Single copies of the latest editions of the "Rules Applying to Dog Shows" and Obedience Regulations and Regulations for Agility Trials may be obtained WITHOUT CHARGE from THE AMERICAN KENNEL CLUB, 5580 Centerview Drive, Raleigh, NC 27606-3390. AGREEMENT I (we) acknowledge that the Rules Applying to Dog Shows and the Regulations for Agility Trials have been made available to me (us), and that I am (we are) familiar with their contents. I (we) waive any and all claims and cause of action that I (we) might otherwise have against the AKC and any AKC approved judge judging at this trial, under AKC rules, regulations, and guidelines. I (we) agree that the club holding this agility trial has the right to refuse this entry for causes which the club shall deem to be sufficient. In consideration of the acceptance of this entry and of the holding of the Agility Trial and of the opportunity to have the dog judged and to win prize money, ribbons or trophies, I (we) agree to hold this club, its members, directors, governors, officers, agents, superintendents or trial secretary and the owner or lessor of the premises and any employees of the aforementioned parties harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or upon the agility trial premises or grounds or near any entrance thereto, and I (we) further agree to hold the aforementioned parties harmless for any claim for loss of this dog by disappearance, theft, death, or otherwise, and for any claim for damage or injury to the dog, whether such loss, disappearance, theft, damage or injury be caused or alleged to be caused by the negligence of the club or any of the parties aforementioned, or by the negligence of any other person, or any other cause or causes. I (we) hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person, including myself (ourselves) or on account of damage to property, arising out of or in consequence of my (our) participation in this agility trial, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may be alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons. INSTRUCTIONS (Actual Owners) A dog must be entered in the name of the person who actually owned it at the time entries for a show closed. If a registered dog has been acquired by a new owner, it must be entered in the name of its new owner in any show for which entries closed after the date of acquirement, regardless of whether the new owner has received the registration certificate indicating the dog is recorded in his name. State on entry form whether transfer application has been mailed to A.K.C. (For complete rules, refer to Chapter 11, Section 3). JUNIOR HANDLERS By signing the entry form we certify that the Junior Handler does not now, and will not at any time, act as an agent/handler for pay while continuing to compete in the Junior Showmanship program. AKC JR HANDLER # NAME OF JUNIOR HANDLER ADDRESS JR S DATE OF BIRTH If Junior Handler is not the owner of the dog identified on the face of this form, what is the relationship of the Junior Handler to the owner? Jump Height Cards Please enclose a photocopy of your Official AKC Jump Height Card, if you have one, with your entry form. For dogs two years and older, Owners/Handlers must send a photocopy of a permanent height card or be measured prior to competing. For dogs under two years of age, the Owner/Handler must send a photocopy of a temporary height card or be measured prior to competing. Exhibitors should carry the original valid official Jump Height Card to each trial to present upon request. All dogs that have not been officially measured for a Jump Height Card may still compete; however, they must be measured by the Judge of record, prior to running at each trial, unless jumping at 24" in the Regular Classes or 20" in the Preferred Classes. The Judge s measurement will be valid for this trial only. For the Judge s planning purposes, the Trial Secretary should provide the Judge with a list of dogs that will require measuring. If a measurement is necessary, it is the exhibitor s responsibility to have their dog(s) measured prior to running. If an AKC Field Representative or a Volunteer Measuring Official (VMO) is present, all dogs not yet officially measured must be measured for a Jump Height Card. If a dog is measured by an AKC Field Representative or Volunteer Measuring Official (VMO) and is now eligible to jump in a lower height class, the measurement is only effective for any trials that have not yet reached their closing date. Each dog is allowed one re-measurement per lifetime if the dog is past its second birthday. Prior to the second birthday, a dog may have one additional measurement after the initial official measuring. In both cases, the measurement official is to be told the remeasurement is a "challenge" measurement. Misrepresentation of a dog, including altering information on a Jump Height Card, is grounds for misconduct. Can You Help? It takes a lot of people to put on an agility trial. If you can help for even one class, your fellow exhibitors will appreciate it. Pick a job that suits you and mail this form back with your entry.
