Frequently Asked Questions Drop Off & Pick Up Camp runs from 1 to 5:00 p.m. Drop off and pick up will be located at the front entrance of Cat Depot s Education Center, located at 2525 17th Street. Official Drop Off Times at Camp Entrance: 12:45-12:55 p.m. Official Pick Up Times at Camp Entrance: 5-5:15 p.m. Before Care Drop Off Times at Camp Entrance*: 12:15-12:45 p.m. After Care Pick Up Times at Camp Entrance*: 5:00-5:45 p.m. (*Additional fees) To pick up or drop off your child at any other time, please ring bell at the front entrance of the Cat Depot s Education Center. If no answer, please head across the street to the Adoption Center for assistance and direction on where to find your child s camp group. There will be a late charge of $1.00 per minute if a child is not picked up at designated times. Daily Activities Time with Cat Depot cats, age appropriate games, crafts, and science experiments, lectures and hands-on workshops. Each activity will be based on the week s theme. How do I register? Registration forms are available on our website (www.catdepot.org) or by e-mail at education@catdepot.org. Payment is due in full at the time of registration. Registrations cannot be taken over the phone or on the first day of the camp week. Incomplete registrations will not be accepted and will not reserve a camper s spot. There will be a $15 fee for all changes submitted after initial registration. How will I know I am registered? You will receive an e-mail confirmation within 5-7 business days after submitting your completed registration forms. You will be contacted by phone only if your registration is incomplete or your requested camp is full. How do I know what camps are full? An updated list of closed camp weeks will be available on our website (www.catdepot.org). What to Wear & Pack Campers must wear closed-toed shoes (no sandals or croc -type shoes), clothes that can get dirty, and a camp t-shirt each day. Each camper will receive one free T-shirt for each registered camp week; any additional T-shirts may be purchased for an additional $12 fee. Please do NOT bring cell phones or electronic games, as these items are easily lost and can interfere with camp activities. If your child would like to take photos, we suggest packing a simple camera, as electronics that feature games and internet connection (such as cell phones and ipads) cannot be used at camp. Group Size and Ratio There will be 10-12 campers to each paid adult staff member. In addition to our paid staff, we will have many teen and adult volunteers to support our staff. We do background checks on all camp staff and volunteers. What about lunch, snacks and water? Campers may bring their own bag lunch in a cooler bag (we do not have refrigerated storage available). Cat Depot will provide a mind-afternoon snack. Campers are encouraged to bring a reusable water bottle; there is a water fountain available for refills. If campers have food allergies, please let us know in advance. Please be specific about what the camper is allergic to, the severity of the allergy, and whether it is triggered by touching or ingesting the food. If your child does have food allergies, we suggest that you pack your own snacks. Cat Depot cannot guarantee that your child will not be exposed to peanuts, peanut butter, or other nuts.

An 80% refund will be given if cancellation is received five (5) days or mo re before the first day of camp. A 50% refund will be given for requests made three (3) days or fewer prior to the first day of camp. NO REFUNDS ARE GIVEN FOR CANCELLA- TIONS ON OR AFTER THE FIRST DAY OF CAMP. All refunds will be based on total camp fees. T-shirts are not refundable. Please allow 4 weeks processing time for refunds. Cat Depot reserves the right to limit program enrollment and to cancel program completely if registration is below the required minimum. HAZARDOUS ACTIVITY FACILITY GENERAL RELEASE I, agree, assume and understand that participation in Cat Depot s Critter Camp and activities in all its forms may include hazardous activities. These hazardous activities can include, but are not limited to, contact with domestic and exotic animals, pod time and cat care activities in, behind-the-scenes tours, animal encounters provided by third-party organizations and/or individuals, recreational activities such as play time, and craft activities. Participation also includes the use of the Cat Depot facilities (Adoption Center and Education Center), equipment, supplies and/or materials which are provided by Cat Depot or are available at Cat Depot which may also involve risks of injury. I, on behalf of myself and my children, understand that it is my burden and obligation to insure that each child participates with care and control at all times. In using the facilities of Cat Depot and participating in these activities, on behalf of my children and myself, I understand and accept that hazards exist, assume the inherent risk that injury may result to myself or my children and agree to unconditionally and fully release Cat Depot, its respective employees, volunteers, board of directors, advisory board members, assigns and/or agents and representatives (collectively Cat Depot ) from any and all past or future claims, demands, actions, causes of action, suites, or other liabilities of whatever kind, including, but not limited to, those arising out of the use of Cat Depot, its respective employees, assigns and/or agents and representatives alleged negligence. I give Cat Depot permission to acquire emergency medical treatment if no parent or guardian or emergency contact can be reached. The terms of this agreement may not be modified by any employee, representative, agent and/or assigns of the aforementioned companies. Field Trip Permission Slip Registration Form - Part Three Critter campers will participate in a field trip each day of camp to the Cat Depot Adoption Center. Each field trip will be a WALKING TRIP where campers will need to cross 17th Street. Campers will be accompanied by a Cat Depot staff member and several camp volunteers. For your child to participate in these activities, we will need a parent or guardian to complete the following consent form. Before & After Care Agreement Cat Depot is pleased to offer before care and after care options during our Critter Camp program. We understand that working parents may have difficulties dropping off or picking up their children during regular camp hours. To ensure that the before/after care program runs smoothly, please follow the guidelines listed below: All participants must register in advance. Registration must be for the entire week. One-day registrations will not be permitted. Camp runs from 1:00 to 5:00 p.m. Drop off and pick up will be located at the front entrance of Cat Depot s Education Center, located at 2525 17th Street. If