Any exhibitor with a bitch entered who will be in season on the trial date will be refunded the entry fee less a $5.00/day for each entry for that bitch. Injured dogs: Any exhibitor with a dog that is injured prior to the trial date will be refunded half of the amount of each entry for that particular dog, upon written certification by a veterinarian. Notification must be in the hands of the Trial Secretary not later than 9:00 a.m. on the day of the trial.. Make check or money orders payable to Central Indiana Shetland Sheepdog Club (CISSC). Canadian exhibitors must make fees payable in U.S. funds. CISSC assumes no responsibility for cash sent through the mail. Post dated or returned checks do not constitute a valid entry fee. The secretary will add a collection fee of $25.00 to the amount of each returned check. Failure to make good on returned checks will result in rejection of future entries for any dog owned by the that handler Please include instructions as to what you wish to do with your entry if you cannot get in all the classes and days you entered. Entries will be acknowledged by e-mail as soon as possible after the entry is received, or if you don t have e-mail you may include a self addressed stamped postcard. After closing date entries will be acknowledged by e-mail and will include the judging program. They will be sent by USPS if you do not have e-mail. Should such acknowledgment not be received by the exhibitor within a reasonable time, please notify the trial secretary. Declination of entries: CISSC may decline any entries and may remove any dog on account of disease, viciousness, or other cause and no one shall have any claim against CISSC or any official thereof. No entry shall be made or entry accepted which specifies any conditions as to its acceptance. Error on the entry forms: Owners are responsible for errors and legibility in making out entry forms, whoever may make such errors, and no entry fee will be refunded in the event of such errors or cancellation of entries after the published closing date. Please type or print carefully the information on entry forms to eliminate errors. Entry fees shall not be refunded in the event the dog is absent, disqualified, excused by the judge, or banned from competition by action of the Agility Committee. If because of riots, civil disturbances or any other acts beyond the control of the management of the trial it is impossible to open or to complete the trial, no refund of entry fees will be made. Exhibitors must be governed by any rules or regulations of civil authorities and/or building or location restrictions. Dogs must be ready for judging. CISSC will not be responsible and is not obligated to furnish any service for notifying exhibitors or their agents of class judging. Judges need not wait for any dog holding up a class. Owners or agents alone are responsible for the presence of their dogs in the judging ring. All dogs present must be held on leash or confined to their crates except when being judged or in the warm-up area. Exhibitors alone are responsible for the behavior of their dogs and/or children. Any exhibitor whose dogs and/or children create unnecessary disturbances or repeatedly engage in unsafe or disruptive behavior may, at the direction of the Agility Committee, be asked to leave the trial site. In such a case, no refund of any entry fees paid will be made.

The walk-through allows handlers to plan their strategy for running the course. A judge s briefing will be held prior to each class. A warm-up area with a regulation jump will be provided. Prizes and Awards Rosettes will be awarded to the 1st through 4th place dog in each jump height in each class, provided they have earned a qualifying score. Qualifying ribbons will be awarded to each dog earning a qualifying score. A title ribbon will be presented to each dog finishing a title. MACH and PACH bars and rosettes will be presented to each dog earning a MACH or PACH title. (MACH, MACH 2, MACH 3, etc.) One toy (value $3.00) per weekend for each dog entered. Bitches in season, aggressive, blind or deaf dogs or dogs suffering from any deformity, injury or illness which may affect the dog s physical or mental performance are ineligible to participate. Mixed breed dogs that have been listed with AKC Canine Partners participate provided their listing number has been recorded on the entry form. Obstacles All obstacles used will meet the current specifications as set forth in the AKC Agility Regulations We will be using the break away tire Exhibitor Information Exhibitors may wear shirts or insignia of their training group if they wish. Height Cards Jump Height Card Process When filling out the entry form, there is a new section at the top of the form for exhibitors to check if your dog needs to be measured. Please check the box so that the Trial Secretary knows whether to include your dog on the Dogs To Be Measured List. It is no longer required that you submit a copy of your dog s jump height card with your entry form or show it during day of show check-in.

