the target area and in what ways are these resources currently insufficient to meet community needs? Our Foundation provides free spay/neuter vouchers for families that Animal Control makes contact with and animals potentially being relinquished to the shelter. Be the Solution, Inc offers low cost spay/neuter vouchers to Tallahassee residents. Neither program currently has a target of this particular area as these animals are most at risk in entering Animal Services both as walkins from the public and through Animal Control. Florida Animal Friend is highly supportive of proposals that are focused on animal populations that are identified as substantial sources of dog or cat overpopulation rather than being diluted over too broad of a geographic area or diverse animal populations. What is the criteria used for determining the target area(s) and/or eligibility for this program? Describe whether the targeted area is rural, suburban, or urban. Yes Provide any additional information that will help the grant selection committee understand how this program will operate to achieve its goals. Objectives What do you hope to accomplish with these funds (objectives should be specific and quantifiable)? The target area for the grant is chosen to decrease the number of intakes at Animal Services. Approximately 2,400 animals coming into Animal Services as strays and surrenders originate from this target area so by increasing spay/neuter efforts we hope to decrease shelter intakes from that area by 25% (600 animals). Through shelter statistics from the area, less that 15% of the animals entering the shelter are altered. How does this program increase the number of sterilization surgeries above the existing baseline? The current Animal Birth Control program alters approximately 500 animals annually. Through the grant, ASF would be able to provide 300 additional spay/neuter surgeries. Methods What criteria will you use to determine eligibility for your program? Our criteria for eligibility will be geographic area based on the heat density map provided. Residents must be living in Tallahassee, south of the interstate and west of Monroe Street. How will you advertise the program? Explain how the advertising will reach the target audience. Advertising the program will include a lot of grassroots aspects including flyering of neighborhoods, libraries, community centers, churches and general online forums such as craigslist.com. There are a number of social service programs already in place serving many in this community and we are certain this will provide us the much needed effect. A Pets For Life door-to-door model would also be a possibility in a joint effort with Animal Services. How will you address barriers to full use of the program such as transportation, illiteracy, and cultural hurdles? As with any program, reaching out to the audience on their familiar turf in their community, communicating with sincerity and respect will open minds to the prospects of spay/neuter. Many people in this area are not within easy access to services like spay/neuter so working with volunteers and Animal Services to transport animals is also a possibility of the need arises. Does this project involve the transportation of animals by someone other than the client? If so, describe the vehicles, methods for confinement, personnel training, liability releases used to assure the safety of the animals and handlers. Animal Services as offered to help with transport in their vans if needed. Veterinary Services Objectives What arrangements have you made with veterinarians to perform the surgeries? Our current ABC (Animal Birth Control) Program already has 18 participating veterinarians performing spays/neuters for our organization at discounted rates. A sheet is attached of all the veterinarians that participate in the program. Please note there are

only 4 vet clinics in the target area (there are over 25 practices in Tallahassee), all of which are part of our current spay/neuter voucher program. We feel strongly that our clients are not saddled with this expense. Is this a voucher program? Yes If so, how will you assure compliance with the program? Under our current program, due to the variety of veterinarians involved, the cost of pain medication and rabies vaccination included, we have excellent compliance. We also track the vouchers distributed to those that result in surgeries so we can follow up with those that are not in compliance.

For your voucher program, how have you determined the capacity of the veterinarians listed above to handle the projected capacity? We currently have the capacity with our 18 participating veterinarians to complete many more surgeries than we currently have financial means so we do not anticipate an issue. Will you have the ability to report the number of vouchers issued and the percentage that result in S/N surgeries? Yes! Our current program charts the number of surgeries we offer annually and the number that are ultimately performed. We have a new program coordinator in place to follow up on those vouchers distributed yet not redeemed to ensure continued compliance. Community Collaboration To assure the success of your program, are there any local groups (such as rescue groups, animal control agencies, TNR groups, local businesses, local media, social service agencies,etc.) other than your organization and your cooperating veterinarians who are committed to assist? Yes No Please list them and detail their level of involvement with the proposed effort. Name Level of Involvement Tallahassee Animal Services Tallahassee Animal Services is committed to aid in the distribution of vouchers to the target area to decrease their intakes. Distribution by Animal Services will occur in the field and in the shelter. ASF and Tallahassee Animal Services work in close partnership in saving more lives in Tallahassee. This grant would be the work of both organizations working towards a common goal. Budget Total number of sterilization surgeries projected: Cats: 400 Dogs: 400 Total budget requested (Budget should not exceed $25,000): $25,000.00 Average cost/surgery projected: $32.00 Describe any expenses that are not included in the grant and how they will be paid for: ASF will subsidize any remaining surgery cost per animal, rabies vaccines and pain medication. Describe any other funding sources for this program, i.e. other grants, targeted fundraising efforts, budget allocation, etc. ASF fundraises each year with the goal of spaying and neutering 400 animals, the additional grant money would double our program efforts. What percent of the total cost of the program would this projected grant cover? 33% Timeline All projects must be completed within 12 months of receipt of funding. Any unexpended funds must be refunded to Florida Animal Friend within 30 days of the end of the project. Requests for time extensions must be made in writing at least 30 days prior to the end of the project. Funding for accepted projects will be available after August 1, 2009. Unexpended funds Any unexpended funds must be refunded to Florida Animal Friend within 30 days of the end of the project. Requests for extensions Requests for time extensions are discouraged and not often granted. If it is imperative to request an extension, such request must be made in writing at least 30 days prior to the end of the project. It is FAF s policy to seldom grant more than a 30-60 day extension. Failure to submit reports and requests within the required time period will impact your agency s future grant applications. Future Funding to Sustain Public Spay/Neuter *Explain how the organization plans to fund this program in the future. Having sustainable plans including other grants, local donations and other services generating revenue enhances the chances of receiving this grant. ASF is committed to increasing their fundraising and grant efforts to focus on free spay/neuter in the Tallahassee area. Their ABC program has been successful on many levels in their short history and the Board has discovered new fundraising tactics to focus on funding additional spay/neuter surgeries should this grant be funded. Promotion of Florida Animal Friend Spay/Neuter License Plate *Applicants selected for funding are expected to publicize their grant in support of their spay/neuter program and promote the sale of the Animal Friend license plate via press releases, newsletters, website links, social media, etc. Please describe your plan to promote the Florida Animal Friend Spay/Neuter License Plate. Grantees are required to submit documentation of promotional endeavors. ASF will promote the Florida Animal Friend Spay/Neuter License Plate through our website, Facebook page, newsletter and the vouchers themselves. Animal Services will also provide information in their lobby advertising the license plate.
