Cockers Across PA Cocker Spaniel Rescue, Inc. ADOPTION APPLICATION Cockers Across PA Cocker Spaniel Rescue, Inc. (CAP) evaluates all applicants on a case by case basis and does not guarantee that all prospective adopters will be approved. We reserve the right to choose the adoptive families we feel are most suited to our dogs' needs at the time of application. In addition, CAP reserves the right to redirect applicants to a dog that better fits their lifestyle in order to have a successful adoption. Disapproval simply means that CAP does not feel it has the right cocker for the applicant at the present time; it is not a reflection on the applicant s character. If concerns present themselves at any time during the adoption process, CAP also reserves the right to reevaluate your application. Why do you feel a rescue dog is right for you? The following questions are asked so that prospective adopters fully realize what the cost and maintenance would be to have a cocker spaniel in their home. Many adopters are surprised at the cost of food, medical expenses, and grooming that are necessary to keep a cocker spaniel in good health. Please answer these questions to the best of your ability. 6. Expenses - Please estimate the amount you think it will cost yearly for the following: Food (Premium food) (Food allergies are common in cockers, premium food can help) Grooming (Every 6-8 weeks, but can go as long as 3 months max, for some dogs) Veterinary care (Annual physical, shots, heartworm /flea preventive and maintenance of ears, teeth etc.) 7. Misc. Questions Would you consider a special needs dog (ex. requires daily medication, is blind or is deaf)? Yes Maybe No Would you consider adopting more than one dog? Yes Maybe No Would you consider fostering for CAP? Would you like to volunteer with CAP? Yes Maybe No Yes No (If yes, we will send you an application to help identify your interests) May we sign you up to receive our monthly electronic newsletter? Yes No If yes, preferred email address: (CAP will never sell or give away your personal information) How did you hear about us? If on the Internet, which site? Previous adopter/volunteer? _ (please list so we may thank them) IMPORTANT INFORMATION: These cocker spaniels are in an animal rescue group because, for one reason or another, their previous owners could no longer care for them. The main purpose of this organization is to find responsible and committed homes for all cockers placed in our care. Adopting a dog is a very important decision and one that affects not only you and your family, but the dog you decide to adopt, who is a thinking, feeling creature. Millions of dogs are euthanized every year because people do not take seriously the maintenance and care of animals. The care and maintenance of a cocker spaniel can be especially costly. Time and attention are also major factors that need to be considered. All cockers in our care have been taken to the vet for an examination, received basic shots, and have been spayed/neutered (unless medically unable). It is the responsibility of the new owner to provide routine vetting, shots and preventative care (such as teeth cleaning, heartworm and flea preventative etc) throughout the lifetime of the dog. Cockers Across PA Cocker Spaniel Rescue, Inc. Adoption Application Rev. 07/31/14 Page 4 of 5

The adoption fee is used to provide medical care, food, shelter, and publicity for the cockers. The fee must be paid upon signing the contract. If, for any reason, you can no longer provide care to the dog you adopt, it must be returned to Cockers Across PA Cocker Spaniel Rescue, Inc. The adoption fee is non-refundable. The average cocker spaniel has a life span of 12 to 15 years. You must be willing to make a commitment to this dog for the rest of its life. Are you willing to provide the necessary love and attention as well as the necessary food, vet care, and grooming required to maintain a happy and healthy cocker spaniel for the rest of his/her life? Yes. No. It is extremely important that once a dog is placed in a new home that it remains there and that all parties involved are satisfied. CAP is looking for COMMITTED FAMILIES ONLY. If you have any questions or concerns about adopting a cocker it is imperative that you finalize them before adopting. Please sign below to confirm that you have read the above information and that you understand it, and that the information you have supplied on this application is accurate. If you have any questions regarding the care and maintenance of a cocker spaniel please let the organization know. We will be more than happy to help with any questions you may have.