fees are not paid, the child will not be admitted into camp. Any person picking up your child will need to be listed on your authorized pick up list, and need to show photo ID every day. Children will not be released to people not on your list or who cannot show photo ID. I have read and fully understand the above guidelines for the Before/After Care Program. Registration Form - Part Four Critter Camp Expectations & Code of Conduct We are so excited to welcome your child to camp this summer! We look forward to providing all campers with a fun and educational experience. For each camper to have a positive and safe experience at Cat Depot's Critter Camp we establish the following expectations. The camp leaders will discuss these expectations with the campers, but we would appreciate your help in reinforcing them. We know this is going to be a wonderful experience for all involved, camp staff and campers alike. We only walk at Cat Depot. We may have food in the designated camp area only. No food/drink is permitted in the shelter. Gum is not permitted. The Golden Rule: 'Treat others as you want to be treated' - This means raising your hand to talk, respecting other's personal space, being considerate and helpful, etc. Stay with your camp group at all times. Most activities are conducted in groups. It s important that your child be able to work well in a group setting. THREE STRIKES AND YOU RE OUT POLICY: To encourage a safe learning environment, we employ the "Three Strikes & You're Out" policy. On the first infraction of the rules a warning will be issued. Camp leaders will stop and make sure the camper realizes he/she are duly warned. With the second infraction there will be time-out during cat socialization time. If there is a third offense, the camp coordinator will be called, and there will be a counseling session. This is the camper's first strike. On any subsequent warnings after the first process has ran its course, the camp coordinator will counsel the camper and the parent will be contacted. This is the camper's second strike. After the above policy is followed for the third time, the camp coordinator will send the camper home. This is the camper s third strike and the camper shall not be permitted to return to camp. Spay/Neuter Discussions: Cat Depot promotes the spaying and neutering of all cats to limit pet overpopulation. This will be discussed during camp. Dress Code No flip-flops (they present a safety hazard). Please wear closed-toe and heel shoes at all times. Campers should wear a Critter Camp t-shirt (provided by Cat Depot the first day of camp). Comfortable shorts or pants no shorter than the top of the knee. No skirts or dresses as we will be down on the floor for some activities Sleeveless shirts of any kind are not allowed. Campers will be given a name tag the first day of camp. Absence / Illness: If your child is unable to come to camp, please call 941-366-2404 to notify our staff. If your child should become ill during camp, we will notify you and or the emergency contact person listed on the medical form. If your child becomes seriously ill or injured and requires immediate medical care, Cat Depot staff will call 911. Sign-In / Out: You or your authorized designee must sign your child in and out at the beginning and end of each camp session. You must pickup your child within fifteen minutes of the end of each camp session. There is no after-care program at Cat Depot. If you know you will be late, you must make other arrangements for on-time pick-up. You acknowledge that you understand that your child will be released only to those persons listed on the emergency and medical information form, regardless of circumstances. Medications: A letter of consent indicating the type of medicine and the time at which your child will be taking the medication is requi red. Cat Depot cannot and will not be responsible for administering medications of any kind to children. For your camper's protection and safety, please ensure that you have your State issued Driver's License or Identification Card with you when picking up your child and that you sign your camper out with a camp staff member. Cat Depot staff will require proof of identity from any persons arriving to pick up your child.

Parent s Permission Form I understand that all possible precautions are taken to ensure that the programs and activities at the Cat Depot, Inc. camp ( Camp ) are conducted in a safe and responsible manner. However, I further understand that because of the nature of Camp activities, regardless of the high degree of supervision, there is a potential for injury during any Camp activity. I do recognize these risks and agree to allow my child(ren) to participate in all activities offered in the Camp program in which I am enrolling my child(ren). I hereby give permission for my child(ren) to be transported by Cat Depot, Inc. to Camp field trips. I HEREBY RELEASE, INDEMNIFY AND AGREE TO HOLD HARMLESS CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS FROM, AND HEREBY WAIVE ANY CLAIM FOR, ANY INJURY OR OTHER HARM THAT MAY OCCUR TO MY CHILD(REN) WHILE ATTENDING THE CAMP, OR ANY INJURY OR HARM THAT MAY OCCUR TO ME AS A RESULT OF THE INJURY OR HARM SUFFERED BY MY CHILD(REN). THIS RELEASE, INDEMNITY AND WAIVER SPECIFICALLY INCLUDES BUT IS NOT LIMITED TO (A) ANY CLAIM OF NEGLIGENCE OR NEGLIGENT SUPERVISION AGAINST CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES, AND/OR AGENTS; (B) ANY INJURY OR HARM THAT MAY OC- CUR WHILE MY CHILD(REN) IS OTHERWISE ON THE PROPERTY OF CAT DEPOT, INC. BEFORE, DURING, OR AFTER THE SCHEDULED CAMP HOURS FOR ANY REASON WHATSOEVER; AND (C) ANY INJURY OR HARM THAT MAY OCCUR WHILE MY CHILD(REN) IS ON A CAMP FIELD TRIP INCLUDING WITHOUT LIMI- TATION DURING SUCH TIMES AS MY CHILD(REN) IS BEING TRANSPORTED TO AND FROM A CAMP FIELD TRIP DESTINATION. I hereby acknowledge and represent that I have been given the opportunity to review the Camp application, the medical information form and this permission form. I further represent that I have been given the opportunity to ask any questions concerning the information contained in the Camp application, the medical information form and this permission form and I am satisfied with whatever response was given to any inquiry that I have made. I hereby release and hold harmless Cat Depot, Inc, its officers, directors, employees, and agents from any responsibility or liability for any lost, stolen, or damaged personal property which my child(ren) brings to camp. Cat Depot, Inc. is hereby granted permission to use any individual or group photographs taken at the Camp or on a Camp field trip showing my child(ren) for publicity or brochure purposes. Additionally, I grant permission for photos and/or videos of my child(ren) to be posted on the Cat Depot, Inc. website and Facebook page.
