Subject: Animal Bite Initial Report 05/25/2015 2236 SCHOCK, MICHAEL ALLEN, DAWN 964, 2022, 2123, 2074, 2021 Animal: black and brown mixed breed male dog Report received of a dog bite at the listed location with the Sandusky Fire Department already en route. On arrival Elsie Warren was seated in the boulevard with her pant leg pulled up. She advised she was walking on the east side of Tiffin Avenue when the dog that lives at 1921 Tiffin Avenue attacked her. I observed her to have blood and a laceration on her right knee, and an ambulance was already on the way. I was getting a description of the dog that attacked Elsie when we observed the listed dog across the street running loose, and she was obviously fearful of the dog. I then confirmed this was the dog in question, and due to it having already attacked Elsie I decided to stop the interview to secure the dog. Elsie then pointed out a male, later found to be Kevin Roth, indicating he was the owner, but he was observed to turn and walk away, so I was unsure if he was the owner or not. It should be noted that there is a playground at this intersection with numerous adults and children in the area so it was important that the dog be secured to prevent anyone else from being attacked. I retrieved my snare from the trunk and stepped into the roadway when Kevin approached calling out to the dog, which immediately responded to him in a positive manner then ran up to him. Kevin was obviously unaware that his dog was running loose. Once the dog reached Kevin its demeanor changed drastically and it charged at me. I was drawing my sidearm, while using the snare to keep the dog at bay. Kevin was trying to gain control of his dog, which prevented me from discharging my sidearm for fear of striking Kevin. He was able to get control of his dog and was walking it to the house to secure it. Due to the dog being so aggressive I dropped my snare and kept my sidearm out while he secured it. I feared the dog would get loose again, so I followed Kevin to a spot just off the west side of the street so I could stop the dog from approaching Elsie and the ambulance personnel, along with the numerous people in the area of the playground if it got loose again. Kevin was on the front porch and appeared to be having a problem, either getting the door open or maintaining control of the dog. The dog suddenly broke free of Kevin's grasp and charged at me with its head down in a very aggressive manner, and it was obvious that the dog was attacking. The dog closed the distance extremely fast, with me firing my sidearm at him. He bit my right calf with me firing again and backing away. I then fell to the ground with the dog about four feet away from me and it was struggling to get to its feet, at which time Kevin approached and I ordered him away. I had called for assistance, due to my right arm being injured and being bitten by the dog, with additional units arriving a short time later. The ambulance crew approached to check our welfare and summoned another ambulance.

Schock's and Elsie injuries and Ofc. Schock was then transported to Firelands Regional Medical Center for treatment. Three spent shell casings from Ofc. Schock's duty weapon were located in the weeded area between the old Juanco Body Shop driveway and Kevin's personal driveway. The casings (#1, #4 and #5), along with blood from the dog (#2 and #3), were tagged with the yellow evidence markers and then photographed. Subject: Supplement Supplement 05/26/2015 0518 RITTERBACH, STEPHEN CAMPBELL, MICHAEL H 05/26/15 Supplement 2074 I spoke with a resident, Dorothy Gentry, who lives across the street from Roth's residence. She advised she saw the dog loose earlier and she was keeping an eye on it because her children were due home at any time. Dorothy stated she observed a female walking southbound on Tiffin Ave. and when she got out in front of the house she watched the dog approach and bite the female. She stated the female stayed in the area and an officer arrived a short time later. Dorothy advised she was watching as Ofc. Schock was speaking with the female. She said he started to cross the street from the area of the park and he had his catch pole in his hand. She said as he got across the street the owner of the dog approached telling him it was his dog and he would take care of it. Dorothy advised Roth got a hold of the dog and started walking it toward the front porch. She stated when they got to the porch the dog got loose from Roth's grasp and the dog charged Ofc. Schock. She said the dog was attacking the officer and he shot it. Dorothy was advised she would be added to this report. Ofc. S. Ritterbach Page: 7 of 10

Subject: SUPPLEMENT Supplement 05/26/2015 1012 SUSANA, TRACEY E CAMPBELL, MICHAEL H 954 Nina of the Erie County Health Department called to update the status of the animal involved. She indicated the dog has been euthanized by the Lorain Animal Emergency vet. The dog's head has been preserved and sent out for testing. Nina advised results should be received in two days. SGT. T. Subject: Supplement Supplement 05/26/2015 1617 SCHOCK, MICHAEL ALLEN, DAWN Supplement 964 It should be noted while defending myself from the dog I fell to the ground. When I fell to the ground my firearm jammed and had to be cleared, ejecting a live round into the grass. Officers were unable to locate the round prior to me being transported to the ER for treatment. MSCHOCK Page: 9 of 10

Subject: Supplement Supplement 05/26/2015 1837 GILLIAM, MARK ALLEN, DAWN 1895 Supplement 05/26/2015 Ofc. Ritterbach had brought Ofc. Schock's duty weapon, clip and his ankle holster with his back up gun on station after this incident. He requested I secure the weapons and clip, so I placed them in my locker and locked same. I did check the clip and it did have 11 live rounds still in the clip. A short time later Sgt. Allen asked about the weapons. I advised her, when Ofc. Schock came back on station, I had given the items back to him and he was putting them in his locker now. She requested his duty weapon and clip be placed in a gun locker in the evidence room, per the Assistant Chief. I did retrieve them from Ofc. Schock and placed a zip tie down the clip opening on the gun and the receiver so it did not close. Both the clip and the duty weapon were placed in the gun locker in the evidence receiving room. Ofc.
