SDR Dog Adoption Application We are committed to matching you with the right dog so that all will have many happy years together. All of our rescue dogs are in private volunteer foster homes. We do not have a central shelter or kennel location. Our volunteer fosters are most qualified to answer any questions regarding the dog. To protect our foster families' privacy, we must have an application on file before we release their contact information. We may receive several applications for one dog and we do our best to keep our site up to date with available dogs. Please note that an approved application DOES NOT guarantee that the dog will be placed with you. The final decision rests with our fosters as they know the dogs the best, and please be aware that there may be other applicants interested in the same dog. Small Dog Rescue requires you to fill out an adoption application (you must be 21 years of age) and submit a (refundable) deposit of $200 in order to start the adoption process. Once we have received these, the dog you chose will be put on hold. The Adoption Manager will call you to go over the adoption process and answer questions you may have. We will then check references and conduct a home visit. Depending on your location, we may ask you to submit photos of the interior and exterior of your home in lieu of a home visit. Your deposit goes towards the adoption fee, the reminder will be due as well as the adoption contract. All dogs are altered, up to date on vaccinations, on heartworm preventative and dewormed. If we cannot alter a dog due to age a spay/neuter agreement will be required. We reserve the right to refuse an applicant based on the information provided, as well as the results of the application process, with or without disclosure of cause. In the event that we discover that any information entered on this application has been knowingly falsified we will reclaim possession of the adopted dog. We do not like to adopt out small dogs to homes with children under 5 or dogs under 10 lbs to homes with children under 8yrs. Fenced in yards are preferable. Each adoption is based on the best home for the dog. Our Adoption donations cover: vetting, transporting, feeding, and caring for all the dogs in our program. Thank you for your interest in rescue.
