PAWS Animal Care, Adoption & Education Ctr Located at 368 South Street Mailing Address: P.O. Box 182, Calais, Maine 04619 207-454-7662 pawscalais@yahoo.com Dog/Puppy Adoption Application Dog/puppy name(s) Thank you for choosing PAWS in your search for a pet. We adopt out many cats and dogs and this application is designed to help determine if this adoption is in the animal's best interest and to assist you in finding a companion most compatible with your lifestyle. An unwise adoption can be an unpleasant experience for adoptive families and the animal. We hope that you will agree that the animal s long term welfare must be our foremost consideration. The dog/puppy adoption fee is $125.00 and includes the dog/puppy initial Distemper/parvo vaccination, flea preventative, deworming. (if over 4 months old), a rabies vaccination, spay/neuter surgery and a wellness check. Puppies require 2 additional distemper vaccinations 3-4 weeks apart; these are the adopter s responsibility. The animals adopted from PAWS come to us from a variety of backgrounds. Some have had previous routine medical care, while others have not. We typically do not have information about an animal's healthcare, aside from what is learned during our initial examination and/or ongoing medical care while the animal waits to be adopted. PAWS does not guarantee the health or temperament of any animal in our care and we encourage adopters to take advantage of the free health exam provided by our veterinarian within 10 days of the adoption. Any medical care deemed necessary after an animal's adoption is the adopter's responsiblity. After reading above, please initial Date: Name: Date: Address: Apt#