Move-ups will also be allowed during the trial If your dog qualifies to move-up preceding the trial, you must fill out the form below and send either by regular mail or e-mail. The Trial Secretary must have the completed form by 6:00 PM on the Monday, November 10, 2014. From Chapter 1, Section 13: Dogs may be moved up to a higher class at the request of the owner as a result of qualifying for an agility title, provided that the request is made to the Trial Secretary. Entry move-ups are valid only if the dog was already entered at the event and only if approved by the Trial Secretary. Dogs having obtained their first AKC Agility Title (Std. or JWW) must be moved to the Novice B Class for all trials for which entries have not closed. Once a trial has closed its entries, a dog cannot be moved from Novice A to Novice B. Crating space will be limited so please be mindful of that when you set up your crating areas and arrange them so as to use as small a space as possible. We will be unable to allow exercise pens unless multiple dogs in an x-pen can fit in less space than they would in their crates. Also, you will need to bring a waterproof floor covering to place underneath your crates, chair and everything else you bring. CISSC is offering a Wait List to fill openings created by withdrawn entries after the trial's closing, Sat. November 1, 6:00pm A maximum of 20 entries will be placed on the club's wait list. All entries, including wait listed entries, must be received by the Trial Secretary prior to Nov,1 2014 at 6:00pm. Any entries withdrawn that are received by November 1, 2014 will be replaced by a wait listed entry if any are available. If a withdrawn entry is replaced by a wait listed entry, a full refund will be issued for the withdrawn entry. Any wait listed entry that does not move off the wait list into the trial by Monday, Nov.10, 2014 will receive a full refund. AGREEMENT I (we) agree that the club holding this event has the right to refuse entry for cause which the club shall deem sufficient. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors, governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog. Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend, and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and whether or not the same may have been caused or may be alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons. I (WE) AGREE THAT ANY CAUSE OF ACTION, CONTROVERSY OR CLAIM ARISING OUT OF OR RELATED TO THE ENTRY, EXHIBITION OR ATTENDANCE AT THE EVENT BETWEEN THE AKC AND THE EVENT-GIVING CLUB (UNLESS OTHERWISE STATED IN ITS PREMIUM LIST) AND MYSELF (OURSELVES) OR AS TO THE CONSTRUCTION, INTREPRETATION AND EFFECT OF THIS AGREEMENT SHALL BE SETTLED BY ARBITRATION PURSUANT TO THE APPLICABLE RULES OF THE AMERICAN ARBITRATION ASSOCIATION. HOWEVER, PRIOR TO ARBITRATION ALL APPLICABLE AKC BYLAWS, RULES, REGULATIONS AND PROCEDURES MUST FIRST BE FOLLOWED AS SET FORTH IN THE AKC CHARTER AND BYLAWS, RULES, REGULATIONS, PUBLISHED POLICIES AND GUIDELINES. INSTRUCTIONS Handlers may opt to enter in a higher height division for all Regular titling classes, but not in a division lower than their proper height division. Dogs entered in the Preferred classes must compete at the required jump height and may not compete in a jump height division higher or lower than their proper height division. All dogs that have not been officially measured for an Agility Jump Height Card may still compete; however, they must be measured by a Judge of record, an Agility Field Representative, or a Volunteer Measuring Official, prior to running. If measured by the Judge of record, that measurement will be valid for this trial or back-to-back trials only. If a measurement is necessary, it is the exhibitor's responsibility to have their dog(s) measured prior to running. Dogs listed in the AKC Canine Partners SM program may be eligible to enter all-breed AKC agility trials at the club s option. These dogs should be listed as All American Dog and must include their AKC number on the entry form and check the AKC No. box. Junior Handlers should enter their Junior Handler number on the front of this form. Should you not have your Junior Handler number, it may be obtained from the American Kennel Club (919) 233-9767.
